Functional Outcome of Uncemented (Hydroxyapatite Coated) Bipolar Hemiarthroplasty in Fracture Neck of Femur in Elderly.

BACKGROUND: Femoral neck fractures in the elderly are associated with high morbidity and reduced mobility. Uncemented hydroxyapatite-coated bipolar hemiarthroplasty is increasingly used to avoid cement-related complications. This study evaluated postoperative functional outcomes using the Harris Hip Score (HHS). METHODS: A prospective observational study was conducted on 38 elderly patients (≥65 years) with displaced intracapsular femoral neck fractures who underwent uncemented hydroxyapatite-coated bipolar hemiarthroplasty. Functional outcome was assessed using the Harris Hip Score at nine m

Challenging the Paradigm: Fascia Iliaca Blocks May Not be Necessary for All Neck of Femur Fractures.

BACKGROUND: Fascia iliaca blocks (FIB) are recommended by national guidelines for analgesia in neck of femur (NOF) fractures to reduce opioid burden. However, evidence supporting universal administration is largely derived from patients with moderate-to-severe pain. It remains unclear whether ``blanket'' FIB protocols benefit the heterogeneous population presenting to emergency departments. OBJECTIVES: To evaluate real-world analgesic efficacy of FIBs across baseline pain severities and to assess whether a targeted approach is justified. METHODS: Single-centre, retrospective case-control stu

Outcome of Primary Total Hip Arthroplasty in Displaced Fracture Neck of Femur in Elderly Patients.

Femoral neck fractures are common fractures and constitute a major health problem especially in elderly. Primary total hip arthroplasty has advantages with little or no comorbidities because it gives good functional outcomes. The study was aimed to assess the outcome of primary total hip arthroplasty in displaced fracture neck of femur in elderly patients. This quasi-experimental study was conducted at National Institute of Traumatology and Orthopaedic Rehabilitation (NITOR), Dhaka, from March 2021 to September 2023 among 30 patients with displaced fracture neck of femur, aged 60 years or more

When the Heart and Hip Collide: The Interplay Between Atrial Fibrillation and Neck of Femur Fractures.

The association of atrial fibrillation (AF) and neck of femur fractures (NOF) are common in old people, creating a complex clinical scenario with significant implications for morbidity, mortality, and healthcare systems. This narrative review explores the bidirectional relationship between AF and NOF, focusing on shared risk factors, pathophysiological links, and challenges in clinical management, and also reviews the benefit of an orthogeriatric model. Advanced age, frailty, osteoporosis, polypharmacy, and cardiovascular comorbidities predispose patients to both conditions, while AF itself in

Total hip arthroplasty for head and neck of femur fractures secondary to civilian gunshot injuries.

BACKGROUND: The optimal treatment for intracapsular neck of femur fractures secondary to civilian gunshot injuries (GSI) remains a challenge. Surgical fixation is associated with a high failure rate due to avascular necrosis and non-union. This manuscript reports on the largest series of patients who underwent total hip arthroplasty (THA) for civilian GSI involving the hip joint. The objectives are to assess clinical outcomes and to report on complications as well as associated injuries. METHODS: All patients who had undergone THA for a civilian GSI to the hip joint at a single Level 1 Trauma

The Utility and Necessity for Radiographic Follow-Up After Arthroplasty for Geriatric Neck of Femur Fractures.

BACKGROUND: Hip fractures are a common and debilitating condition posing not only a huge health care but also socioeconomic burden. Surgical management for a neck of femur fracture is typically with arthroplasty in the form of total hip arthroplasty or hip hemiarthroplasty. Serial radiographs are typically performed routinely as part of follow-up to look for complications, although their clinical utility in asymptomatic patients is yet to be validated. Our paper therefore aims to review the utility and necessity of radiographic follow-up following arthroplasty for NOF fractures. MATERIALS AND

Outcomes of operative management in the delayed presenting neck of femur fractures in children: A case series.

INTRODUCTION: Normal pressure hydrocephalus (NPH) has three characteristics: Gait disorder, urinary incontinence, and amnesia. We report a case of atlantoaxial subluxation-related NPH in a patient whose symptoms were dramatically improved by atlantoaxial fusion surgery. CASE REPORT: A 67-year-old woman presented complaining of walking disturbance. 4 weeks after her first presentation and initial diagnosis of atlantoaxial subluxation associated with rheumatoid arthritis, the patient fell and fractured her right patella. After open surgery for the fracture with spinal anesthesia, she had urinar

The effect of time to surgery in neck of femur fracture patients with ASA Grade of 3 and above.

