Analysis of factors influencing improvement of idiopathic flatfoot.

Idiopathic flatfoot is common in infants and children, and patients with this condition are frequently referred to pediatric orthopedic clinics. Flatfoot is a physiologic process, and that the arch of the foot elevates spontaneously in most children during the first decade of life. To achieve a consensus as the rate of spontaneous improvement of flatfoot, the present study aimed to estimate the rate of spontaneous improvement of flatfoot and to analyze correlating factors. We reviewed the records of patients examined between May 2013 and May 2019 so as to identify those factors associated with

Interference Screw for the Treatment of Pediatric Flexible Flatfoot.

Flexible flatfoot is a common deformity in the pediatric population and can cause a range of symptoms and reduce the quality of life. Subtalararthroereisis may be appropriate for pediatric population whose conservative management had failed to relief their symptoms typically for at least 6 months. Subtalararthroereisis has been developed for a long time, but the use of interference screw for the treatment of pediatric flexible flatfoot has not been reported. From January, 2016 to June, 2017, we operated on 21 children (39 feet) between the ages of 8 and 14 years. The clinical assessment was ba

Prevalence of pathological flatfoot in school-age children.

BACKGROUND: Prevalence of pathological flatfoot has not been well defined in the literature. The objective of this study was to establish the prevalence of pathological flatfoot in school-age children and investigate the factors associated with this deformity. METHODS: A total of 667 children (327 girls and 340 boys) were recruited for this cross-sectional study with multi-stage clustered random sampling. The diagnosis of flatfoot was based on clinical observations and measurements using a suggested systematic protocol. Pathological flatfoot in this study did not merely screen for structural

Naviculocuneiform Sag in the Acquired Flatfoot: What to Do.

Sag at the naviculocuneiform (NC) joint represents an important aspect of the flatfoot deformity. Failure to address medial column instability could lead to continued deformity and poor patient outcomes. No single procedure is enough to address the complexity of the adult acquired flatfoot deformity. Whether in combination with other procedures or in isolation, NC fusion and Cotton osteotomy are important pieces of the armamentarium to address all aspects of the flatfoot deformity.

Osteotomies for the Flexible Adult Acquired Flatfoot Disorder.

Flexible adult acquired flatfoot disorder is commonly treated with the use of osteotomies in the calcaneus and medial column. The combination of these joint-preserving osteotomies with additional soft-tissue procedures allows realignment of the hindfoot with the goal of preventing further deformity or degenerative joint disease. A thorough understanding of each patient's condition allows the surgeon to match the correct osteotomy to the clinical indication, while also successfully executing the planned surgery.

Flatfoot in Müller-Weiss syndrome: a case series.

INTRODUCTION: Spontaneous osteonecrosis of the navicular bone in adults is a rare entity, known as Müller-Weiss syndrome. We report here on our experience with six patients with Müller-Weiss syndrome accompanied by flatfoot deformity, but on a literature search found no reports on this phenomenon. Because the natural history and treatment are controversial, an understanding of how to manage this deformity may be helpful for surgeons when choosing the most appropriate operative procedure. CASE PRESENTATION: Six patients (five women, one man; average age, 54 years) with flatfoot caused by osteo

A robotic cadaveric flatfoot analysis of stance phase.

The symptomatic flatfoot deformity (pes planus with peri-talar subluxation) can be a debilitating condition. Cadaveric flatfoot models have been employed to study the etiology of the deformity, as well as invasive and noninvasive surgical treatment strategies, by evaluating bone positions. Prior cadaveric flatfoot simulators, however, have not leveraged industrial robotic technologies, which provide several advantages as compared with the previously developed custom fabricated devices. Utilizing a robotic device allows the researcher to experimentally evaluate the flatfoot model at many static

The use of arthrodesis to correct rigid flatfoot deformity.

Rigid adult flatfoot deformity ranges in severity and is caused by a variety of conditions. Treatment is based on the etiology, the severity of symptoms, the stage of the deformity, and patient goals. Posterior tibial tendon pathology, osteoarthritis, posttraumatic arthritis/deformity, inflammatory arthropathy, and neuropathic arthropathy are all known causes of adult flatfoot deformity. Regardless of the cause, treatment goals are the same-restore a plantigrade foot, decrease symptoms, and increase function. When nonsurgical modalities have failed, many surgical reconstructive options are ava

Management of the rigid adult acquired flatfoot deformity.

