Patients' perspectives on the short- and long-term outcomes following surgery for benign parotid neoplasms.

Surgery for benign parotid gland salivary neoplasms is associated with sequelae: scar and divot defect and complications, facial nerve paresis/paralysis and Frey's syndrome. These potential sequelae and complications are discussed with all patients prior to operation. We contacted 212 patients who had undergone surgery for benign parotid disease during 1988-1997, by postal questionnaire. We enquired about their perception and recollection of the information they had received pre-operatively and to document and comment upon what problems they had experienced in the early and late post-operative

Symptomatic trigeminal neuralgia.

In four cases with trigeminal neuralgia an intracranial neoplasm was detected. In the first two cases a meningioma of the cerebellopontine angle was producing symptomatic trigeminal neuralgia almost identical to the essential one. In the other two cases with a tumor in the gasserian ganglion area the facial pain was of a more atypical nature. In all four cases associated sensory trigeminal deficits were present. The causative factors which may produce symptomatic trigeminal neuralgia are especially discussed in this paper.

Atypical Intratemporal Meningioma Masquerading as Otitis Media-Clinical and Radiological Diagnostic Dilemma.

Meningiomas are the most common intracranial extra-axial neoplasms with mostly straightforward radiological diagnosis; however, they can have unusual clinical and imaging manifestations posing diagnostic dilemma for radiologists and clinicians. The objective of this case report is to highlight the infrequent and misleading presentations of meningiomas to make an accurate diagnosis. A 65-year-old male patient presented with complaints of left recurrent otitis media and facial nerve palsy for past 6 months. On imaging, an aggressive predominantly cystic intracranial neoplasm was found in the lef

Clinical management of a patient with advanced mucosal malignant melanoma in the sinonasal area.

We describe a case of mucosal malignant melanoma in the sinonasal area of a 65-year-old woman. She presented with a history of nasal obstruction and epistaxis with subsequent tenderness, facial anesthesia involving cranial nerve V2, red eye, proptosis, diplopia, and conjunctival chemosis. Computed tomography detected a nonspecific solid mass that had involved the left maxillary sinus and surrounding tissues, with extension into the nasal cavity and invasion of the orbital floor and eye muscles. Histopathologic examination of the neoplasm revealed that it was a malignant melanoma. We performed

A rare benign parotid gland tumor: angiomyolipoma.

Angiomyolipomas are benign neoplasms composed of smooth muscle, vasculature, and mature adipose tissue, which most commonly occur in the kidney and located in the head and neck region. A very rare neoplasm, there are only 3 cases of angiomyolipoma in the parotid gland that have been reported to date. Here, we report a case of a 38-year-old man who had a slow-growing mass in the parotid gland for the past 7 years. The results of a physical examination revealed a rubbery mass that was 2.5 cm in diameter in the below superficial lobe of the left parotid gland. A computed tomographic scan showed a

Promising Technique for Facial Nerve Reconstruction in Extended Parotidectomy.

INTRODUCTION: Malignant tumors of the parotid gland account scarcely for 5% of all head and neck tumors. Most of these neoplasms have a high tendency for recurrence, local infiltration, perineural extension, and metastasis. Although uncommon, these malignant tumors require complex surgical treatment sometimes involving a total parotidectomy including a complete facial nerve resection. Severe functional and aesthetic facial defects are the result of a complete sacrifice or injury to isolated branches becoming an uncomfortable distress for patients and a major challenge for reconstructive surgeo

Solitary fibrous tumor of parotid gland: a case report and short review of literature.

OBJECTIVE: This case report represents a solitary fibrous tumor, which is a very rare neoplasm in the parotid gland. CASE PRESENTATION: 80-year-old Caucasian female patient with palpable, immobile, painless, slow growing, semi-rigid-elastic neoplasm in the left parotid gland, that was existing for four months. There were no signs of inflammation and facial paralysis. The tumor was initially interpreted as a salivary gland neoplasm of uncertain origin. Fine needle aspiration was performed 2 times; however, the precise diagnosis was challenging. The tumor was excised, regional lymphadenectomy w

Composite tumors associating trichoblastoma and benign epidermal/follicular neoplasm: another proof of the follicular nature of inverted follicular keratosis.

BACKGROUND: Trichoblastomas (TBs) are benign follicular tumors that can present either as solitary nodule or as multiple facial papules. Only one case of sporadic TB in a composite tumor has been reported, arising in an apocrine poroma. We report the association of TB and benign epidermal/follicular neoplasm in six patients. METHODS: Clinical data available were collected. Serial sections and immunohistochemical (IHC) analysis for CK17 and human papilloma virus (HPV) were performed on available materials. RESULTS: Five of the six patients were males. Mean age at diagnostic was 56 years. Lesi

Anterior facial vein and submandibular gland together: predicting the histology of submandibular masses with CT or MR imaging.

PURPOSE: To test the hypothesis that the relationship of the anterior facial vein to the submandibular salivary gland and a mass in the submandibular fossa could help identify the origin of the mass at computed tomography (CT) or magnetic resonance (MR) imaging. MATERIALS AND METHODS: Thirteen patients with 14 palpable submandibular masses, surgical and histologic confirmation, and contrast material-enhanced CT images (12 patients) or MR images (one patient) that showed the anterior facial vein were identified with review of files from 1985 to 1997. Six patients had submandibular gland diseas

Review of accessory parotid gland tumors: pathologic incidence and surgical management.

