Raised intraocular pressure occurs in a significant proportion of patients with uveitis, and may lead to glaucomatous visual loss. Liposomes, small bilayer vesicles composed of phospholipids, can entrap and thus slow the release of drugs. We investigated the use of liposome-encapsulated 5-fluorouracil (5-FU), an antiproliferative agent, in the treatment of proliferative vitreoretinopathy (PVR) in an animal model. Doses of up to 1.6 mg administered intravitreally in rabbits demonstrated no retinal toxicity by histologic or electroretinographic criteria. In an experimental animal model of PVR, intravitreal injection of homologous rabbit corneal fibroblasts caused tractional retinal detachments in 90% of eyes after 4 weeks.

A prospective study of 5-Fu in the treatment of prostatic hyperplasia was commenced in 1981. The short term results were fair in 70% of 43 patients. Atrophy of the glandular tissue could be observed on histopathologic examination. The study showed that the larger the glandular component the better was the therapeutic effect. If abundant fibromuscular tissue was present in the gland the treatment was unsatisfactory. Abnormal anatomy of the bladder neck, represented a contraindication to medical treatment.

Topically applied 5-fluorouracil has recently gained acceptance in the treatment of actinic keratosis of the lip; however, histologic evidence confirming the effectiveness of the therapy has not yet been established. 5-Fluorouracil (5-FU) cream was compared with podophyllin 25 percent in alcohol for treating genital and urethral condylomata acuminata. The cure rate after four weeks was found to be 6 of 18 patients treated with 5-FU and 10 of 19 treated with podophyllin. Topical fluorouracil is widely used for the treatment of precancerous and cancerous lesions of the skin. The most common side effect of this medication is localized irritant dermatitis. The authors report a case of dysgeusia with metallic taste as a side effect of this medication. While not previously seen with topical use, this is not an uncommon side effect seen with systemic administration of 5-fluorouracil. The etiology of dysgeusia from chemotherapeutic agents and systemic absorption of fluorouracil is discussed.
