Intraocular pressure and choroidal thickness respond differently to lower body negative pressure during spaceflight.

Spaceflight-associated neuro-ocular syndrome (SANS) develops during long-duration (>1 mo) spaceflight presumably because of chronic exposure to a headward fluid shift that occurs in weightlessness. We aimed to determine whether reversing this headward fluid shift with acute application of lower body negative pressure (LBNP) can influence outcome measures at the eye. Intraocular pressure (IOP) and subfoveal choroidal thickness were therefore evaluated by tonometry and optical coherence tomography (OCT), respectively, in 14 International Space Station crewmembers before flight in the seated, sup

[Liquid shifts in the lung under fenoterol and verapamil. Experimental study on the origin of pulmonary oedema under tocolysis. III (author's transl)].

The question of the causal relationship between tocolysis and pulmonary edema has been unresolved up till now. Eight mongrel dogs were given infusions of Fenoterol (0.06 mcg/kg/min) and later of Fenoterol (0.06 mcg/kg/min) and Verapamil (2 mcg/kg/min). In the case of infusion of Fenoterol alone measurements by the thermo-dye-technique showed massive shifts of lung fluids from the intravascular to the extravascular space corresponding to interstitial edema. The addition of Verapamil induced no further changes. The intrapulmonary haemodynamical overload by increased cardiac output and pulmonary

Metabolic Profiles of Encapsulated Chondrocytes Exposed to Short-Term Simulated Microgravity.

The mechanism by which chondrocytes respond to reduced mechanical loading environments and the subsequent risk of developing osteoarthritis remains unclear. This is of particular concern for astronauts. In space the reduced joint loading forces during prolonged microgravity (10-6 g) exposure could lead to osteoarthritis (OA), compromising quality of life post-spaceflight. In this study, we encapsulated human chondrocytes in an agarose gel of similar stiffness to the pericellular matrix to mimic the cartilage microenvironment. We then exposed agarose-chondrocyte constructs to simulated microgra

Bioinspired milliscale near-boundary undulatory motion for fluid transport and adhesive locomotion.

Gastropods use pedal waves to transport thin fluid films at the body-substrate interface. Inspired by this strategy, we present a near-boundary pumping mechanism using magnetically actuated, prebuckled undulatory sheets. Through theoretical modeling and experimental validation, we systematically analyze how design, actuation, and fluid properties influence wave propagation and transport performance of a single unit under full and partial immersion. We then demonstrate that optimized temporal and spatial phase shifts among multiple units notably enhance fluid transport. Leveraging these insight

Multisystem laboratory changes before and after blood purification in acute pancreatitis: a real-world descriptive analysis of 141 cases.

BACKGROUND: Acute pancreatitis (AP) is a dynamic, multisystem disorder. Blood purification (BP) is increasingly utilized in severe or hypertriglyceridemia-associated AP, yet the overall patterns and cross-dimensional heterogeneity of post-BP laboratory shifts remain poorly characterized. OBJECTIVE: To map the patterns, magnitudes, cross-system correlations, and patient-level heterogeneity of paired laboratory parameter changes observed before and after BP in AP patients using real-world data, strictly without inferring causal efficacy. METHODS: This retrospective observational study analyzed

Management of massive ovarian tumors.

Two patients with massive ovarian tumors, one with a 51 kilogram Stage IC mucinous cystadenocarcinoma and the other with a 34 kilogram mucinous cystadenoma, are presented. Problems associated with resection of massive ovarian tumors, including respiratory failure, intraoperative fluid shifts, adequate exposure, orthostatic hypotension and adynamic intestine, are identified. Guidelines for avoiding these pitfalls by the use of appropriate monitoring, controlled drainage of the cyst and transverse elliptic incision with abdominoplasty are suggested.

Reduction of interstitial fluid retention by combination therapy with dapagliflozin, an SGLT2 inhibitor, and conventional diuretics: comment on a post-hoc analysis of the DAPA-BODY trial.

Conceptual framework illustrating differential fluid dynamics between SGLT2 inhibitor monotherapy and combination therapy with conventional diuretics in patients with chronic kidney disease. Combination therapy preferentially reduces interstitial fluid while preserving plasma volume, potentially through attenuation of vasopressin activity, natriuresis, and transcapillary fluid shift.

Different Effects on Fluid Distribution between Tolvaptan and Furosemide in a Liver Cirrhosis Patient with Chronic Kidney Disease.

A 73-year-old man with liver cirrhosis and advanced chronic kidney disease was admitted to our hospital due to bilateral lower leg edema and appetite loss. Furosemide to treat fluid retention markedly decreased extracellular water compared with intracellular water, but the addition of tolvaptan equally decreased both with a greater diuretic response than furosemide alone. Furthermore, tolvaptan administration increased the plasma colloid osmotic pressure, which might facilitate the shift of fluid from the extravascular space to the intravascular space. This is the first case showing different

Cardiorenal syndrome: diagnosis, treatment, and clinical outcomes.

The pathophysiologic interactions that link the heart and kidney are multiple and complex, and have been grouped under the umbrella term "cardiorenal syndrome." In the setting of acute decompensated heart failure, worsening renal function has been directly associated with poor clinical prognosis and complicates treatment. However, the pathophysiology underlying acute cardiorenal syndrome remains incompletely understood and treatment options remain limited. Traditionally, the development of worsening renal function in acute decompensated heart failure has been attributed to renal arterial under

Diuretic as a means for rapid adaptation to weightlessness.