OBJECTIVE: The current study presents our experience with accessory parotid gland masses and reviews the literature on accessory parotid tumor incidence and surgical management. Based on our results, we advocate a standard parotidectomy approach with routine facial nerve dissection at the time of excision. STUDY DESIGN: We performed a retrospective chart review and comprehensive literature review on incidence of accessory parotid gland neoplasms. METHODS: A retrospective chart review of all patients with mid-cheek masses treated by the senior author was conducted from January 2003 to January

Fine needle aspiration diagnosis of carotid body tumor in a case of multiple paragangliomas presenting with facial palsy: a case report.

BACKGROUND: Carotid body tumors (CBTs) constitute the most common extraadrenal paragangliomas. Many lesions diagnosed as CBTs by fine needle aspiration (FNA) cytology are clinically unsuspected. The main differential diagnosis is a thyroid neoplasm. The location of the mass in the lateral neck with prolonged history, hemorrhagic FNA and cytologic features resembling the endocrine neoplasm help in arriving at a suggestive diagnosis of paraganglioma. CASE: A 32-year-old male presented with left-sided facial palsy and swelling in the left side of the neck of 8 months' duration. The FNA sample wa

Sinonasal undifferentiated carcinoma: a distinctive clinicopathologic entity.

The sinonasal undifferentiated carcinoma (SNUC) is an aggressive and rare neoplasm arising in the nasal cavity and the paranasal sinuses. To date, over 50 cases of histologically proven SNUCs have been reported since its original description in 1986. Presenting symptoms include facial pain, nasal obstruction, diplopia, epistaxis, proptosis, and periorbital swelling. The histologic features of this neoplasm include cohesive cells arranged in nests, ribbons, and trabeculae. The cells exhibit hyperchromatic nuclei and a high nuclear to cytoplasmic ratio. A brisk mitotic rate, tumor necrosis, and

Successful treatment with concurrent chemoradiotherapy followed by surgery for a patient with thymic adenocarcinoma.

Most neoplasms arising from the thymic epithelium are considered to be 'thymomas', which are composed of cytologically benign, neoplastic epithelial cells and nonneoplastic lymphocytes. In contrast, thymic epithelial neoplasms displaying cytologically malignant features have recently been classified as thymic carcinomas of various types of histology. However, primary thymic adenocarcinoma is extremely rare and only four cases of it have been reported in the literature. We report a rare case of primary thymic adenocarcinoma of 4-year complete remission with concurrent chemoradiotherapy followed

Rhinofacial Conidiobolomycosis - A rare fungal infection of tropics.

Fungal sinusitis may be caused by filamentous fungi such as mucorales, aspergillus and entomophthorales. Mucormycosis and aspergillosis have immunocompromised states as specific risk factors, whereas entomophthorales may occur in apparently healthy persons having significant soil contact. This is, nonetheless, a rare condition with involvement of mucosa of the nose, para nasal sinuses and centrofacial soft tissues, without bony or angioinvasion. It grows relentlessly, however, and may mimic soft tissue neoplasm causing facial disfigurement.

Solitary metastasis of lung carcinoma to the internal auditory canal.

We report a case of solitary metastasis of lung carcinoma into the internal auditory canal. On admission, the 60-year-old patient was complaining about a rapidly developing hearing loss with ipsilateral facial nerve paralysis. Magnetic resonance imaging revealed an intracanalicular tumor, which was interpreted as vestibular schwannoma. After tumor resection, the histopathologic examination revealed a metastasis of a squamous cell carcinoma. Computed tomographic scan and bronchoscopy identified an asymptomatic primary pulmonary neoplasm.

Gamma knife radiosurgery in the treatment of tumor-related facial pain.

BACKGROUND: Intracranial neoplasms can cause pain similar to trigeminal neuralgia. Literature regarding radiosurgery for this is limited. We present a retrospective review of patients with tumor-related facial pain from benign lesions treated with gamma knife radiosurgery (GKRS) at Wake Forest University. OBJECTIVES: The primary objectives were to determine long-term pain relief and predictive factors for pain alleviation. METHODS: We reviewed 515 patients treated with GKRS for benign meningioma, vestibular schwannoma or trigeminal schwannoma between August 1999 and August 2010. Twenty-one e

Acoustic neurinomas associated with vascular compression syndromes.

Trigeminal neuralgia and hemifacial spasm are caused by vascular compression of the cranial nerves at the brainstem in the majority of cases. Trigeminal neuralgia occurring in 3.3% of acoustic neurinomas is usually assumed to be a sign of large tumour size; if associated with small tumour size, an additional pathology, such as typical vascular compression must be suspected and has to be explored at surgery. While facial paresis will usually lead to immediate radiological diagnosis of a possible cerebellopontine angle (CPA) neoplasm, facial spasm is usually not expected to be associated with a

[Complications in the surgery of the parotid salivary glands. Causes, mechanisms of development, methods of prevention].

Operations for neoplasms of the parotid salivary gland require from the surgeon a deep knowledge of the anatomical and topographic features of this area and technical skill, which puts it in a number of «jewelry» operations that require patience and concentration. The review is devoted to the topic of specific postoperative complications after parotidectomy or subtotal resection of the gland. These include relapses of tumors, paralysis or paresis of facial muscles, Frey syndrome, salivary fistulas or sialocele, violation of skin sensitivity, soft tissue deformity. The review presents the frequ
