Freezing cold injuries (frostbite) of the extremities are a common injury among alpinists participating in high altitude expeditions, particularly during inclement weather conditions. Anecdotally, a digit that has suffered frostbite may be at greater risk to future cold injuries. In this case study, we profile a 62-year-old elite alpinist who suffered multiple digit amputations on both his hands and foot after historic summit attempts on Makalu (8481 m) and Mt. Everest (8848 m) in 1974-1979. We describe the clinical treatment he received at that time, and follow up his case 40 years after the

High-altitude regions, cold deserts, permafrost regions, and the polar region have some of the severest cold conditions on earth and pose immense perils of cold injuries to exposed individuals. Accidental and unintended exposures to severe cold, either unintentionally or due to occupational risks, can greatly increase the risk of serious conditions including hypothermia, trench foot, and cold injuries like frostbite. Cold-induced vasoconstriction and intracellular/intravascular ice crystal formation lead to hypoxic conditions at the cellular level. The condition is exacerbated in individuals h

BACKGROUND: Apart from clean cut finger amputations, every kind of hand injury can be seen in mountain and winter sports but only skier's thumb and injuries of the pulley system in sport climbers are seen in a greater number of cases. Nevertheless, these two common injuries as well as the rare frostbite of the fingers are often underdiagnosed or overdiagnosed as well as undertreated or overtreated.
PURPOSE: This paper describes the diagnostics and treatment of skier's thumb, injuries of the pulley system in sport climbers and frostbite of the fingers.
RESULTS: Before checking the metacarpoph

BACKGROUND: These experiments aimed to study the in vivo short and long term neurovascular regeneration after frostbite.
METHODS: The rabbit central ear-artery was used as the experimental model. The effects on the noradrenergic innervation of the artery were measured in isolated vascular ring segments the first day and 2, 3-4, and 8-10 or 10-20 weeks following freezing at -9 degrees C or -18 degrees C for 15 min with slow rewarming for 7 min at room temperature.
RESULTS: Two days after freezing the sympathetic nerves were completely degenerated, as observed with glyoxylic acid-induced fluor

BACKGROUND: A vast number of conditions ranging from simple trauma to hereditary and collagen vascular disease had been described in association with acroosteolysis.
OBJECTIVE: To demonstrate that severe cold exposure not mounting to frostbite may be associated with acroosteolysis.
METHODS: A 16-year-old girl with acroosteolysis presenting with brachyonychia was fully investigated for possible cause of her nail and bone deformity.
RESULTS: Lab investigations including Parathormone levels, Anti Scl 70, ANA, Anti-CCP and RF levels were all normal. X-ray imaging revealed resorption of the tuft

Ointments are traditionally used in Finland for protection against facial frostbite. Recent epidemiological reports showed unexpectedly, however, that the use of ointments is a statistically considerable risk factor for frostbite of the face and ears. The effects of 4 different emollients on facial temperature were studied in 46 acute cold exposures. The voluntary test persons sat in a cold chamber after emollients were applied thickly on half of the face, while the other half acted as an untreated control. Thermistors and an infrared scanner were used to measure skin temperature of symmetrica

Cyclists are vulnerable to weather and commonly experience problems as they relate to extremes in both heat and cold. Educating cyclists on prevention of sunburn, dehydration, heat illness, hypothermia, and frostbite is the key. Limited sun exposure and proper use of sunscreens are the mainstay of treatment for prevention of sunburn. Adequate fluid and electrolyte replacement is necessary to avoid problems with dehydration and heat illness. Wind- and water-resistant clothing is recommended to protect against hypothermia and frostbite. Adequate training and acclimatization are, of course, recom

We report a 36-year-old woman with complex regional pain syndrome (CRPS) type 1 presenting with extensive skin necrosis of the left arm. The patient cooled her arm with ice packs to ease severe pain due to CRPS, in spite of repeated cautions against frostbite injury. The regions of skin necrosis corresponded with the sites where she had applied ice packs. We considered that the severe skin necrosis in our case was due to a self-induced frostbite injury.

