Comparison of the effects of foot-toe orthoses on three-dimensional pelvic kinematics in individuals with hallux valgus during gait.

BACKGROUND: Most previous studies focused on the possible relationship between hallux valgus deformity and biomechanical characteristics in the foot and lower leg. Very few investigated the association of pelvic kinematics and hallux valgus during gait according to various foot-toe orthoses for hallux valgus. OBJECTIVE: To investigate the effect of soft or hard foot-toe orthoses on pelvic kinematics in individuals with hallux valgus deformity during gait. STUDY DESIGN: This study has a repeated measures design. METHODS: Three-dimensional range of motion in the pelvic segment was collected f

Adults with flexible pes planus and the approach to the prescription of customised foot orthoses in clinical practice: A clinical records audit.

A clinical records audit of the University of South Australia's podiatry clinic clients attending in 2010 was undertaken to determine prevalence of symptomatic flexible pes planus, presenting reasons and treatment options most frequently used. Analysis of rearfoot measures (resting calcaneal stance position, subtalar joint range of motion) between those prescribed a vertical (heel) or inverted (heel) cast pour and a medial heel (Kirby) skive was undertaken. Of 223 clinical records audited, 50% (111/223) of clients were assessed with flexible pes planus, 77% (86/111) of clients with pes planus

The use of a low cost 3D scanning and printing tool in the manufacture of custom-made foot orthoses: a preliminary study.

BACKGROUND: Custom foot orthoses are currently recognized as the gold standard for treatment of foot and lower limb pathology. While foam and plaster casting methods are most widely used in clinical practice, technology has emerged, permitting the use of 3D scanning, computer aided design (CAD) and computer aided manufacturing (CAM) for fabrication of foot molds and custom foot orthotic components. Adoption of 3D printing, as a form of CAM, requires further investigation for use as a clinical tool. This study provides a preliminary description of a new method to manufacture foot orthoses using

Effect of ankle-foot orthoses on the sagittal plane displacement of the center of mass in patients with stroke hemiplegia: a pilot study.

BACKGROUND: Ankle-foot orthoses (AFOs) have been reported to have positive effects on the temporal-spatial parameters and kinematics and kinetics of gait in patients with stroke. The center of mass (COM) may be used to represent whole body movement and energy cost in gait, and therefore COM movement would also be positively influenced with use of an appropriate AFO. OBJECTIVE: To investigate the effect of AFOs on the sagittal plane displacement of the COM in patients with stroke hemiplegia. METHODS: Five male subjects with stroke hemiplegia participated in this pilot study. The trajectory of

Visualisation to enhance biomechanical tuning of ankle-foot orthoses (AFOs) in stroke: study protocol for a randomised controlled trial.

BACKGROUND: There are a number of gaps in the evidence base for the use of ankle-foot orthoses for stroke patients. Three dimensional motion analysis offers an ideal method for objectively obtaining biomechanical gait data from stroke patients, however there are a number of major barriers to its use in routine clinical practice. One significant problem is the way in which the biomechanical data generated by these systems is presented. Through the careful design of bespoke biomechanical visualisation software it may be possible to present such data in novel ways to improve clinical decision mak

The influence of two different types of foot orthoses on first metatarsophalangeal joint kinematics during gait in a single subject.

OBJECTIVE: To quantify the effect of two distinct foot orthotic designs on in vivo multisegment foot and leg motion; in particular, the first metatarsal and first metatarsophalangeal (MTP) joint during gait. METHODS: A 23-year-old man had an excessively pronated foot structure as measured during a clinical orthopedic examination. The Optotrak Motion Analysis System was used to collect three-dimensional position and orientation data from four modeled rigid body segments (hallux, first metatarsal, calcaneus, and tibia) during the stance phase of walking. The subject walked at a self-selected co

A comparison of the Sport Stirrup, Malleoloc, and Swede-O ankle orthoses for the foot-ankle kinematics of a rapid lateral movement.

Little information exists about the efficacy of various brace designs to restrict inversion during lateral movements. The purpose of the study was to determine whether semi-rigid stirrup (AirCast Sport Stirrup), semi-rigid modified stirrup (Malleoloc), or a soft, sleeve design (Swede-O) brace varied in their abilities to restrict inversion without hindering plantar/dorsiflexion when a lateral cutting movement is performed compared to a no-brace condition (No-Br). Nineteen volunteers who had previously sprained their right ankles performed 10 sideward cutting trials/brace condition. Based on ki

Validation of the estimated Effect of Ankle Foot Orthoses on Spinal Cord Injury Gait Using Subject-Adjusted Musculoskeletal Models.

