During a ten year period (1978-1988) 565 patients, aged 70 years and over, suffering a fresh pertrochanteric fracture have been treated in the Department of Traumatology at the University Hospitals of Leuven, Belgium. According to the system of Evans and Jensen, 388 fractures were classified as unstable. Special attention was given to the 324 cases of type I C and I D fractures. The method of treating greatly changed during the period of study. All patients were followed up prospectively during one year. Our study showed that for these unstable fractures, fixation with an angled plate or Ender

Osteoporosis international : a journal established as result of cooperation between the European Foundation for Osteoporosis and the National Osteoporosis Foundation of the USA2025eng
UNLABELLED: Metaphyseal comminution in distal radius fracture (DRF) cases might indicate severe osteoporosis. The patients with DRFs and metaphyseal comminution showed 5.2-fold increased secondary fractures compared with those receiving combination osteoporosis therapy. High-risk DRF patients require aggressive osteoporosis management and fracture risk stratification.
PURPOSE: Distal radius fractures (DRFs) are common in patients with osteoporosis and associated with increased risks for subsequent fractures. Metaphyseal comminution in patients with DRFs may indicate severe osteoporosis and he

We aimed to assess the influence of fracture location and comminution on acute scaphoid fracture displacement using three-dimensional CT. CT scans of 51 adults with an acute scaphoid fracture were included. Three-dimensional CT was used to assess fracture location, comminution and displacement. Fracture location was expressed as the height of the cortical breach on the volar and dorsal side of the scaphoid relative to total scaphoid length (%), corresponding to the fracture's entry and exit point, respectively. We found a near-linear relation between dorsal fracture location and displacement.

OBJECTIVE: Middle third clavicular fracture is effectively treated by conservative means. Previous studies showed that comminution and displacement of clavicular fractures might affect fracture healing. However, the clavicle horizontally aligns in the shoulder girdle and has different biomechanics from vertical weight-bearing bones. Therefore, this study was conducted with the hypothesis that comminution has no effect in worsening fracture healing and union configurations in conservatively treated middle third clavicular fractures.
METHODS: One hundred ninety-eight middle third clavicular fra

Fractures of the proximal interphalangeal (PIP) joint with fragment displacement should be promptly repaired after injury, though this does not ensure the return of pre-injury finger function. The most common surgical treatment involves open reduction and internal fixation (ORIF) or closed reduction and internal fixation (CRIF) using locking compression plate (LCP), intramedullary nails, or screws. In articular fractures of the hand and wrist, the surgical reduction of small fragments is generally less feasible to occur compared to long bone fractures. This article describes the case of a 58-y

BACKGROUND: Extra-articular but severely comminuted distal basal fractures of the proximal phalanx (PP) are rarely reported. Therefore, the aim of this study was to achieve proper union and desirable outcomes using low-profile locking plates/screws. We introduced our own surgical approach and reported the clinical/radiographic outcomes via retrospective case series.
METHODS: Thirty-eight patients diagnosed with comminuted basal fracture of the PP were admitted between January 2014 and December 2020. Nineteen of these patients met the inclusion/exclusion criteria and were investigated in this

Comminuted mandibular fracture poses a surgical challenge. Extensive comminution and absence of reference adds complexity particularly in anterior region. In this technical note, we describe a novel application of virtual surgical planning combined with 3D printing which grants precise anatomic reduction in such cases. Computed Tomography data (mandible) of another patient of same age and sex was utilized to create reference model by virtual surgical planning. Virtually created model was 3D printed and utilized for precontouring the reconstruction plate which assisted in achieving satisfactory

BACKGROUND: Although there are several different kinds of fixation techniques for displaced comminuted patellar fracture, the treatment remains a challenge for orthopaedic surgeons. The purpose of this study is to evaluate the effectiveness and safety of a fixation technique for comminuted patellar fracture fixation using modified cerclage wiring.
METHODS: From February 2016 to April 2018, 38 cases of simple unilateral closed comminuted patellar fracture were treated by modified cerclage wiring. Among these cases, 16 patients were males and 22 were females, aged 23-68 years (average 40.4 ± 9.

INTRODUCTION: Facial and mandibular aggression with gunshot wound (GSW) is highly complex and represents a challenge for the assistant surgical team because in addition to the inherent lethal potential, they have the capacity for destruction and mutilation caused by the high kinetic energy conferred to the firearm projectile. Among these lesions, comminutive mandibular fractures are difficult to treat because the anatomical planes are distorted, soft tissues are in severe disarrangement, and bone fragments are without periosteum, which, if opted for an open treatment, with internal fixation, f

PURPOSE: Traditionally, the treatment of comminuted mandibular fractures involves both closed and open reduction. However, modern treatment principles increasingly tend toward open reduction and internal fixation to shorten oro-functional rehabilitation. Although this method increasingly gained popularity to date, a controversy regarding the extraoral versus the intraoral surgical approach still exists. The current study aimed to objectively evaluate the outcome of comminuted mandibular fracture treatment involving open reduction and internal fixation using an intraoral approach.

