The coordination of binocular eye movements: vertical and torsional alignment.

Precise binocular alignment of the visual axes is of utmost importance for good vision. The fact that so few of us ever experience diplopia is evidence of how well the oculomotor system performs this function in the face of changes due to development, disease and injury. The capacity of the oculomotor system to adapt to visual stimuli that mimic alignment deficits has been extensively explored in laboratory experiments. While the present paper reviews many of those studies, the primary focus is on issues involved in maintaining good vertical and torsional alignment in everyday viewing situatio

Spatial object representation and its use in planning eye movements.

The eye movements we make to look at objects require that the spatial information contained in the object's image on the retina be used to generate a motor command. This process is known as sensorimotor transformation and has been generally addressed using simple point targets. Here, we investigate the sensorimotor transformation involved in planning double saccade sequences directed at one or two objects. Using both visually guided saccades toward stationary objects and objects subjected to intrasaccadic displacements, and memory-guided saccades, we found that the coordinate transformations r

Occipitotemporal activity elicited by viewing eye movements: a magnetoencephalographic study.

The temporal and spatial processing of viewing eye movements was studied by magnetoencephalography (MEG) in six normal subjects. Three visual stimulus types were studied: (1) moving eyes (EYES), (2) moving simulated eyes (SIM), consisting of checks moving in the same spatial location as EYES, and (3) an inwardly moving radial pattern (RADIAL). A large clear MEG component, 1M, with mean peak latency of approximately 170 ms, was seen in the right hemisphere to RADIAL and EYES in all six subjects. The 1M to EYES was significantly longer in latency and smaller in amplitude than that seen to RADIAL

Dissociated vertical deviation (DVD): The saccadic and slow eye movements.

Dissociated vertical deviation (DVD), a vertical strabismus, is often associated with infantile esotropia, latent nystagmus and excyclo-torsion. DVD usually becomes apparent at about three years of age. The authors wished to determine whether DVD is a manifestation of an abnormal motor vergence system or is part of a visual system disorder. Vertical eye movements of six subjects with DVD, on cover-uncover and alternate cover tests, were studied using the magnetic search eye coil system. Asymmetry between the eyes' saccades were quantified during the DVD upward movement and during a trial of pu

Spinocerebellar degeneration with slow eye movements and abducens nerve palsy.

A 51-year-old woman with spinocerebellar degeneration manifested an unusual disorder of eye movements. She presented with bilateral abducens palsy and slow eye movements in the horizontal plane. Slow eye movements typically are seen with supranuclear lesions, whereas abducens palsies are of nuclear or infranuclear origin. This unique combination of eye movement disorders is discussed, as well as other features of ophthalmoplegias associated with spinocerebellar degeneration.

Oculomotor function in Wernicke-Korsakoff's syndrome: I. Saccadic eye movements.

Saccadic eye movements from three patients diagnosed as having Korsakoff's syndrome and one patient who had suffered the acute Wernicke stage of the disorder were examined for changes in saccadic characteristics. The three Korsakoff patients showed increased saccadic latencies; two of the three also had reduced saccadic peak velocities and increased saccadic durations. A higher than expected incidence of hypometric saccades was also found in all four patients; saccadic intrusions were recorded in two Korsakoff patients. These abnormal oculomotor responses were found 2-7 years after the onset o

Velocity of rapid eye movements and vertigo of central origin.

Peak velocities of voluntary saccades and fast phases of different qualities of nystagmus were studied in 20 normal subjects, 5 patients with pontine lesions and 5 patients with lesions in the frontal cortex. The peak velocity of saccades was related to the amplitude of the eye movement, the alertness of the subject and whether the subject could see the target or not. The effect of the age was not significant. The quick phases of caloric, optokinetic and angular acceleration induced nystagmus were equally rapid but somewhat slower than that of vestibular nystagmus recorded in standardized visu

The dynamics of fusional vergence eye movements in binocular dysfunction.

Two subject groups were designated as either normal or abnormal on the basis of their clinically determined vergence and heterophoria data. The question was asked, can these groups be differentiated on the basis of certain objectively measured dynamic properties of fusional vergence response? Vergence tracking rate, percentage completion of step responses, response velocity, and latency discriminated between the two experimenal groups; latency of convergence responses did not.

Rapid eye movements in myasthenia gravis. II. Electro-oculographic analysis.

Voluntary saccades were studied by electro-oculography in ten patients with myasthenia gravis (MG) and in eight patients with other types of ophthalmoplegia. Despite limited range of eye movements, maximum velocities of 20 degree and 40 degree saccades in patients with MG were not significantly different from those in normal individuals, whereas maximum velocities in patients with other types of ophthalmoplegia were significantly decreased. In some myasthenic patients, small amplitude saccades were hypermetric and had high velocities, appearing clinically as "quiver" movements characteristic o

Narcolepsy and rapid eye movement sleep.