In a number of flights, cosmonauts and astronauts have experienced aggravation of their health status and general condition in the initial hours and days in a weightless environment. One of the trigger mechanisms for the onset of these unfavourable conditions at the start of space flight is a redistribution of body fluids and a blood shift towards the head. To ensure controlled hypohydration as a countermeasure to the deleterious effects of 0-g and to investigate the feasibility to control adaptation, six cosmonauts were administered lasix once a day during the first 3 days of a mission. All d

Delayed-Onset Fibrotic Capsular Bag Distension Syndrome.

Capsular bag distension syndrome (CBDS) is an uncommon complication of cataract surgery following in-the-bag posterior chamber intraocular lens (PCIOL) implantation. It is characterized by the accumulation of turbid fluid between the IOL optic and the posterior capsule, frequently resulting in reduced visual acuity and a myopic refractive shift. The late-onset fibrotic variant is particularly rare and may manifest several years after otherwise uneventful surgery. A 52-year-old man presented with progressive visual blurring 10 years after uncomplicated phacoemulsification with in-the-bag PCIOL

Postoperative Capsular Bag Distension Syndrome: Case Report of an Early Case Following Athalamia due to Vigorous Eye Rubbing.

INTRODUCTION: Capsular bag distension syndrome (CBDS) is a rare complication of phacoemulsification, characterized by fluid retention in the capsular bag leading to reduced visual acuity, a myopic shift, and a risk of pupillary block. CASE PRESENTATION: We report the case of a 65-year-old male who developed CBDS after rubbing his eye vigorously postoperatively. On Postoperative Day 1, his best corrected visual acuity (BCVA) was 0.1 with a myopic shift. We made the diagnosis of CBDS with the Scheimpflug camera and AS-OCT. Conservative management was initially attempted, but due to persistent s

Shifting Selves-The (In)flexibility of Part-White Mixed-Race Identities in Response to Identity Rejection and Acceptance.

This study explores identity shifts among part-White mixed-race individuals in response to identity acceptance and rejection. Two experiments (N = 256 and N = 337) examined how such experiences influence the strength of White, Minoritized, and Mixed identification. Participants recalled and described an experience of acceptance or rejection from a racial-ethnic ingroup. We assessed White, Minoritized, and Mixed identification both weeks before and immediately after this manipulation. Results consistently showed that participants increased identification with an accepting ingroup but did not de

Optimizing autonomous artificial intelligence diagnostics for neuro-ocular health in space missions.

Spaceflight-Associated Neuro-Ocular Syndrome (SANS) presents a critical risk in long-duration missions, with microgravity-induced changes that threaten astronaut vision and mission outcomes. Current SANS monitoring, limited to pre- and post-flight exams, lacks in-flight diagnostics, highlighting an urgent need for autonomous tools capable of real-time assessment. Grok, an AI platform by xAI, offers promising potential as an advanced diagnostic tool for space-based health monitoring. Originally developed for broader applications, Grok's high-resolution imaging capabilities could be adapted to d

Supine fluid redistribution: should we consider this as an important risk factor for obstructive sleep apnea?

INTRODUCTION: Obstructive sleep apnea (OSA) is a common medical disorder affecting at least 2 % of women and 4 % of men living in Western societies. Obesity, older age, male gender, alcohol and sedative use, smoking, craniofacial parameters, and volume overload are some of the risk factors for this disorder. DISCUSSION: OSA is a known risk factor complicating the course of arterial hypertension, heart failure, and chronic kidney disease. It is important to note that all of the aforementioned comorbid disorders are associated with volume overload. This explains why patients with OSA and comorb

Case report: Physiological changes due to a rapid ascent from 55 metres underwater.

We present the case of a diver who experienced an uncontrolled ascent from 55 m and presented with a severe decompression illness. She was clinically shocked and in multi organ failure due to massive fluid shifts. She demonstrated bilateral lower limb loss of power and sensation and required multiple hyperbaric therapy sessions. With joint critical care, hyperbaric and physical therapy involvement, she was discharged some five weeks after her presentation with an independent level of function.

Excessive Postoperative Fluid Administration in Infants with Gastroschisis.

Careful fluid management is a cornerstone of neonatology because the cardiovascular, respiratory, and gastrointestinal systems in the newborn are sensitive to overhydration. Fluid management in gastroschisis is complicated by insensible fluid loss and postoperative third-space fluid shifts. Study of perioperative fluid management in gastroschisis is limited and has not undergone careful scrutiny. We reviewed perioperative fluid administration and urine output in all infants with gastroschisis over a 5-year period. Data included whether the patient underwent primary closure or staged repair, we

Cyclic GMP excretion blocked by isatin administration under conditions of fluid overload.

Isatin is a potent inhibitor of atrial natriuretic peptide (ANP) receptors and ANP-induced generation of cGMP in vitro. This study was designed to determine whether it had a similar effect in vivo, using a model of fluid overload known to induce ANP. We confirmed that this model increased urinary output of cGMP 3 hr after volume loading, and showed that this effect was blocked by i.p. injection of isatin (50 mg/kg). Isatin had no effect on urine volume or sodium output. However, isatin did have an effect on plasma protein concentration, both compared with control values, compatible with shifti

Soluble epoxide hydrolase derived lipid mediators are elevated in bronchoalveolar lavage fluid from patients with sarcoidosis: a cross-sectional study.

BACKGROUND: Sarcoidosis is a systemic inflammatory multi-organ disease almost always affecting the lungs. The etiology remains unknown, but the hallmark of sarcoidosis is formation of non-caseating epithelioid cells granulomas in involved organs. In Scandinavia, > 30% of sarcoidosis patients have Löfgren's syndrome (LS), an acute disease onset mostly indicating a favorable prognosis. The impact of dysregulation of lipid mediators, which has been investigated in other inflammatory disorders, is still unknown. METHODS: Using three different liquid chromatography coupled to tandem mass spectrome
