OBJECTIVE: To compare the efficacy of intratympanic gentamicin injection (ITG) on vertigo control, drop attacks, and functional level in Menière's disease patients with and without a history of migraine.
PATIENTS: Menière's disease patients (patients with migraine and age- and sex-matched control patients without migraine) treated from 2002 to 2012 who failed medical management and received ITG, with a minimum 2-year follow up.
INTERVENTION: ITG.
MAIN OUTCOME MEASURES: Vertigo control, drop attack prevalence, and change in functional level.

Intratympanic gentamicin therapy is widely used clinically to suppress the vestibular symptoms of Meniere's disease. Aminoglycosides constitute one of the oldest classes of antimicrobials. Despite their relative toxicity, mainly nephrotoxicity and ototoxicity, aminoglycosides are valuable in current clinical practice. They are bactericidal agents with activity against aerobic gram-negative infections and against gram-positive cocci when added to a cell wall active antimicrobial-based regimen (e.g. betalactams). The aim of this work was to evaluate the interactions between gentamicin and the ethanol extract of the fern Lygodium venustum SW (EELV). The ethanol extract of L. venustum was obtained, the phytocompounds were identified and the EELV was assayed by the checkerboard method with gentamicin against two bacterial strains multiresistant to antibiotics. OBJECTIVE: To investigate the outcomes of intratympanic (IT) gentamicin sulfate injection to treat intractable Ménière's disease (MD) using evidence-based methods.
DATA SOURCE: Data were retrieved from a MEDLINE search (January 1995 through January 2011).
STUDY SELECTION: Selected were English-language articles presenting outcomes of IT gentamicin injection to treat MD that had a prospective study design and an evidence level of II or higher.
DATA EXTRACTION: The database was searched using Boolean combinations of the keywords intratympanic, gentamicin, and Ménière's disease.

OBJECTIVES/HYPOTHESIS: With low-dose and titration protocols, subsequent intratympanic (IT) gentamicin injections are frequently necessary for vertigo control in Ménière's disease. To date, studies have not provided detailed descriptions of the time course of recurrent vertigo and repeated injections.
