Preoperative Ultrasound-Guided Wire Localization of the Lateral Femoral Cutaneous Nerve.

BACKGROUND: Difficulty and sometimes inability to find the lateral femoral cutaneous nerve (LFCN) intraoperatively is well known. Variabilities in the course of the nerve are well documented in the literature. In a previous paper, we defined a tight fascial canal that completely surrounds the LFCN in the proximal thigh. These 2 factors sometimes render finding the nerve intraoperatively, to treat meralgia paresthetica, very challenging. OBJECTIVE: To explore the use of preoperative ultrasound to minimize operative time and eliminate situations in which the nerve is not found. METHODS: Since

Meralgia Paresthetica.

PURPOSE OF REVIEW: This review article summaries the epidemiology, etiology, clinical presentations, and latest treatment modalities of meralgia paresthetica, including the latest data about peripheral and spinal cord stimulation therapy. Meralgia paresthetica (MP) causes burning, stinging, or numbness in the anterolateral part of the thigh, usually due to compression of the lateral femoral cutaneous nerve (LFCN). RECENT FINDINGS: There are emerging data regarding the benefit of interventional pain procedures, including steroid injection and radiofrequency ablation, and other interventions in

[Paresis of the femoral nerve in pelvic extraperitoneal hematoma--case reports and literature review].

The authors describe two rare cases of femoral nerve paresis in pelvic extraperitoneal hematoma in children. The first case was a 13-year-old girl, with no trauma in her medical history, in whom the paresis was suspected to have inflammatory or tumorous etiology; the other case was a 14-year-old boy injured during playing football. In both patients, extraperitoneal pathology of the pelvis was evident clinically and was shown in CT scans; paresis of the femoral nerve was demonstrated by EMG. In both cases, surgical intervention revealed a hematoma causing neuropathy. This was removed and, withi

[Pregnancy-associated femoral nerve affection].

BACKGROUND: Common neuropathies during pregnancy are carpal tunnel syndrome or peripheral facial paralysis. However, there is little information about femoral nerve affection during pregnancy. CASE REPORT: We report on a female at 30 weeks of gestation, complaining of pain in hips and thighs and gait difficulty. Pregnancy was normally developed. Neurological examination showed a bilateral weakness in both quadriceps and iliopsoas muscles combined with soreness in hips and thighs, without deficits of sensibility. Laboratory findings and results of a lumbar punction were normal. Pelvic ultrasou

Neurostenalgia of the femoral nerve: a treatable cause of intractable hip pain in a young adult.

Neurostenalgia of the femoral nerve is a compressive neuropathy that can result in debilitating pain. It often presents as hip pain, may coexist with other pathologic condition, and may be missed. Diagnosis and treatment affords great relief of symptoms. We present a patient with hip pain that persisted for 10 years, despite multiple orthopedic interventions including pelvic osteotomy and arthroplasty. Femoral neurostenalgia was eventually identified, and pain was relieved after decompression of the nerve.

A Case Report of Neuropathic Pain and Unilateral Lower Extremity Weakness Following a Cardiac Arrest.

Femoral nerve palsy is a rare, but significant complication following percutaneous coronary intervention (PCI) for conditions such as myocardial infarction. We present a case of a 61-year-old male patient who presented for cardiac rehabilitation following an emergent PCI procedure for cardiac arrest secondary to ST-elevation myocardial infarction. He later developed right lower extremity weakness and severe neuropathic pain on arrival to the acute rehabilitation unit. After physical examination and electrodiagnostic studies, he was determined to have a right femoral nerve neuropathy. This case

Postpartum Sciatic Neuropathy After Uncomplicated Vaginal Delivery.

Postpartum neuropathies are common, including femoral neuropathy, peroneal neuropathy, lumbosacral trunk plexopathy, and lateral femoral cutaneous neuropathy. Sciatic mononeuropathy in the peripartum period is rare. Postpartum sciatic neuropathy (PSN) in the setting of cesarean section has been reported before. We present a case series of 2 sciatic mononeuropathies after vaginal delivery. Case 1 is a 25-year-old woman who presented with a left foot drop after normal vaginal delivery after being in labor for 3 hours. Case 2 is a 24-year-old woman who presented with a right foot drop after norma

Refinement of symptoms of neuropathic pain measurements after various transections of the nerve endings of the sciatic and femoral nerve in rats: an exploratory behavioral analysis.

