Suicidal ideation among patients with facial pain disorders.

INTRODUCTION: Facial pain can be severely debilitating. Although etiologies vary, facial pain commonly results in diminished quality of life and a pronounced psychosocial burden. OBJECTIVES: To determine whether severe facial pain is associated with a heightened risk of suicidal ideation. METHODS: We performed a retrospective review of patients with facial pain disorders who, during a presurgical evaluation, completed the Columbia-Suicide Severity Rating Scale and Brief Pain Inventory-Facial (BPI-Facial) scale, which assess suicidal risk and the effect of facial pain on quality of life, resp

Alcohol-Induced Disinhibition of Facial Responses to Pain.

BACKGROUND: Alcohol in sub-toxic dosages has appeared to slightly reduce experimental pain in psychophysical paradigms. However, this effect may also reflect impaired scaling performance in subjective ratings. To address this, we additionally assessed facial responses as a more direct and cognitively unbiased pain measure, while acknowledging the potential confound of alcohol's effects on motor inhibitory function. METHODS: We investigated 41 healthy participants (22 females) in a randomised, double-blind, and placebo-controlled design; targeting two moderate breath-alcohol levels (0.6‰, 0.8‰

Anorexia Nervosa Dampens Subjective and Facial Pain Responsiveness.

Background/Objectives: Individuals with anorexia nervosa (AN) are known to exhibit both reduced pain sensitivity-when assessed via thresholds and subjective ratings-and diminished facial expressions of emotion. Therefore, investigating the facial response to pain in this population is of particular interest. Method: Seventeen patients with AN and 18 age- and sex-matched healthy controls were assessed using a thermode to induce heat pain. Subjective pain measures included pain threshold, pain tolerance, and pain ratings of supra-threshold stimuli, rated on a numerical rating scale (NRS). Facial

Interventional Treatment Strategies for Headache and Facial Pain.

This article reviews interventional treatments for headache and facial pain. Procedures, such as botulinum toxin injections, occipital and peripheral nerve blocks, trigger point injections, and sphenopalatine ganglion blocks, are discussed with protocols and evidence supporting their efficacy. These approaches serve both therapeutic and diagnostic purposes, particularly in cases refractory to medication or conservative management. Emphasis is placed on anatomic knowledge and careful pre-procedural considerations to enhance precision and safety, with procedural guidance provided for reducing ri

Thalamic changes in patients with chronic facial pain.

PURPOSE: To investigate structural alterations in the thalamus in patients with primary trigeminal neuralgia and provide a detailed perspective on thalamic remodeling in response to chronic pain at the level of individual thalamic nuclei.  METHODS: We analyzed a sample of 62 patients with primary trigeminal neuralgia who underwent surgical treatment, along with 28 healthy participants. Magnetic resonance imaging (MRI) data were acquired using a 3T system equipped with a 16-channel receiver head coil. Segmentation of the thalamic nuclei was performed using FreeSurfer 7.2.0. We divided the group

Neuralgia and Atypical Facial, Ear, and Head Pain.

Though there have been considerable strides in the diagnosis and care of orofacial pain disorders, facial neuralgias, and myofascial pain dysfunction syndrome remain incredibly cumbersome for patients and difficult to manage for providers. Cranial neuralgias, myofascial pain syndromes, temporomandibular dysfunction (TMD), dental pain, tumors, neurovascular pain, and psychiatric diseases can all present with similar symptoms. As a result, a patient's quest for the treatment of their orofacial pain often begins on the wrong foot, with a misdiagnosis or unnecessary procedure, which makes it all t

Imaging of Trigeminal Neuralgia and Other Facial Pain.

We review and illustrate the radiology of facial pain, emphasizing trigeminal neuralgia, relevant anatomy, current classification, concepts about etiology, and the role of imaging and its influence on the choice of treatment. We discuss glossopharyngeal neuralgia, other neuropathic causes of facial pain, postinflammatory and neoplastic causes, and nociceptive (end-organ) causes of facial pain, as well as referred otalgia. Other conditions that may present with facial pain, including trigeminal autonomic cephalgias and giant cell arteritis, are reviewed briefly. We discuss the elements of a com

Case Series: Deep Brain Stimulation for Facial Pain.