IntroductionCold exposure initially induces peripheral vasoconstriction. After 5-10 min, distal blood vessels will transiently and cyclically vasodilate and vasoconstrict in a phenomenon known as cold-induced vasodilation (CIVD). Increased CIVD response is thought to improve dexterity and confer a reduced risk of frostbite injuries. Current guidelines recommend the use of an intravenous synthetic prostacyclin (PGI2) analog for the treatment of some cases of severe frostbite. This double-blind crossover study investigates the effects of the inhaled PGI2 analog epoprostenol on CIVD response thro

In spite of the fact that extremity frostbites are a widespread type of injuries in many countries, specialized equipment for frostbite treatment is lacking. Treatment is carried out by conservative methods. The best results are obtained by using thermal isolation of injured tissues and activation of internal warming. It is proposed to initiate deep warming of frostbitten extremities on exposure to low-power microwave radiation. A microwave chamber has been developed to implement this technique. The efficiency of the approach was earlier demonstrated on animals. An example is given of successf

As more people take to extreme sporting activities and hobbies, the sports medicine physician should be proficient in early identification and initial sideline or field management of cold weather injuries. There is significant mortality and morbidity associated with these conditions. The most severe of these are hypothermia and frostbite, which have limited evidence for their field management. Nonfreezing cold injuries and chilblains are much rarer, although appropriate prevention and treatment strategies can be used to minimize harm to athletes. This article will provide the most updated reco

Frostbite is a cold injury that results in soft tissue loss and can lead to amputation. Vascular thrombosis following injury causes ischemic tissue damage. Despite understanding the pathology, its treatment has remained largely unchanged for over 30 years. Threatened extremities may be salvaged with thrombolytics to restore perfusion. The authors performed a systematic review to determine whether thrombolytic therapy is effective and to identify patients who may benefit from this treatment. The Pubmed, EBSCO, and Google Scholar databases were queried using the key words "thrombolytics," "frost

For decades, the Wind Chill Temperature Index (WCT) and its various iterations have been used to assess the risk of frostbite on unclothed body parts. This paper presents an innovative knowledge-based Cold Weather Ensemble Decision Aid (CoWEDA) that can be used to guide the selection of the most appropriate cold weather ensemble(s) relative to anticipated mission physical activities and environmental conditions. CoWEDA consists of a validated six-cylinder thermoregulatory model, a database of clothing properties, algorithms for calculating the whole ensemble properties from individual garments

The current standard of care for acute frostbite rewarming is the use of a circulating warm water bath at a temperature of 37 °C to 39 °C. There is no standardized method to achieve this. Manual management of a warm water bath can be inefficient and time consuming. This case describes the clinical use of a sous vide cooking device to create and maintain a circulating warm water bath to rewarm acute frostbite. A 34 year-old male presented to the emergency department with acute frostbite. Each of the patient's feet were placed in a water bath with a sous vide device attached to the side of the b

Report on therapeutic procedures in patients with second and third degree congelations (frostbite) on the feet. Two mechanisms of tissue damage caused by exposure to cold temperature will be discussed pathophysiologically: direct freezing injuries and cell death through intra and extracellular ice crystal formation as well as transient and finally irreversible tissue damage due to decreased perfusion. The condition of decreased perfusion results from persistent vaso-constriction induced by cold temperature, increased blood viscosity, sludge phenomenon and occlusion by platelet thrombi in the m

During cold exposure, peripheral tissues undergo vasoconstriction to minimize heat loss to preserve the maintenance of a normal core temperature. However, vasoconstricted tissues exposed to cold temperatures are susceptible to freezing and frostbite-related tissue damage. Therefore, it is imperative to establish a mathematical model for the estimation of tissue necrosis due to cold stress. To this end, an explicit formula of finite difference method has been used to obtain the solution of Pennes' bio-heat equation with appropriate boundary conditions to estimate the temperature profiles of der

This study examines the phenomenon of focal skin discoloration observed on corpses that died from general hypothermia, outside the areas of classic livor mortis. The authors note that these discolorations are more common in areas with less intense blood flow and less muscle tissue. The severity and number of these discolorations correlate with the intensity and duration of cold exposure, the condition of the clothing, and the degree of alcohol intoxication. A hypothesis on the mechanism of these spots formation is presented, based on the in vivo freezing of blood in superficial vessels with su

OBJECTIVE: Limb frostbite caused by cold exposure is a prevalent injury in winter. However, effective therapies remain limited, particularly for cold-induced edema. This study evaluated the therapeutic potential of DangGuiSiNi Decoction (DSD), QiShenYiQi Pills (QSYQ), and their combination in a rat model of cold-induced limb injury.
METHODS: Male Sprague-Dawley rats (180-220 g) were subjected to hind limb frostbite through 5-min dry ice exposure. Animals were administered with DSD, QSYQ, or their combination via oral gavage either 2 h pre-exposure or 5 min post-exposure, followed by daily dos
