Pediatric glaucoma surgery is challenging because of the differences in anatomy from the adult, differences in the behavior of the tissues of a child's glaucomatous eye, the variety in causes of the disease, and difficulties with postoperative management. Goniotomy and trabeculotomy are the preferred initial treatments for primary congenital glaucoma. Trabeculectomy with adjunctive mitomycin C is more likely to succeed in older, phakic patients, but carries the long-term risk of bleb-associated endophthalmitis. Glaucoma drainage devices may be preferred in younger children and in patients with

Glaucoma drainage devices are an option in the management of complicated glaucomas that carry a high risk of failure from conventional filtering surgery. Examples include the glaucomas associated with aphakia or pseudophakia, neovascular glaucoma, and glaucomas associated with trauma, uveitis, epithelial downgrowth, iridocorneal endothelial syndrome, vitreoretinal disorders, and penetrating keratoplasty. Modifications in the various implant designs have been developed to limit the occurrence of postsurgical complications such as hypotony, serous and hemorrhagic choroidal detachment, tube and p

PURPOSE OF REVIEW: This review evaluates the surgical management of glaucoma in patients undergoing cataract surgery. Combining both procedures present challenges in balancing intraocular pressure (IOP) control, minimizing postoperative complications, and achieving optimal refractive outcomes.
RECENT FINDINGS: Recent studies highlight the effectiveness of combined glaucoma and cataract surgeries, with traditional filtering surgeries and MIGS showing significant IOP reduction. The abundance of long-term studies shows that MIGS can offer an effective and safer alternative when carefully tailore

The mutual relationship among medical therapy, ocular surface (OS) and filtration surgery (FS) represents one of the most crucial issues in glaucoma management. As the long-term use of intraocular pressure-lowering medications significantly affect the OS health, patients with an uncontrolled disease frequently undergo glaucoma surgery in less-than-ideal conditions. As we known, OS changes strongly affect the post-operative bleb filtration capability. Therefore, improving the OS conditions before proceeding with FS is needed. Currently, given the rapid diffusion of new surgical procedures, this

There is a large body of evidence from clinical and experimental studies indicating that the long-term use of topical drugs may induce ocular surface changes, causing ocular discomfort, dry eye, conjunctival inflammation, subconjunctival fibrosis, corneal surface impairment, and, as a consequence of chronic ocular surface changes, the potential risk of failure for further glaucoma surgery. Subclinical inflammation has also been widely described in patients receiving antiglaucoma treatments for long periods of time, with inflammatory cell infiltration and fibroblast activation in the conjunctiv

Filtering surgery evolved from the classic trabeculectomy, in which penetration into the anterior chamber is a necessary step, toward nonpenetrating deep sclerectomy. The first procedure presents several serious complications, such as durable hypotony, hyphema, flat anterior chamber, choroidal detachment, endophthalmitis, and surgery-induced cataract. To avoid such drawbacks, a novel nonpenetrating technique was designed to improve the predictability of the intraocular pressure (IOP)-lowering action, while reducing the incidence of the immediate postoperative complications encountered with the

BACKGROUND: To report the incidence and clinical characteristics of aqueous misdirection (AM) after glaucoma surgery in Chinese patients with primary angle-closure glaucoma.
METHODS: Medical records of all patients diagnosed with primary angle-closure glaucoma who underwent glaucoma surgery in the Eye Hospital of Wenzhou Medical University between January 2012 and December 2021 were retrospectively reviewed. Cases of AM were identified through a keyword-based search. The incidence of AM was calculated. Demographic and clinical characteristics of the AM patients were also described.

During recent years, glaucoma surgery has been modified by the introduction of new antiglaucoma drugs and by laser therapy. Various glaucoma operations have, however, retained their value in the treatment of severe glaucoma cases. Acute angle-closure glaucoma is best treated by iridectomy. When a clear cornea is present, laser iridectomy can be performed. Prophylactic treatment of the contralateral eye is mandatory. In chronic open-angle glaucoma, filtration surgery with a scleral flap is usually performed (goniotrephination or trabeculectomy). Modifications in the conjunctival incision and th

PURPOSE: This study investigates the results of trabeculectomy with peroperative topical application of mitomycin-C performed on eyes with previous failed glaucoma filtering surgery.
METHODS: The study includes 34 eyes of 26 patients operated on open angle glaucoma between March 1992 and October 1996, with a minimum of 6 months follow-up (mean 28.1 months). All the eyes had undergone prior one (27) o two (7) filtering surgeries. We evaluated intraocular pressure (IOP) and treatment reduction after surgery. Success was defined as IOP <20 mmHg, without deterioration of the visual field and/or p