BACKGROUND: Many animal models can be used to study the underlying pathophysiological mechanisms of neuropathic pain. Most of these models rely on a partial denervation of the limb of the animal by ligating a selected nerve. In this study, we performed nerve lesions on three peripheral nerves supplying the plantar side of the rat hindpaw by differentially transecting the saphenous, the tibial, and the sural nerves alone or in paired combinations. METHODS: The development of neuropathic pain symptoms at three different anatomical areas (medial, central, and lateral) of the glabrous skin of the

POEMS syndrome - a unique presentation of a rare paraneoplastic syndrome.

POEMS (peripheral neuropathy, organomegaly, endocrinopathy, M protein, skin changes) syndrome is a rare multisystem paraneoplastic disorder. A 40-year-old male with a history of peripheral neuropathy and erectile dysfunction presented with a pathological fracture of the neck of the femur, found to be a solitary plasmacytoma. Additional unusual features included splenomegaly, hyperprolactinaemia and skin changes. The patient had a total hip replacement at a specialist orthopaedic hospital and is due to undergo radiotherapy to the femoral lesion and autologous stem cell transplantation.

High-resolution metal artifact reduction MR imaging of the lumbosacral plexus in patients with metallic implants.

OBJECTIVE: To assess the quality and accuracy of metal artifact reduction sequence (MARS) magnetic resonance imaging (MRI) for the diagnosis of lumbosacral neuropathies in patients with metallic implants in the pelvis. MATERIALS AND METHODS: Twenty-two subjects with lumbosacral neuropathy following pelvic instrumentation underwent 1.5-T MARS MRI including optimized axial intermediate-weighted and STIR turbo spin echo sequences extending from L5 to the ischial tuberosity. Two readers graded the visibility of the lumbosacral trunk, sciatic, femoral, lateral femoral cutaneous, and obturator nerv

Giant retroperitoneal solitary neurofibroma.

Solitary, benign retroperitoneal neurofibromas are exceedingly rare. Our case report highlights a man who presented with left-sided abdominal pain and left lower extremity numbness and weakness. On cross-sectional imaging, a 12.1 cm × 9.8 cm × 7.5 cm lobulated, well-marginated mass emanating from the left iliopsoas muscle at the level of the pelvic rim was visualised. A tissue biopsy suggested the diagnosis of a neurofibroma. The patient underwent resection of a femoral nerve sheath tumor, which resulted in femoral nerve palsy. Subsequent distal reconstructive surgeries and physical therapy re

Involvement of the lateral femoral cutaneous nerve as source of persistent pain after total hip arthroplasty.

This report describes a situation in which the lateral femoral cutaneous (LFC) nerve was the source of incisional pain in a patient after a total hip arthroplasty. The painful scar was denervated by an approach that resected just the posterior branch of the LFC nerve. This was done through an incision near the anterior superior iliac crest, avoiding any potential exposure of the implant. One of the 3 patients reported here had a coexisting meralgia paresthetica that was treated by the same operative approach. The orthopedic surgeon should include the LFC nerve as an origin of persistent pain a

A clinical case and anatomical study of the innervation supply of the vastus medialis muscle.

The innervation supply to the vastus medialis (VM) muscle, a component of quadriceps femoris (QF), is provided by a branch of the femoral nerve (FN) running along the muscle. The course of the nerve from lumbar roots to the muscle has been described by many researchers. It is known to ride along the femoral vein, artery and saphenous nerve and enter the adductor canal (Hunter's canal), and then to divide into branches that supply vastus medialis and the knee joint. Femoral mononeuropathy is uncommon, and is usually due to compression in the spinal level. Hematoma in the psoas and iliacus muscl
