High-resolution surface electromyographic activities of facial muscles during the six basic emotional expressions in healthy adults: a prospective observational study.

High-resolution facial surface electromyography (HR-sEMG) is suited to discriminate between different facial movements. Whether HR-sEMG also allows a discrimination among the six basic emotions of facial expression is unclear. 36 healthy participants (53% female, 18-67 years) were included for four sessions. Electromyograms were recorded from both sides of the face using a muscle-position oriented electrode application (Fridlund scheme) and by a landmark-oriented, muscle unrelated symmetrical electrode arrangement (Kuramoto scheme) simultaneously on the face. In each session, participants expr

[The age-related functional characteristics of the facial and soft palatal muscles in healthy children based on rheo- and electromyographic data].

Rheo- and electromyographies were used to examine facial and soft palate muscle functions in 120 children aged 4 to 12 years. The findings evidence that the blood flow intensity and velocity reduce with age, as does the response to the functional test; the vascular tone increases. The bioelectric activity of the above-mentioned muscles grows with age, this evidencing the development of a child's neuromuscular and vascular systems.

Botulinum toxin injections into masticatory muscles: A brief review on applications in dental occlusion management for orthognathic surgery and facial fractures.

BACKGROUND: Orthognathic and maxillofacial trauma surgeries are often challenged by postoperative instability resulting from persistent masticatory muscle forces acting on the repositioned bone segments. Botulinum toxin type A (BTX-A) has been proposed as an adjunctive intervention to transiently reduce these forces and enhance surgical stability. OBJECTIVES: The aim of this narrative review was to evaluate the adjunctive role of BTX-A in orthognathic and maxillofacial trauma surgery, with particular emphasis on its potential to reduce postoperative masticatory muscle forces, thereby enhancin

Newer Understanding of Specific Anatomic Targets in the Aging Face as Applied to Injectables: Facial Muscles--Identifying Optimal Targets for Neuromodulators.

The muscular anatomy of the face is complex. Animation patterns of facial muscles vary significantly among individuals. Activity of facial muscles determines expression and emotion and affects the eyes aperture and the amount and extent of facial rhytids. Injection of botulin toxin for facial rejuvenation has become a very popular procedure and allows the practitioner the ability to modulate facial expression and to decrease the amount of facial rhytids. A thorough knowledge of the variant facial anatomy is necessary to maximize the efficiency of botulin toxin injection. This knowledge will al

Electrical stimulation of paralyzed vibrissal muscles reduces endplate reinnervation and does not promote motor recovery after facial nerve repair in rats.

The outcome of peripheral nerve injuries requiring surgical repair is poor. Recent work has suggested that electrical stimulation (ES) of denervated muscles could be beneficial. Here we tested whether ES has a positive influence on functional recovery after injury and surgical repair of the facial nerve. Outcomes at 2 months were compared to animals receiving sham stimulation (SS). Starting on the first day after end-to-end suture (facial-facial anastomosis), electrical stimulation (square 0.1 ms pulses at 5 Hz at an ex tempore established threshold amplitude of between 3.0 and 5.0V) was deliv

[Study on the matching feature of current parameters for inducing contraction of orbicularis oris muscle by artificial facial nerve in rabbit with peripheral facial paralysis].

Objective: To explore the optimal matching of the current parameters about artificial facial nerve (AFN) for inducing contraction of the orbicularis oris muscle (OOM) in rabbit with peripheral facial paralysis. Methods: According to the combinations of different stimulus parameters, at different time points, we used AFN to induce contraction of the affected side OOM of the paralyzed rabbit in waking state. We recorded the current amplitudes of the threshold stimulation and peak stimulation under each combination, then compared the total charge of the stimulus consumption under different combin

Dual-innervated multivector muscle transfer using two superficial subslips of the serratus anterior muscle for long-standing facial paralysis.

Recent reports have described several cases of double muscle transfers to restore natural, symmetrical smiles in patients with long-standing facial paralysis. However, these complex procedures sometimes result in cheek bulkiness owing to the double muscle transfer. We present the case of a 67-year-old woman with long-standing facial paralysis, who underwent two-stage facial reanimation using two superficial subslips of the serratus anterior muscle innervated by the masseteric and contralateral facial nerves via a sural nerve graft. Each muscle subslip was transferred to the upper lip and oral

Induction of facial muscle neurotization by temporalis muscle transposition: literature review and animal model evaluation using horseradish peroxidase uptake.

