OBJECTIVE: The authors presented the short term and long term effects of the trigeminal neuralgia treated by RFT with anterior-lateral facial, lateral facial and trans-subzygomatic approaches.
METHODS: 320 primary patients with trigeminal neuralgia were selected for this study. All these patients were followed up with the period of 1 to 6 years.
RESULTS: The first-time effective ratios of pain-relieve treated by using the anterior-lateral facial approach, and the lateral facial and the trans-subzygomatic, were 93.3% and 73.3% respectively. Most of the patients (92.5%) achieved excellent or g

BACKGROUND: Trigeminal neuralgia is a common neuropathic pain of the oral and maxillofacial region. P2X7 receptor, a neural mediator, P2X7 receptor is closely related to this condition. Despite several studies on the role of Jujuboside A in sleep, few reports are available on its effects on chronic neuropathic pain.
METHODS: In this study, a rat trigeminal neuralgia model was established by chronic constriction injury of the infraorbital nerve. Rats were intervened with Jujuboside A and P2X7 shRNA. For the behavioral aspect, the changes in the facial mechanical pain withdrawal threshold of th

OBJECTIVES: Pharmacotherapy of chronic orofacial pain is unsatisfactory. Here we set out to prepare a systematic review of randomized controlled clinical trials (RCTs) on pharmacotherapy of facial pain.
METHODS: The diagnostic groups "temporomandibular disorders" (TMDs), "atypical facial pain", and "trigeminal neuralgia" were included. RCTs published between 1966 and August 2001 were identified by Medline search, from review articles, and from the Cochrane and Bandolier databases. The quality of the trials was judged according to established criteria. Good or excellent pain reduction or >50%

BACKGROUND: Stereotactic radiosurgery (SRS) is commonly used to treat medically intractable trigeminal neuralgia. The target is either a point on the trigeminal nerve (isocentric treatment) or a finite length of the nerve (non-isocentric treatment). The superiority of one approach over the other is not well established. We compare our treatment outcomes for the two approaches in terms of pain relief and the most frequent side effect, facial numbness.
METHODOLOGY: A total of 83 consecutive patients (41 isocentric, 42 non-isocentric) were treated for trigeminal neuralgia with CyberKnife. Pain r

OBJECTIVE: To determine the effect of dalfampridine (4-aminopyridine), a broad-spectrum, voltage-dependent potassium channel blocker, on patients with trigeminal nerve dysfunction due to multiple sclerosis (MS).
METHODS: We reviewed histories of 71 patients in our clinic with clinically definite MS who were treated with dalfampridine for at least 2 to 3 months. Of the 71 patients, 5 had a history of either trigeminal neuralgia or altered facial sensation.
RESULTS: Of these 5 patients, 3 with preexisting trigeminal neuralgia had a marked worsening of facial pain in close proximity to starting

A 60-year-old man presented to the ER with lancinating right-sided facial pain following a URI. He was diagnosed with trigeminal neuralgia, yet returned several days later with right-sided facial paralysis. He was then also diagnosed with Bell's palsy and treated with corticosteroids and antiviral medication. He continued to experience facial pain and paralysis, and sought out integrative medical care including acupuncture, nutritional supplements, diet and lifestyle changes, and parenteral therapy with intravenous vitamins and minerals. Eight weeks later he reported near complete resolution o

Atypical facial pain or neuralgia and lower-half headache are confusing terms and should be discarded. Recurrent unilateral, throbbing, frontal headaches should be referred to as facial migraine. Patients whose trigeminal branches have been subjected to repeated surgical procedures and who have relentless unilateral face-jaw pain should be classified as having chronic traumatic trigeminal neuralgia. Effective treatment is available provided surgical manipulations cease.

Trigeminal neuralgia secondary to vertebrobasilar dolichoectasia and basilar artery dissection is rare. The authors report the case of a 72-year-old man with a 5-year history of right electrical facial pain identical with trigeminal neuralgia. Finally, magnetic resonance imaging and digital subtraction angiography revealed basilar artery dissection and vertebrobasilar dolichoectasia. The patient underwent partial basilar dissecting aneurysm embolization. The facial pain was relieved immediately after the operation and disappeared completely 6 months later. Three years after surgery, the patien

The paroxysmal facial pain of trigeminal neuralgia is usually idiopathic, but familial cases have been described. We describe a family with apparent autosomal dominant transmission of trigeminal neuralgia. Our cases and a review of the literature suggest that the etiology of trigeminal neuralgia may be vascular compression of the fifth cranial nerve. Autosomal dominant vascular and epileptic disorders are reviewed, and possible relationships to familial trigeminal neuralgia are considered.