OBJECTIVE: To report metacarpal fracture repair in a bull.
STUDY DESIGN: Clinical report.
ANIMALS: Simmental bull (n = 1; 870 kg).
METHODS: A comminuted right metacarpal fracture was repaired using 2 transfixation pins (6 mm diameter) through the distal metaphysis and diaphysis of the radius and a full limb synthetic resin cast.
RESULTS: Slight bending of both pins occurred 1 day after surgery. Both pins were loose and removed at 31 days and a full-limb cast reapplied. The cast was changed at approximately -4 week intervals. On day 50, the right carpus had moderate overextension and a valg

BACKGROUND: Most of the Intraarticular distal radius fractures are complex and unstable. They are caused by high-energy injuries. The optimal and appropriate treatment remains a topic of controversy. Many different methods of treatment have been advocated such as closed reduction maintained by cast, K-wire fixation with cast, and opened reduction with internal fixation with plate and screws or external fixation with or without K-wire augmentation.
OBJECTIVE: To study the efficacy of the TU Model External Fixator for the treatment of unstable intraarticular fracture of distal radius. This was

BACKGROUND: Therapeutic management of proximal humerus fractures is still controversial. There are several ways of proceeding in these cases, ranging from conservative treatment to shoulder joint arthroplasty. Open surgical reduction and stabilization with a locking compression plate is currently the most widely used method of treatment, especially in younger patients. The purpose of this study is to analyze the results of surgical treatment of fractures of the proximal humerus using locking compression plates.
MATERIAL AND METHODS: The study included 69 patients, 57 of them reported for the

A fracture construct, representing a worst-case model of a comminuted intertrochanteric fracture, was created in order to compare the fixation stability of two different cephalomedullary nails: one where the lag screw can telescope within itself to achieve displacement of the head-neck fragment, and the other where the solid lag screw slides only. After nail fixation, the models were loaded and then cycled, and positions of the head-neck fragment and lag screw were determined. Both nails similarly acted to limit motion of the head-neck fragment by the sliding of their lag screws, causing impin

BACKGROUND: The purpose of this study was to evaluate the various factors, which could contribute towards redisplacement of distal radius fractures, including comminution of the dorsal cortex of the distal radius, treated in our department.
METHODS: In this retrospective study, we evaluated the risk of redisplacement of distal radius fractures in our department and also looked at the probable factors predisposing to this risk. A total of 134 fractures(129 children) were included in the study after exclusions. The variables that were assessed as possible causes of redisplacement were age, gend

INTRODUCTION: Comminuted intraarticular fractures of the distal radius metaphysis are a major challenge for orthopedic surgeons. The aim of this study was to present results of the survey on treatment of these fractures using an external fixator.
MATERIAL AND METHODS: 73 patients (30 females and 43 males) with closed comminuted intraarticular fractures of the distal radius, type C AO/ASIF (based on radiography at the moment of injury) were treated by a Mitkovic external fixator and followed-up for at least 2 years. An external fixator and Kirschner wires were used in 43 patients. An external

A burst fracture of T 12, without neurological impairment, was treated by plaster cast. Prior to treatment, there was a significant narrowing of the spinal canal, due to posterior displacement of a bone fragment. This was not modified by treatment, as the narrowing was unchanged on the second CT scan, done eight days later. At 17 months, there was no more narrowing on the CT scan, as a consequence of vertebral body remodeling. The patient had no pain and performed several sport activities.

From 1982 to 1987 we treated 85 extra-articular comminuted distal third humeral fractures in adults with prefabricated plastic braces. Of these, 15% were open fractures and 18% had initial peripheral nerve injury. On average, the sleeve was applied 12 days after injury and used for 10 weeks. There was 96% union, with no infections. All nerve injuries resolved or were improving at the latest examination. At union there was varus deformity averaging 9 degrees in 81% of patients, but loss of range of movement was minimal and functional results were good.

Fractures of the femoral shaft are often treated by an established method of internal fixation, such as a medullary nail or a plate and screws, to obviate the many disadvantages of traction and prolonged rest in bed. Fractures of the femoral shaft which are severely comminuted and open are usually unsuitable for internal fixation. Between July 1981 and March 1983 we treated seven patients with severely comminuted fractures of the femoral shaft (of which three were open), using the ASIF tubular external fixator system or the Wagner apparatus. The technique of application of the external fixator