BACKGROUND: Deep brain stimulation (DBS) has been used for chronic pain for decades, but its use is limited due to a lack of reliable data about its efficacy for specific indications. OBJECTIVE: To report on 9 patients who underwent DBS for facial pain, with a focus on differences in outcomes between distinct etiologies. METHODS: We retrospectively reviewed 9 patients with facial pain who were treated with DBS of the ventral posteromedial nucleus of the thalamus and periventricular gray. We report on characteristics including facial pain etiology, complications, changes in pain scores using

Deep brain stimulation for intractable neuropathic facial pain.

OBJECTIVE Deep brain stimulation (DBS) is a well-established, evidence-based therapy with FDA approval for Parkinson's disease and essential tremor. Despite the early successful use of DBS to target the sensory thalamus for intractable facial pain, subsequent studies pursuing various chronic pain syndromes reported variable efficacy, keeping DBS for pain as an investigational and "off-label" use. The authors report promising results for a contemporary series of patients with intractable facial pain who were treated with DBS. METHODS Pain outcomes for 7 consecutive patients with unilateral, int

Aspergillus Mycetoma Causing Epiphora and Ipsilateral Facial Pain.

Tearing is a frequently encountered chief complaint in an ophthalmologist's office. Certain associated atypical symptoms may warrant further workup. The authors present a case of a patient presenting with painful tearing which elicited further evaluation with CT imaging. This revealed a maxillary sinus fungus ball as the cause for the patient's tearing.

Psychological Reactions and Persistent Facial Pain following Enucleation.

Background. Enucleation is a psychologically and physically traumatic event associated with chronic pain. It would be desirable to better predict which patients will have pain after surgery. Methods. A cross-sectional postal questionnaire study of adults undergoing enucleation captured the demographic details, Pain Quality Assessment Scale (PQAS), Pain Catastrophizing Scale (PCS), and the Facial Pain Assessment questionnaire. Patients were classified as suffering from chronic pain if they reported a pain score of >1 out of 10 on the numerical pain score (NRS). Results. Seventeen of 60 adults p

Parry Romberg syndrome associated with chronic facial pain.

Parry Romberg Syndrome (PRS) is a rare condition of unknown cause and pathophysiology. It is characterized by progressive facial hemiatrophy, and neurological abnormalities are found in 20% of cases. We describe a 50-year-old woman with PRS and severe neurological involvement (lateralised epileptic seizure activity and facial pain refractory to medication). Pain intensity and frequency was reduced and control of epileptic crises was improved using levetiracetam as an additional therapy. In previous published cases associated with facial pain, the most frequent diagnoses were migraine and trige

Headache and facial pain: differential diagnosis and treatment.

Headaches affect 90% of the population sometime during their life. Most are benign and fleeting, some are serious and life-threatening, and others require ongoing medical consultation and treatment. A careful history and physical is necessary to establish a differential diagnosis and to guide the choice of testing to make an accurate diagnosis. The most common types of headaches are discussed in this review. They are divided into primary and secondary headache disorders as classified by the International Headache Society. Primary headache disorders include migraine without and with aura, clust

Temporomandibular disorders and facial pain: a psychophysiological perspective.

This article presents a psychophysiological perspective on temporomandibular muscle and joint disorders (TMJD) and facial pain. After a brief introduction to TMJD, the article presents data, largely derived from work carried out in my laboratory, that address four questions: (1) What are the consequences of parafunctional activities? (2) Do TMJD patients engage in parafunctional activities? (3) Why are TMJD patients unaware of these activities? and (4) What are the implications of these findings for treatment? The findings suggest that low-level parafunctions increase pain in otherwise pain-fr

Facial pain of cardiac origin: a case report.

CONTEXT: Cardiac pain may radiate to the face and lead patients to seek dental care. Dentists may contribute towards the diagnosing of ischemic heart disease and thus refer patients for cardiological evaluation. CASE REPORT: A 50-year-old female patient was referred to a dentist for evaluation of a suspected temporomandibular disorder after repeated visits to medical emergency departments due to excruciating facial and left temporal pain associated with exertion. The pain would start in the chest and radiate to the neck, face and left temporal region. The patient's chief complaint was the fac
