Genetic counseling for patients with birth defects.

Genetic counseling for birth defects is complex because of the etiologic and pathogenetic heterogeneity that exists. It is necessary to arrive at as precise a diagnosis as possible by evaluating gestational history for environmental factors, family history for genetic factors, and patient anatomy for clues to embryologic mechanisms. Recurrence risk counseling is based on a combination of theoretic risk assessment and empiric data. Genetic counseling may be a difficult and stressful task because of the imprecision in these data and the uncertainty inherent in interpreting them. Health professio

[Genetic counseling. Past time, present time, future].

Recent literature concerning the effects of genetic counselling is reviewed. Although the vast increase in our knowledge about the human genome has opened new possibilities for genetic diagnosis, few scientific investigations consider the questions of the social and individual impact. In general, there is an international consensus that genetic counselling is and should be non-directive, and analyses of reproductive decisions following counselling conclude that it seems to reinforce decisions already made by the families, and that objective genetic risks only play a limited role for the decisi

[Genetic counseling, a contribution to family planning].

Since introduction of oral contraceptives "family planning" is mostly understood as birth control. In contrast the gynecologist and the genetic counselor are often confronted with the desire of proconceptive methods. The counselor has different possibilities in family planning: reduction of the age of reproduction; consideration of "safety interval" between the birth of children: a. in families affected with a hereditary disease with late manifestation, b. after exposition to x-rays, cytotoxic or immunosupressant agents; warning against reproduction in case of high genetic risk and missing of

Genetic counselling and genetics of cleft lip and cleft palate.

Modern neonatal care and advanced plastic surgical correction have led to the survival of most newborns with oral clefts. These children are likely to reproduce. A slight increase in the incidence of oral clefts may be expected in the future. The genetics of cleft lip and cleft palate is reviewed. The inheritance is usually multifactorial. With normal parents the risk of having a first affected child with cleft lip is about one per thousand, the risk of having a second affected child 4 per cent and the risk of having a third affected child 10 per cent. If a parent has already a cleft lip, the

Premature ovarian insufficiency: clinical orientations for genetic testing and genetic counseling.

Premature ovarian insufficiency (POI) is a heterogeneous disorder diagnosed in women before 40 years old and describes a wide range of impaired ovarian function, from diminished ovarian reserve to premature ovarian failure. Genetic etiology accounts for 20% to 25% of patients. The evidence that POI can be isolated (nonsyndromic) or part of a pleiotropic genetic syndrome highlights its high heterogeneous etiology. Chromosomal abnormalities as a cause of POI have a prevalence of 10% to 13%, being 45,X complement the most common cytogenetic cause of primary amenorrhea and mosaicism with a 45,X ce

[Genetic counseling for children with abnormal embryonic development of genetic origin].

Until recently, clinical dysmorphology was poorly considered in medicine and human genetics. However, the studies of children affected with multiple congenital anomalies syndromes lead to syndrome identification, prognosis and care for the affected children, and genetic counselling for the families. The identification of developmental genes involved in normal and abnormal morphogenesis leads to a better understanding of genetic causes and embryological development.

Spinal muscular atrophy genetic counseling access and genetic knowledge: parents' perspectives.

Spinal muscular atrophy is characterized by degeneration of alpha motor neurons in the anterior horns of the spinal cord, which leads to progressive symmetrical muscle weakness and atrophy. Spinal muscular atrophy is the leading fatal autosomal recessive disorder in infancy, and genetic counseling is an essential component of the care of families of these patients. However, little guidance is available in the published literature regarding the process and benefit of genetic counseling for families. Accordingly, the authors designed a questionnaire to assess parents' knowledge of the disease, g

The frequency of genetic eye diseases in a genetic counseling center.

The frequency of genetic eye diseases in a genetic counseling center: In this study the incidence of eye diseases of genetic origin in patients attending our genetic counseling center for a period of almost six years is documented. The frequency of retinitis pigmentosa, congenital cataracts, lens dislocation, microphthalmos, retinoblastoma, congenital glaucoma, congenital ptosis, degenerative myopia, strabismus, optic atrophy, various genetic metabolic diseases have been investigated, and the results are presented. Preventive approaches and prenatal diagnostic possibilities are discussed and t

[Genetics and genetic counselling in childhood epilepsy (author's transl)].

According to electroencephalographical studies on the pathogenesis of epilepsy a hereditary predisposition plays a greater role than believed before. It is far more frequent than the manifest disease. The phenomenon of cerebral excitability is influenced by different genetically determined, differently age- and sexdependent but amongst each other independent factors. When defining the risk of epilepsy for siblings and descendents in practice, one is forced to proceed from the statistical empirical value with all its uncertainty. During the past few years, the figures have become more exact for

Who declines genetic counseling? An assessment of genetic counseling declination among individuals participating in a population-wide genomic screening program.

OBJECTIVES: Population-wide genomic screening (PWGS) programs can identify individuals at increased risk for Hereditary Breast and Ovarian Cancer (HBOC), Lynch Syndrome (LS), and Familial Hypercholesterolemia (FH). However, uptake of follow-up genetic counseling (GC) remains inconsistent, and limited data describe why individuals decline these services. This study aimed to characterize patterns of GC uptake and declination within a statewide PWGS program and understand participant-reported reasons for declining GC as documented by study staff. METHODS: We analyzed GC uptake among 603 PWGS par

[Clinics for counseling on cancer genetics. Experiences with genetic studies and counseling on familial breast cancer and colorectal cancer].

Five to ten percent of cases of breast cancer and colorectal cancer are familial. These families can be divided into high-risk families and moderate-risk families. Cancer in high-risk families can often be explained by dominant inheritance of a gene causing increased susceptibility to cancer. There is a great demand for genetic counseling in these families, and the structure of and experiences from a familial cancer clinic at Odense University Hospital is described. The establishment of a familial cancer clinic involves three steps: 1) Identification of families with increased cancer susceptib

Genetic counselling implementation in dilated cardiomyopathy.

Genetic testing has become an integral part of the diagnostic workup of patients with dilated cardiomyopathy (DCM). While the initial goal of genetic testing was to identify family members at risk, recent advances have now extended their relevance to clinical decision-making. Our knowledge of the genetic architecture of DCM has expanded significantly, promoting a shift from the monogenic dogma towards a broader polygenic spectrum. However, current genetic testing strategies still primarily rely on the model of monogenic inheritance with an incomplete penetrance. Large studies have shown a yiel

Genetic counselling in the era of genomic medicine.

BACKGROUND: Genomic technology can now deliver cost effective, targeted diagnosis and treatment for patients. Genetic counselling is a communication process empowering patients and families to make autonomous decisions and effectively use new genetic information. The skills of genetic counselling and expertise of genetic counsellors are integral to the effective implementation of genomic medicine. SOURCES OF DATA: Original papers, reviews, guidelines, policy papers and web-resources. AREAS OF AGREEMENT: An international consensus on the definition of genetic counselling. Genetic counselling

Prenatal genetic counselling for psychiatric disorders.

Psychiatric disorders like schizophrenia, bipolar disorder, depression, anxiety, and obsessive-compulsive disorder are common disorders with complex aetiology. They can exact a heavy toll on the individual with the condition and can have significant impact on family members too. Accordingly, psychiatric disorders can arise as a concern in the prenatal context - couples may be interested in learning about the chance for their child to develop the illness that manifests in the family and may be interested in discussing options for prenatal testing. However, the complex nature of these conditions