Since the first description of narcolepsy at the end of the 19th Century, great progress has been made. The disease is nowadays distinguished as narcolepsy type 1 and type 2. In the 1960s, the discovery of rapid eye movement sleep at sleep onset led to improved understanding of core sleep-related disease symptoms of the disease (excessive daytime sleepiness with early occurrence of rapid eye movement sleep, sleep-related hallucinations, sleep paralysis, rapid eye movement parasomnia), as possible dysregulation of rapid eye movement sleep, and cataplexy resembling an intrusion of rapid eye move

Eye movement abnormalities in essential tremor.

Essential tremor (ET) is the most prevalent movement disorder, characterized mainly by an action tremor of the arms. Only a few studies published as yet have assessed oculomotor abnormalities in ET and their results are unequivocal. The aim of this study was to assess the oculomotor abnormalities in ET patients compared with the control group and to find the relationship between oculomotor abnormalities and clinical features of ET patients. We studied 50 ET patients and 42 matched by age and gender healthy controls. Saccadometer Advanced (Ober Consulting, Poland) was used to investigate reflex

Eye movement control and its disorders.

We review the supranuclear control centers and pathways leading to individual cranial nerve nuclei in the brainstem. We discuss horizontal and vertical gaze and their abnormalities, and review the cranial nerves which subserve eye movements, III, IV, VI, including their clinical testing. We highlight the sites at which these nerves are clinically affected, which often result in characteristic associated features with neurological localizing value. Differential diagnoses of cranial nerve palsies including Duane and Möbius syndromes are also described. Lastly, we discuss the nature of childhood

Eye movement abnormalities in multiple sclerosis.

Patients with multiple sclerosis commonly describe visual symptoms that result from several eye movement abnormalities that occur from disruption of critical pathways in the brainstem, cerebellum, and cerebral hemispheres. These abnormalities include internuclear ophthalmoplegia, ocular motor palsy, ocular misalignment, pathologic nystagmus, impaired saccades, saccadic intrusions, and impaired pursuit. Detailed knowledge of these problems and their neuroanatomic localization will aid the physician by guiding diagnosis and therapeutic decision making.

Therapeutic considerations for eye movement disorders.

Advances made in understanding the pathophysiology of eye movement disorders have only recently with the publication of the first well-planned studies been translated into better treatment strategies. The following chapter summarizes the pharmacological treatment options for a variety of oculomotor syndromes. Cortisone is useful, for example, for acute vestibular neuritis to improve the restitution of the labyrinthine function. For the widespread benign paroxysmal positioning nystagmus, a series of liberating movements that free the semicircular canal from the causative otoconia is now a well-

Pursuit eye movement deficits in autism.

Oculomotor studies provide a novel strategy for evaluating the functional integrity of multiple brain systems and cognitive processes in autism. The current study compared pursuit eye movements of 60 high-functioning individuals with autism and 94 intelligence quotient, age and gender matched healthy individuals using ramp and oscillating target tasks. Individuals with autism had normal pursuit latency, but reduced closed-loop pursuit gain when tracking both oscillating and ramp targets. This closed-loop deficit was similar for leftward and rightward pursuit, but the difference between individ

Neuro-ophthalmology of eye-movement disorders.

Magnetic resonance and functional brain imaging continue to expand their roles in complementing the clinical examination to localize the causes of eye-movement disorders. A retrospective review from county-based clinical practices emphasized generally benign causes of third, fourth, and sixth cranial nerve palsies in children, but a study from a tertiary referral center found a high incidence of neoplasms causing sixth-nerve palsies in children. A prospective study found that 38% of diabetic patients with third-nerve palsies had anisocoria up to 2.5 mm. An initial attempt to develop an evidenc

Eye movement patterns in hemianopic dyslexia.

Homonymous parafoveal field loss impairs reading at the visual-sensory level. To elucidate the role of parafoveal visual field in reading, reading eye movements were recorded, by means of an infra-red registration technique, in 50 patients with homonymous hemianopia and visual field sparing ranging from 1 degree to 5 degrees; for comparison, a group of 25 normal subjects was studied. The degree of reading impairment in patients was found to depend on the extent of visual field sparing. Patients with right-sided loss of parafoveal visual field were more impaired than patients with left-sided lo

Eye movement abnormalities in rod monochromacy.

Eye movements were studied quantitatively using electro-oculography in seven patients with rod monochromacy. Attention was given to eye movement abnormalities that could help to differentiate rod monochromacy from other forms of congenital nystagmus. Horizontal nystagmus in center gaze had pendular and jerk waveforms with much lower amplitude than that in patients with other forms of congenital nystagmus and visual acuity of 20/200-20/400. Slow build-up of slow component velocity over many seconds and directional asymmetry of optokinetic nystagmus (OKN) were observed during monocular optokinet

[Eye movement disorders in visual agnosia].

By the aid of photooptical methods the authors studied eye movements in 6 patients with disorders of visual gnosis due to focal lesions in the occipitallbrain lobes. The examination included studies of a fixation of immovable objects, a tracing of linear and pendulum moving objects and a voluntary change of points of fixation in space. It was demonstrated that expressed symptoms of agnosia are accompanied by defects in the organization of all forms of opticomotor activity. The authors discuss problems of the pathogenesis of oculomotor disorders in optical agnosia.
