Elective free vascularized double transfer of toe joint from second toe to proximal interphalangeal joint of index finger: a case report.

Alternatives to the treatment of major injuries to the proximal interphalangeal (PIP) joint include fusion, implant arthroplasty, perichondrial grafting, single autogenous free vascularized transfer, and double autogenous free vascularized transfer. A patient presented a gunshot wound to the index finger with loss of skin and extensor tendon and PIP joint disruption. The finger was reconstructed with a composite free flap of skin and extensor tendon and PIP and distal interphalangeal joints of the second toe. A follow-up of 10 months is presented, which demonstrates PIP joint motion and finger

Morbid anatomy of 'erosive osteoarthritis' of the interphalangeal finger joints: an optimised scoring system to monitor disease progression in affected joints.

OBJECTIVES: To develop and validate a quantitative radiographic scoring system, the Ghent University Scoring System (GUSS), with better ability to detect progression over a shorter period of time in erosive osteoarthritis (OA) of the interphalangeal (IP) finger joints compared with the existing anatomic phase scoring system. METHODS: Thirty IP finger joints showing erosive features at baseline or follow-up were selected from 18 patients with erosive hand OA. Posteroanterior radiographs of these joints obtained at baseline, 6 and 12 months--totalling 90 images--were used for the study. All joi

Irreducible Dorsal Distal Interphalangeal Joint Dislocation of Finger: A Case Report and Perspectives on Management.

Irreducible closed dorsal dislocation of distal interphalangeal (DIP) joint of the finger is a rare injury. The causes of irreducibility of the DIP joint are volar plate interposition, entrapment of flexor digitorum profundus tendon behind the head of middle phalanx, and buttonholing of the middle phalanx head through the volar plate or flexor tendon. Open reduction with a volar approach is recommended with the advantages of better wound healing, ease of releasing entrapped structures, and possibilities of a volar plate, collateral ligaments, and/or flexor tendon repair. We report a case of ir

Supplementary arthrolysis of the proximal interphalangeal finger joint in Dupuytren's contracture: primary operation versus revision.

INTRODUCTION: In operative treatment of Dupuytren's disease, in certain cases proximal interphalangeal joint (PIP) flexion contracture remains after fasciectomy which can be corrected by a supplementary arthrolysis, but few data comparing primary and revision surgery are available. MATERIALS AND METHODS: Fifteen patients who had a fasciectomy and supplementary PIP arthrolysis at the affected finger for the first time were compared to 13 patients who had a revision fasciectomy with a supplementary PIP arthrolysis. Upon follow-up at 22 months, flexion contracture was measured and data were comp

Toward Sensor Fusion Neuromuscular Interface for Continuous Finger Joint Angle Estimation via Deep Transfer Learning.

Accurate decoding of motor intent from biosignals is an important step toward intuitive upper-limb prosthetic-control interfaces. We propose a novel high-dimensional multimodal deep learning framework that fuses surface electromyography (sEMG) and B-mode ultrasound (US) images to estimate metacarpophalangeal and proximal interphalangeal joint angles continuously. The framework employs a shared Encoder-Decoder-Regression architecture integrating convolutional neural networks (CNNs), transposed convolutions, an action-conditioned multi-head cross-attention module (ATT) that uses the commanded ac

Rotational Deformity of Long Finger Caused by Isolated Metacarpophalangeal Joint Capsular Injury: A Case Report.

CASE: A 49-year-old woman presented after a leash-pulling injury with ring and small finger proximal phalanx fractures. Pain and supination of the long finger metacarpophalangeal (MCP) joint were noted without collateral ligament injury on examination or imaging. Surgical exploration noted isolated capsular avulsion, which was repaired with restoration of alignment and full range of motion with relief of pain and instability at 1-year follow-up. CONCLUSION: This is a unique case of isolated finger MCP capsular injury causing rotatory instability. Surgeons should consider exploration and repai

Minimally Invasive Percutaneous Arthrodesis of a Finger Proximal Interphalangeal Prepared Joint, a Cadaveric Feasibility Study.

OBJECTIVE: Traditionally, the procedure for proximal interphalangeal (PIP) joint fusion has been described with an open approach using a dorsalcurvilinear incision. Due to the inherent risk of soft tissue complications with this technique, we propose a percutaneous technique for resecting bone cartilage and achieving a stable fusion angle with two mini-incisions, one lateral and one dorsal. The goal of all arthrodesis techniques is to create a stable joint fusion at the most functional angle for activities of daily living. MATERIALS AND METHODS: A small, low speed high torque straight burr wa

Sonographic analyses of pulley and flexor tendon in idiopathic trigger finger with interphalangeal joint contracture.