INTRODUCTION: Hip fractures are an important cause of morbidity and mortality. Early surgery has been shown to reduce mortality rates and surgical complications. The American Society of Anesthesiologists (ASA) grade is a widely used tool to assess preoperative health of patients. This study aims to assess is whether delay in surgical time has a greater impact on the mortality rates for high risk patients. METHOD: Retrospective study using the National Hip Fracture Database (NHFD) of 4883 neck of femur fracture patients. Time of surgery, ASA grade, reason for delay and mortality at 120 days wa

The timing of bone SPECT to predict osteonecrosis after internal fixation of femur neck fractures.

BACKGROUND: Bone SPECT can be used after a femur neck fracture to assess the circulation of the femoral head in the immediate postoperative period because the blood supply is one of the major factors affecting bone uptake of radiotracer on bone scintigraphy. The purpose of our present study was to investigate whether osteonecrosis of the femoral head (OFH) after internal fixation of femoral neck fracture could be predicted by early and late bone SPECT. METHODS: This retrospective cohort study enrolled 44 patients (33 women; mean age, 66.9 years) who underwent surgical fixation for femoral nec

Evaluation of the outcome of replacement hemiarthroplasty by uncemented bipolar prosthesis in displaced fracture neck femur.

Despite continued discussion regarding the treatment of displaced femoral neck fractures, controversies continue regarding their optimal treatment, including the choice of implant and fixation method. Hemiarthroplasty is one of the option which eliminate concerns about fixation failure, nonunion, and avascular necrosis and has become the choice of surgery among the aged >60. This prospective interventional study was carried out on 28 cases at the Department of Orthopaedic Surgery, Bangabandhu Sheikh Mujib Medical University (BSMMU), from July 2009 to April 2012 to evaluate cementless, bipolar

Multidisciplinary integrated care pathway for fractured neck of femur. A prospective trial with improved outcome.

Admitting patients with fractured neck of femur using fast tracking procedure is well established in over 75% of hospitals in the United Kingdom. A retrospective study on 143 patients admitted using such procedure revealed inadequacies in the system. Although the waiting time in the Accident and Emergency Department (A&E) was reduced, it did not contribute much to the other aspects of patients outcome. We conducted a prospective trial using a multidisciplinary integrated care pathway (ICP) for the management of patients with fractured neck of femur. The objective of this study was to assess th

Fractures of the neck of femur in children: an experience at the Aseer Central Hospital, Abha, Saudi Arabia.

The Orthopaedics Unit of Aseer Central Hospital, Abha, Saudi Arabia became fully operational in 1992. From then till 1999, 14 cases of fractures of the neck of femur in children under the age of 16 years were studied. The aims of the study were to compare our experience with those of other institutions and to serve as a reference point in this region for future study of this rare but disabling condition. The case notes of all the patients aged less than 16 years with fractures of the neck of femur managed at the Aseer Central Hospital between 1992 and 1999 were analysed retrospectively. Inform

Effect of early hip decompression on the frequency of avascular necrosis in children with fractures of the neck of the femur.

Twenty-eight per cent of 32 children with fractures of the neck of the femur developed avascular necrosis. It occurred in three of the four type I fractures, in five of the 11 type II fractures, in one of the 12 type III fractures and in none of the five type IV fractures. We studied the effect of early hip decompression on the frequency of avascular necrosis. It had no apparent value in type I fractures. To increase the statistical power of our analysis of displaced type II and III fractures we combined our cases with similar cases from other series. Twenty-two (41 per cent) of the 54 combine

Low-dosage ancrod for prevention of thrombotic complications after surgery for fractured neck of femur.

The effect of a low-dosage regimen of ancrod in the prevention of postoperative deep vein thrombosis was assessed in 24 patients having surgical repair of fractured neck femur and compared with 25 control patients who did not receive therapy. The objective of the therapy was to lower the preoperative fibrinogen level and produce a low concentration of fibrin degradation products yet avoid the haemorrhagic complications of total defibrination. Ancrod therapy proved feasible to carry out, was not associated with haemorrhagic complications, and produced sustained, predictable reductions in fibrin