Trigeminal neuralgia is characterized by paroxysmal episodes of facial pain in the distribution of the trigeminal nerve. It can occur in either sex but is most frequently seen in women aged 50-60 years. This article presents case of trigeminal neuralgia in a 55-year-old female patient caused by meningioma of petroclival localization. The article underscores the importance of neurologic and neuroradiologic examination in diagnosing and managing patients with facial pain conditions.

Facial pain is a common presenting complaint requiring patience and diagnostic acumen. The proliferation of eponyms attached to various syndromes complicates the subject. The most frequent cause of pain is likely to be muscle spasm in masticatory or temporalis muscles. This article presents a rank order for the common causes of facial pain that present diagnostic difficulty, such as temporomandibular joint pain, trigeminal neuralgia, giant cell arteritis, and post-herpetic neuralgia.

To the otolaryngologist, there are multiple causes for head or facial pain: headaches due to ear diseases; idiopathic neuralgias; "referred otalgia" involving cranial nerves V, IX, X; temporomandibular joint dysfunction; rhinological pathologies, including post-traumatic trigeminal neuralgia; "facial sympathalgias"; the styloid process syndrome; and cervical spine problems. Less known causes of head and neck pain are stressed, and emphasis placed on their diagnosis and treatment.

INTRODUCTION: Painful tic convulsif syndrome is ipsilateral facial trigeminal neuralgia combined with hemifacial spasm, which is relatively rare in the clinic. Microvascular decompression is currently considered to be an effective treatment. We report extracranial radiofrequency treatment of painful tic convulsif syndrome under local anesthesia, a technique which provides a safer and more economical treatment for this kind of patient.
CASE PRESENTATION: We report a case of painful tic convulsif syndrome which was treated with extracranial radiofrequency therapy of the trigeminal nerve and fac

OBJECTIVE: This study aimed to investigate the specific manifestations and differences in brain network activity and functional connectivity between brain networks in patients with trigeminal neuralgia and migraine, aiming to reveal the neural basis of these two diseases.
BACKGROUND: Head and facial pain, including trigeminal neuralgia and migraine, is prevalent globally. However, the underlying neural mechanisms of these conditions remain unclear. Resting-state functional connectivity studies based on independent component analysis (ICA) may offer new insights into these diseases.

OBJECTIVES: Patients with facial pain are sometimes referred for neurosurgical treatment with a poorly documented diagnosis of trigeminal neuralgia. In such cases, neurosurgery will usually not be helpful. We conducted a re-evaluation of the diagnosis in patients referred for neurosurgical treatment of presumed trigeminal neuralgia. Our objective was to find out why and how often misdiagnosis occurred.
METHODS: A retrospective study was done in consecutive first-time patients referred for trigeminal neuralgia to our outpatient clinic in 2019. We used five ICHD-3-based clinical criteria to ver

Ongoing pain, intermittent sharp pain, or intermittent dull aching pain around the teeth can evoke the suspicion of tooth pathology. However, when no dental cause can be found clinically or radiographically, the differential diagnosis involving neuropathic pain and pulpal pathology is still a challenge. Neuropathic facial pains are still too often misdiagnosed as tooth pain of dental origin, resulting in unnecessary dental extraction or endodontic therapy. The purpose of this study was to determine if electronic thermography was able to differentiate neuropathic facial pains presenting as toot

Trigeminal neuralgia is a known symptom of the tumors and aberrant vessels near the trigeminal nerve and the tentorial notch. There are very few reports of delayed development of trigeminal neuralgia after radiosurgical treatment of a tumor in these areas. This is a case report of a patient treated with radiosurgery for radiation induced meningiomas, 30 years after childhood whole brain radiation. The largest tumor was adjacent to the pons and left trigeminal nerve but did not cause any direct neurologic symptoms or facial pain. Nine months after radiosurgical treatment of the tumors, the pati

Trigeminal-mediated headshaking is an idiopathic neuropathic facial pain syndrome in horses. There are clinical similarities to trigeminal neuralgia, a neuropathic facial pain syndrome in man, which is usually caused by demyelination of trigeminal sensory fibers within either the nerve root or, less commonly, the brainstem. Our hypothesis was that the neuropathological substrate of headshaking in horses is similar to that of trigeminal neuralgia in man. Trigeminal nerves, nerve roots, ganglia, infraorbital, and caudal nasal nerves from horse abattoir specimens and from horses euthanized due to

Symptomatic trigeminal neuralgia due to a brainstem infarction is said to be rare. However, facial pain is not uncommon in Wallenberg's syndrome. Facial pain related to a Wallenberg's syndrome may be either persistent of intermittent, and occasionally occurs in brief attacks. Here, we report a patient with a right lateral medullary infarction who started having first division trigeminal neuralgia 1 month after the stroke. The pain paroxysms were suppressed with gabapentin.