The optimal management of the adult acquired flatfoot requires careful assessment of the deformity. Although it is essential to recognize the location and degree of malalignment, it is of equal importance to appreciate whether the deformity is flexible or rigid. For patients undergoing surgery for a flexible flatfoot, various soft tissue procedures and bony osteotomies may be used to restore alignment while preserving joint motion. However, for patients undergoing surgery for a rigid deformity, such procedures are indicated less often, and arthrodesis is usually necessary.

Calcaneal osteotomies for the treatment of adult-acquired flatfoot.

Calcaneal osteotomies are useful procedures for the treatment of stage 2 adult-acquired flatfoot. Often combined with adjunctive soft-tissue procedures, the posterior calcaneal displacement osteotomy and Evans procedure provide effective realignment of pes planovalgus deformity. Preoperative evaluation, indications, contraindications, surgical considerations and techniques are discussed.

Talocalcaneal and subfibular impingement in symptomatic flatfoot in adults.

BACKGROUND: Patients with symptomatic flatfoot deformity often present with pain in the lateral part of the hindfoot. The cause of this pain has not been clearly established. Impingement between the talus and the calcaneus or between the calcaneus and the fibula has been suggested as a cause but has not been documented. METHODS: We examined the computed tomographic scans, performed with simulated weight-bearing, of nineteen adult patients with symptomatic flatfoot to determine the potential causes of pain in the lateral aspect of the foot. The scans were performed with use of a custom loading

Painful idiopathic rigid flatfoot in children and adolescents.

Nine patients (13 feet) were identified whose primary complaints were of atraumatic-onset, chronic pain in the hindfoot exacerbated with increased activity and who had the diagnosis of idiopathic rigid flatfeet. Eight of 11 were greater than the 95th percentile in weight for their age. Exam under anesthesia showed moderate to significant improvement in hindfoot motion in 9 feet; 4 feet required fractional peroneal lengthenings. Only 5 of 11 patients have had sustained relief of pain and report unlimited activity level. Children and adolescents with painful idiopathic rigid flatfeet without kno

An algorithmic approach to evaluation of the flatfoot. Avoidance of pitfalls.

The authors have attempted to lead the reader through a comprehensive and systematic diagnosis of the patient with the flatfoot. The authors followed the familiar SOAP format. Within each of the major steps (subjective findings, objective findings, assessment, and plan) a systematic algorithm is proposed. These are designed with the objective of comprehensively evaluating the patient. This should dissuade the reader from myopically approaching the flatfoot, and potentially missing comorbid or etiologic systemic disease. A brief introduction to a systematic treatment philosophy is also presente

Planar dominance. A major determinant in flatfoot stabilization.

For treatment of a symptomatic flat foot in a child or an acquired flatfoot from posterior tibial dysfunction in the adult, an understanding of the biomechanics and function of the foot and leg is essential to achieve an excellent outcome. Evaluation of supramalleolar deformity directly influences the procedure choice and eventual success of the procedure. An understanding of the level of deformity also indicates the procedure to be used. Subtalar and midtarsal joint function, although related, have their own primary source of deformity. Correction through the subtalar and midtarsal joints mus

Alterations in talar morphology associated with adult flatfoot.

To gain a better understanding on the anatomy of the factors contributing to symptomatic flatfoot, we compared the shape of the talus in feet that were flat to that in control tali from feet with a normal arch. Computed tomographic (CT) scans were performed on 9 adult patients with 10 symptomatic flatfoot deformities. CT scans of 10 feet being evaluated for acute trauma not involving the talus were randomly selected as controls. Flatfoot tali tended to be of greater overall length than the control tali, and this difference was not statistically significant. Statistically significant difference

Talonavicular arthrodesis for the painful adult acquired flatfoot.

Twenty-seven consecutive patients with posterior tibial tendon insufficiency were treated with talonavicular arthrodesis as the primary stabilizing procedure. Twenty-six patients were followed a minimum of 12 and an average of 27 months. Twenty-four patients had either no pain or pain only after heavy usage. Eleven patients were completely satisfied with postoperative foot function, 13 were satisfied, with minor reservations, and 2 were satisfied, with major reservations. Results were rated as excellent in 10 patients, good in 14 patients, and fair in 2 patients. Successful arthrodesis was ach

Hypermobile flatfoot in children. Comprehension, evaluation, and treatment.

Comprehension of the hypermobile flatfoot deformity depends on an understanding of the three structural deformities of the foot associated with the condition: (1) abduction of the forefoot relative to the weight-bearing line; (2) supination of the forefoot; and (3) heel valgus. Evaluation of the hypermobile flatfoot deformity is by clinical examination to determine the position of the foot with the talonavicular joint in the congruous position: position and motion of these components are determined from this starting point. A standing lateral talometatarsal angle is used to evaluate the deform