OBJECTIVE: To study the concept of facial muscle reinnervation from the trigeminal pathway following facial nerve paralysis. DESIGN AND METHODS: We studied this phenomenon in an animal model using the neuronal marker, horseradish peroxidase (HRP). The temporalis transposition procedure was performed at varying intervals post facial nerve transection. To evaluate the trigeminal-facial reinnervation process and its timing, the zygomaticus major muscle was injected with HRP at varied periods after temporalis transposition, and histologic sections of the brainstem nuclei were examined for the fin

Acquired bilateral inferior oblique muscle paralysis following facial injections of botulinum toxin.

Purpose: To report a unique case of bilateral isolated inferior oblique muscle paralysis (IOP) causing torsional diplopia following facial botulinum toxin (BTX) injections. Case report: A 37-year-old female presented with acute-onset torsional diplopia, exacerbated in primary and upward gaze, and alleviated on downward gaze. Neurological evaluations, including cranial/orbital MRI and neostigmine testing, were negative. Ophthalmic examination demonstrated bilateral incyclotorsion, consistent with bilateral IOP. History revealed recent periocular BTX injections for infraorbital rhytids, and expe

Targeted chemodenervation of the posterior belly of the digastric muscle for the management of jaw discomfort in facial synkinesis.

BACKGROUND: Botulinum toxin (BT-A) chemodenervation has been proved to significantly improve the physical and psychological well-being of patients suffering from facial synkinesis. Despite this, a cohort of patients has persistent tightness and discomfort around the angle of the jaw, which may be caused by synkinesis within the posterior belly of digastric (PBD) muscle. This study was designed to evaluate the benefits of ultrasound-guided BT-A injections into the PBD. METHODS: Thirty-three patients with recalcitrant tightness and discomfort around the angle of the jaw, despite maximal facial

An Evaluation of Muscle Repair Techniques: Implications in Musculoskeletal Healing and Corollaries in Oral-Facial Clefting.

We performed an animal study to identify the techniques associated with the best muscle healing outcomes in cleft lip/palate surgery. The right triceps of thirty adult male Sprague-Dawley rats were cut and repaired by three different suture techniques: simple (n = 10), overlapping (n = 10), and splitting sutures (n = 10). Muscle tissues were isolated from 5 rats per group 1 and 8 weeks postoperation. The inflammatory response and muscle fiber healing were evaluated by hematoxylin and eosin (H&E) staining, Western blotting, immunohistochemistry for TNF-α and IL-1β, and immunofluorescence for la

The success of free gracilis muscle transfer to restore smile in patients with nonflaccid facial paralysis.

BACKGROUND: Development of synkinesis, hypertonicity, and poor smile excursion after facial nerve insult and recovery contribute to disfigurement, psychological difficulties, and an inability to convey emotion via facial expression. Despite treatment with physical therapy and chemodenervation, some patients who recover from transient flaccid facial paralysis never spontaneously regain the ability to perform a meaningful smile. METHODS: Prospective evaluation was performed on 20 patients with nonflaccid facial paralysis who underwent free gracilis muscle transfer. Patients were evaluated using

Lateral rectus muscle palsy, facial numbness and ataxia as the initial manifestation of multiple sclerosis.

Lateral rectus muscle (LRM) palsy due to a nuclear or fascicular sixth nerve lesion is rare as the presenting sign of multiple sclerosis (MS). It is more common to find this palsy in the company of other nearby cranial nerves deficits. Facial numbness in association with a LRM palsy or paresis may go unappreciated and therefore underreported. We report an unusual patient with a LRM palsy, facial anesthesia, ataxia and as the initial manifestation of MS. There was a demyelinating lesion in the pons seen on magnetic resonance imaging (MRI).

Revisional operations improve results of neurovascular free muscle transfer for treatment of facial paralysis.

BACKGROUND: Neurovascular free muscle transfer is currently the mainstay for smile reconstruction. However, problems such as excessive muscle bulk and dislocation of the transferred muscle attachment have been described. Furthermore, dynamic movements of the transferred muscle are sometimes too strong or too weak, resulting in facial asymmetry. In these cases, secondary revisional operations for the transferred muscle are required after neurovascular free muscle transfer. This report describes revisional operative procedures in detail and examines the extent of improvement of the smile by comp

Free split and segmental latissimus dorsi muscle transfer in one stage for facial reanimation.

The authors report the experience in facial reanimation using free innervated split and segmental latissimus dorsi muscle flap one-stage transfer in 86 patients with longstanding facial palsy. The segmental latissimus dorsi was taken from the distal part of the muscle so that the muscle flap had an ultra-long neurovascular pedicle of 13 to 17.5 cm in length. The muscle flap was made thinner by splitting the segmental muscle. The split segmental muscle flap was transferred to the paralyzed side of the face with its ultra-long neurovascular pedicle passing through a tunnel in the upper lip to th