INTRODUCTION: Standard filtering surgery in postoperative glaucoma in aphakic and pseudophakic eyes and eyes after failed filtering surgery is successful in 50% cases. The main cause of failure filtering surgery is scarring of the filtering bleb-fibroblastic proliferation at the subconjunctival episcleral interface [1, 2]. The use of antimetabolites such as 5-fluorouracil and Mitomycin C inhibits fibroblast proliferation and increases the success rate of filtering surgery, especially in postoperative glaucoma [3-6]. The exact dosage and frequency of administration to maximize success and minim

Two patients (two eyes) with end-stage, medically uncontrollable glaucoma underwent transvitreal pars plana filtering procedures with a contact Nd:YAG laser with a sapphire probe. Intraocular pressure was being maintained without medication four months after surgery, with persistence of a diffuse filtering bleb.

We report cases of delayed, sustained elevated intraocular pressure (IOP) associated with repeated intravitreal anti-VEGF injections (IVI), which ultimately resulted in the need for filtering surgery. Two of the three cases demonstrated severe IOP elevation despite maximal medical treatment following unilateral IVI and required urgent filtering surgery. Optic nerve involvement was severe in all three cases. These intravitreal injections were performed for exudative age-related macular degeneration (AMD), and the patients did not show any sign of glaucoma or ocular hypertension prior to the ini

PURPOSE: Using rat conjunctival bleb model, we correlated changes morphology and histology in the bleb with changes in MMP-2 and TIMP-2 levels.
METHODS: Filtering surgeries were performed on rats. Dynamic changes in morphology and histopathology were observed using HE staining. Expression of MMP-2 and TIMP-2 was determined by immunofluorescence microscopy and western blotting.
RESULTS: Well-elevated filtering blebs formed and persisted for an average of 12 days. Histological examination showed that inflammatory was dominant in postoperative days 1-3, and proliferating manifestation became th

After glaucoma filtering surgery, an excessive repopulation of Tenon's capsule fibroblasts (TCFs) with the accumulation of extracellular matrix including collagen types during wound healing may cause scarring of the bleb, resulting in surgical failure. Pentoxifylline (PTX) and pentifylline (PTF), both methylxanthine derivatives, are known to inhibit protein synthesis and proliferation of cells in vitro. We examined the effects of these agents on the proliferation of cultured human TCFs and their production of type-I collagen COOH-terminal peptide (PIP) and laminin to investigate the potential

PURPOSE: Delayed-onset endophthalmitis is a well-recognized complication of glaucoma filtering surgery, but, to our knowledge, no previous reports have associated this entity with occult nasolacrimal duct obstruction.
METHODS: A 16-month-old girl developed endophthalmitis 1 month after a mitomycin-C trabeculectomy for congenital glaucoma. A diagnostic pars plana vitrectomy, anterior chamber paracentesis, and injection of intravitreal antibiotics were performed.
RESULTS: Streptococcus pneumoniae was cultured from the aqueous and vitreous samples. Gradual clearing of the infection was achieved

A choroidal effusion, hypotony, and a shallow anterior chamber developed in a patient following a transconjunctival needling revision of an encapsulated filtering bleb. While choroidal effusions occur frequently following filtration surgery, to our knowledge, this complication has not been previously reported following a filtering bleb needle revision.

PURPOSE: To conduct a narrative review of research articles on the potential anti- and pro-fibrotic mechanisms of noncoding RNAs following glaucoma filtration surgery.
METHODS: Keyword searches of PubMed, and Medline databases were conducted for articles discussing post-glaucoma filtration surgeries and noncoding RNA. Additional manual searches of reference lists of primary articles were performed.
RESULTS: Fifteen primary research articles were identified. Four of the included papers used microarrays and qRT-PCR to identify up- or down-regulated microRNA (miRNA, miR) profiles and direct fur

BACKGROUND: After implantation of a XEN gel stent scarring of the conjunctiva with elevation of the intraocular pressure (IOP) is a frequent occurrence. This article describes a surgical revision technique with opening of the conjunctiva and removal of scar tissue. In order to standardize the technique the surgical procedure was divided into nine steps. These steps are shown in the video and a series of images.
METHODS: Surgery is divided into nine steps: 1) corneal traction suture and topical application of suprarenin, 2) opening of the conjunctiva along the limbus, 3) preparation posteriorl
