Intratemporal schwannoma of the facial nerve.

Two patients with schwannomas that originated from the region of the geniculate ganglion of the facial nerve are reported. They presented with progressive facial nerve paralysis and variable disturbance of hearing. Computed tomograms demonstrated a mass lesion eroding the temporal bone (petrous portion) and extending into the middle fossa. The treatment consisted of complete excision of the tumor, and a hypoglossal-facial nerve anastomosis. Computed tomography should be performed on all patients with facial palsy of a progressive nature. Early diagnosis and surgical treatment of facial schwann

Secondary syphilis of the facial nerve.

A 37-year-old woman had peripheral facial palsy. It took nearly two weeks to establish the syphilitic origin. This rare disease is discussed with reference to nine cases reported in the literature since 1945. The prognosis is good with early antibiotic therapy.

Aberrant course of the facial nerve.

In our patient the facial nerve was situated in the external bony ear canal without bony protection just below the skin in the lower part of the external ear canal 2 to 3 mm lateral to the tympanic membrane. This abnormal position involves a greate risk of damage to the facial nerve by incisions in the ear canal prior to surgical intervention of the middle ear.

Traumatic facial nerve neuroma with facial palsy presenting in infancy.

OBJECTIVE: To describe the management of traumatic neuroma of the facial nerve in a child and literature review. PATIENT: Sixteen-month-old male subject. INTERVENTION: Radiological imaging and surgery. MAIN OUTCOME MEASURES: Facial nerve function. RESULTS: The patient presented at 16 months with a right facial palsy and was found to have a right facial nerve traumatic neuroma. A transmastoid, middle fossa resection of the right facial nerve lesion was undertaken with a successful facial nerve-to-hypoglossal nerve anastomosis. The facial palsy improved postoperatively. CONCLUSION: A trauma

Total facial nerve decompression for recurrent facial paralysis: an update.

Recurrent facial paralysis (RFP) is a rare disorder that in some individuals may lead to worsening sequelae. Melkersson-Rosenthal syndrome is a variant of RFP that is associated with recurrent facial edema. In the past, decompression of the mastoid segment of the facial nerve has not been successful in preventing recurrences. In 1981 we began performing total facial nerve decompression for RFP and in 1986 reported its efficacy in one patient with Melkersson-Rosenthal syndrome and in another in whom both nerves were decompressed for alternating bilateral paralysis. An additional four cases with

The Facial Nerve: Anatomy and Pathology.

The facial nerve is the seventh cranial nerve and consists of motor, parasympathetic and sensory branches, which arise from the brainstem through 3 different nuclei (1). After leaving the brainstem, the facial nerve divides into 5 intracranial segments (cisternal, canalicular, labyrinthine, tympanic, and mastoid) and continues as the intraparotid extracranial segment (2). A wide variety of pathologies, including congenital abnormalities, traumatic disorders, infectious and inflammatory disease, and neoplastic conditions, can affect the facial nerve along its pathway and lead to weakness or p

Microsurgical anatomy of the facial nerve.

The facial nerve connections and pathways from the cortex to the brainstem are intricate and complicated. The extra-axial part of the facial nerve leaves the lateral part of the pontomedullary sulcus and enters the temporal bone through the internal acoustic meatus. In the temporal bone, the facial nerve branches into fibers innervating the glands and tongue. After it emerges from the temporal bone it supplies various facial muscles. It contains a motor, general sensory, special sensory, and autonomic components. The physician needs comprehensive knowledge of the anatomy and courses of the fac

Primary tumors of the facial nerve.

This article describes the pathophysiology, diagnosis, treatment, and outcomes of primary tumors of the facial nerve. These tumors include facial nerve schwannomas, geniculate ganglion hemangiomas, glomus facialis, and granular cell tumors. Although these tumors are rare, collected data help to form a consensus with regard to optimal treatment methods.

A review of facial nerve anatomy.

An intimate knowledge of facial nerve anatomy is critical to avoid its inadvertent injury during rhytidectomy, parotidectomy, maxillofacial fracture reduction, and almost any surgery of the head and neck. Injury to the frontal and marginal mandibular branches of the facial nerve in particular can lead to obvious clinical deficits, and areas where these nerves are particularly susceptible to injury have been designated danger zones by previous authors. Assessment of facial nerve function is not limited to its extratemporal anatomy, however, as many clinical deficits originate within its intrate

Laser decompression of the facial nerve.

Laser decompression of the facial nerve is an experimental approach to evaluating the parameters for laser usage at a nerve-bone interface. This article summarizes the anatomic considerations and indications for facial nerve decompression, and the advantages and disadvantages of laser decompression of the facial nerve. Preliminary studies using the Vanderbilt free electron laser are described, and future directions for research are outlined.

[Asymmetry of the human facial nerve].

Measurements of the right and left facial skull were carried out in 6 different series of skulls: two europeoid, Russian and Georgian; two mongoloid, Buryat and Eskimo; and two skulls of mixed origin, Khakass and Udmurt ones, in order to assess the bilateral asymmetry. The measurements included 18 signs characterizing the width, height, and length of the facial skull. Sums of all right-side and left-side signs were the integral parameters of the right and left halves of the facial skull. A compensatory type of asymmetry variations was revealed. All the components of the facial skull regarded i

Results of reconstruction of the facial nerve.

The results of surgical treatment for facial paralysis are still difficult to compare as there is no universal scoring method. The purpose of this communication is to review the results of reconstruction of the extratemporal facial nerve with nerve grafts using our own evaluation system. Fifty-nine patients were operated on during the period 1981-1991. They had lacerations or other trauma to the face which resulted in loss of continuity of the facial nerve or branches. Three groups were considered: (a) 12 patients had injuries involving the trunk and extending up to the main branches of the fa

[Magnetic stimulation of the facial nerve].

Intracranial activation of the facial nerve and the face-associated motor cortex are now possible with noninvasive magnetic stimulation techniques. Compound muscle action potentials (CMAPs) and the Blink reflex, in response to magnetic stimulation, were investigated. Subjects were 10 normal controls and 2 Bell's palsy patients. CMAPs were elicited in the orbicularis oris muscle by magnetic stimulation at the parieto-occipital skull and stylomastoid foramen. Furthermore, CMAPs were evoked by a magnetic coil oriented over the cortex. CMAP recording was possible with magnetic stimulation and the

[Regeneration of the severed facial nerve].

Twenty-four specimens from distal end of the human facial nerve obtained at reinnervation surgery (nerve grafting) were studied by light and electron microscopies. In 21 specimens, with complete severance of the facial nerve but continuous neurilemma confirmed during surgery, regeneration phenomenon was observed. Histological examination of the auricular branches of vagus nerve (2 cases) and auriculotemporal nerve (1 case) with their communication portion showed regeneration of axons into the facial nerve below the level of sectioning. This phenomenon points to the possibility that surgically