Simulation of assistive devices on pathological gait through musculoskeletal models offers the potential and advantages of estimating the effect of the device in several biomechanical variables and the device characteristics ahead of manufacturing. In this study, we introduce a novel musculoskeletal modelling approach to simulate the biomechanical impact of ankle foot orthoses (AFO) on gait in individuals with spinal cord injury (SCI). Leveraging data from the Swiss Paraplegic Center, we constructed anatomically and muscularly scaled models for SCI-AFO users, aiming to predict changes in gait

Comparing gait performance of people with Charcot-Marie-Tooth disease who do and do not wear ankle foot orthoses.

BACKGROUND AND PURPOSE: Ankle foot orthoses (AFOs) are commonly prescribed for people with Charcot-Marie-Tooth (CMT) disease. Scant evidence exists to guide the type and timing of orthotic prescription. This study explores the latter issue by investigating the differences in presentation and gait function of people with CMT disease who wore AFOs for daily mobility (n = 11) and a group who did not (n = 21). The aim was to see if there was a difference in the characteristics in people who regularly wear AFOs. METHODS: Primary measures of gait function were a 10-m timed walk (comfortable and max

Effectiveness of Foot Biomechanical Orthoses to Relieve Patients' Knee Pain: Changes in Neural Strategy After 9 Weeks of Treatment.

Knee pain is one of the most common lower leg complaints. It is often treated with plantar orthoses to provide cushioning and correct locomotion, imbalances of the foot, and postural deficits. However, the published scientific data are poor concerning the mechanisms involved in pain reduction after wearing foot orthoses, and, to the best of our knowledge, no trial has investigated the mid-term effectiveness. The aim of the present study was to evaluate the effectiveness of foot orthoses according to sound biomechanical principles in the treatment of knee pain. Attention was mainly focused on c

Correlations between Berg balance scale and gait speed in individuals with stroke wearing ankle-foot orthoses - a pilot study.

PURPOSE: The Berg balance scale (BBS) is commonly used to assess balancing ability in patients with stroke. The BBS may be a good candidate for clinical assessment prior to orthotic intervention, if it correlates well with outcome measures such as gait speed. The purpose of this study was to investigate the correlation between the BBS measured prior to walking with an ankle-foot orthosis (AFO) and specific temporal-spatial parameters of gait when walking with an AFO donned. METHODS: Eight individuals with chronic stroke participated in this study. Balancing ability was assessed using the BBS,

Foot orthoses for forefoot pressure reduction and the hypothesized role in calf muscle stretching: A systematic review highlighting an evidence gap.

BACKGROUND: Excessive forefoot plantar pressures are associated with pain, ulceration risk, and functional limitations in both healthy individuals and those with pathologies such as rheumatoid arthritis (RA). Limited ankle dorsiflexion, often related to gastrocnemius-soleus tightness, may contribute to forefoot overload. Although foot orthoses (FOs) are widely used to redistribute plantar loads, it remains unclear whether the existing evidence includes orthotic interventions designed to promote posterior chain flexibility. OBJECTIVE: To systematically evaluate the effectiveness of foot orthos

Effects of sagittal plane axis shifts in hinged ankle-foot orthoses on gait mechanics and shaft-to-leg relative motion.

BACKGROUND: The alignment of the axis in ankle-foot orthoses is essential for ensuring optimal biomechanical function and comfort for the user. Correct alignment reduces orthosis displacement, minimizes skin irritation and joint stress, and improves overall performance. This study explores the effects of varying joint axis positions on ankle-foot orthoses functionality and shaft-to-leg movement. METHODS: Six healthy adult males (mean age: 35 ± 12 years) participated, walking with bilateral ankle foot orthoses configured in five joint axis positions and a shod condition. Randomized, crossover clinical trial on the safety, feasibility, and usability of the ABLE exoskeleton: A comparative study with knee-ankle-foot orthoses.

Wearable exoskeletons are emerging as a new tool for gait training. However, comparisons between exoskeletons and conventional orthoses in terms of safety and feasibility are scarce. This study assessed the safety, feasibility, usability, and learning process of using the ABLE Exoskeleton in people with spinal cord injury (SCI) while comparing it with knee-ankle-foot orthoses (KAFOs). In this randomized, crossover clinical trial, 10 patients with chronic complete SCI (T4-T12) conducted a 10-session training and assessment protocol with each device: KAFOs and the ABLE Exoskeleton. Outcomes on s
