Solitary fibrous tumors (SFTs) are rare mesenchymal neoplasms that were initially identified in the pleura. SFTs in the nasal or paranasal sinuses are especially rare. Most SFTs exhibit indolent behavior, with a low local recurrence rate. A 39-year-old man complained of bilateral nasal congestion, hyposmia, and occasional right eye tears six months prior to hospitalization. From 1965-1978 over 2,700 patients were admitted to the Massachusetts Eye and Ear Infirmary or the Massachusetts General Hospital with carcinoma of the oropharynx, larynx or nasopharynx; only 36 of these patients were less than 40 years of age. We describe the case of a 69-year-old woman who presented with a decline in vision in the left eye and was found to have a choroidal lesion with clinical and echographic features concerning for primary uveal melanoma. Systemic imaging identified numerous metastases to the liver, kidneys, paratracheal lymph nodes, lung, and brain. The hepatic lesion was biopsied, and genetic analysis identified a Val600Glu (c.1799T>A) BRAF mutation, consistent with a cutaneous primary malignancy, although no primary tumor was identified. PATIENT: A 55-year-old man was treated twice with local excision and crycoagulation for a recurrent limbal mass of the left eye. The original histologic diagnosis was squamous cell carcinoma. Three months after the last recurrence the globe was enucleated because of a spontaneous perforation at the corneoscleral limbus with iris prolapse. COMMENTS: Small and hidden from view, most primary iris cysts of the iris pigment epithelium remain clinically silent. Larger cysts may be noted on examination, but often remain difficult to visualize due to their location. By providing a high-resolution view of the iris and ciliary body, ultrasound biomicroscopy is a useful adjunct to the clinical examination in distinguishing primary cysts of the iris pigment epithelium from solid uveal neoplasms.

The prognosis following treatment of uveal melanomata depends not only on factors such as cell type and degree of mitotic activity, size and location of the tumour, and the age of the patient, but also on the method of enucleation, as surgical manipulation of the neoplasm may potentiate the shedding of malignant cells into the blood stream.