This study investigated the sonographic appearance of the pulley and flexor tendon in idiopathic trigger finger in correlation with the contracture of the interphalangeal (IP) joint in the thumb or proximal IP (PIP) joint in the other digits. Sonographic measurements using axial images were performed in 177 affected digits including 17 thumbs and 34 other digits judged to have IP or PIP joint contracture and 77 contralateral control digits. The A1 pulley of the contracture group was significantly thicker than that of the non-contracture group in all digits, whereas the flexor tendon was thicke

Control of an optimal finger exoskeleton based on continuous joint angle estimation from EMG signals.

Patients suffering from loss of hand functions caused by stroke and other spinal cord injuries have driven a surge in the development of wearable assistive devices in recent years. In this paper, we present a system made up of a low-profile, optimally designed finger exoskeleton continuously controlled by a user's surface electromyographic (sEMG) signals. The mechanical design is based on an optimal four-bar linkage that can model the finger's irregular trajectory due to the finger's varying lengths and changing instantaneous center. The desired joint angle positions are given by the predictiv

Resection of the flexor digitorum superficialis for trigger finger with proximal interphalangeal joint positional contracture.

PURPOSE: Open release of the A1 pulley is a widely known procedure for the treatment of trigger finger. A subset of patients presents with both trigger finger and a positional contracture of the proximal interphalangeal (PIP) joint. These patients usually have a long history of trigger finger or have already undergone a surgical release of the annular pulley. This study is a retrospective review of the outcomes of resection of the flexor digitorum superficialis (FDS) for patients whose trigger finger was associated with a positional contracture of the PIP joint. METHODS: Thirty-six patients (

Functional distal interphalangeal joint splinting for trigger finger in laborers: a review and cadaver investigation.

This two-part study evaluates the efficacy of functional distal interphalangeal joint (DIP) splinting for the treatment of trigger finger. Thirty-one fingers from 21 meat packing plant workers were treated with DIP splinting. A single corticosteroid injection was offered if triggering was stage 4 or greater. All workers returned to work immediately. Eighty-one percent of the digits were treated successfully (mean follow-up: 1 year). Treatment failure correlated with duration of symptoms and stage of triggering but did not correlate with age, race, sex, disease in multiple digits, or prior trea

Composite finger metacarpophalangeal joint reconstruction in combined second and third free toe-to-hand transfers.

Composite metacarpophalangeal joint reconstruction using the toe proximal phalanx and the metacarpal head, with a capsular repair, is an option during free toe transfer procedures for absent fingers. Eleven composite metacarpophalangeal finger joint reconstructions were performed in four patients concurrent with combined second and third toe-to-hand transfer. The average follow-up period was 5 years. Postoperative assessment included range of motion, stability, radiographic changes, and pain. The average range of motion was 52 degrees, average ulnar stress deviation was 14 degrees. No patients

Effectiveness of Proximal Interphalangeal Joint Block Orthosis and Metacarpophalangeal Joint Block Orthosis in the Management of Trigger Finger: A Prospective Observational Study.

BACKGROUND: Orthotic intervention is a non-invasive and commonly used treatment for trigger finger (TF). Among the various splinting options, the comparative effectiveness of proximal interphalangeal joint block orthosis (PIPJ-BO) and metacarpophalangeal joint block orthosis (MCPJ-BO) remains inadequately explored. This study compared the clinical effectiveness of PIPJ-BO and MCPJ-BO in reducing pain, symptom severity, and functional limitations in patients with TF. METHODS: A prospective observational study was conducted at a tertiary care hospital. Forty-six patients diagnosed with TF (Quin

Joint distraction and early mobilization using a new dynamic external finger fixator for the treatment of fracture-dislocations of the proximal interphalangeal joint.

BACKGROUND: Dynamic external fixation is a useful treatment option for unstable fracture-dislocations of the proximal interphalangeal (PIP) joint, because it simultaneously reduces axial pressure load on the joint surface, maintains congruent joint reduction, and permits early joint motion. However, most current devices are large, hindering finger movement, and unstable. To address these problems, we developed a dynamic external finger fixator, named the Micro Ortho Fixator®. The purpose of this study was to review the results of using the new external finger fixator to treat unstable fracture
