BACKGROUND: Labyrinthine Aplasia, Microtia and Microdontia (LAMM) syndrome is characterized by the complete absence of inner ear structures (Michel aplasia), microtia and microdontia. Hypophosphatemic rickets results from defects in the renal tubular reabsorption of filtered phosphate.
CASE CHARACTERISTICS: 13-year-old Indian girl presented with deafness since infancy and progressive wrist widening and genu valgum for last one year.
OBSERVATION: Homozygous novel missense mutation in fibroblast growth factor 3.
MESSAGE: LAMM syndrome and hypophosphatemic rickets may be associated.

The involvement of the lower extremity in acrocephalosyndactyly has received less attention than the craniofacial and upper extremity anomalies. The lower extremity deformities are progressive and require greater attention as the patient grows older. The stiffness of the hips and mild to moderate genu valgum rarely, if ever, require intervention. Foot and toe deformities lead to difficulty with malalignment, painful stresses, and shoe wear. Each of these must be dealt with individually, placing emphasis on the underlying bony abnormality.

In this study, we report on two brothers, born to consanguineous parents, with a syndrome of sensorineural deafness, short stature, cryptorchidism, inguinal hernia, brachycephaly, prominent forehead, flat face, downslanting palpebral fissures, low nasal root, hypoplastic alae and round tip to the nose, low-set prominent ears, narrow thorax, genu valgum, wormian bones, fusion of carpal bones, delayed bone age and congenital clubfeet. This combination of anomalies appears to be a previously undescribed syndrome, with probable autosomal recessive inheritance.

The paper presents the observations on a female patient with congenital cataract, oligophrenia, unsteady gait, low muscular strength, diminished osteotendinous reflexes. The patient has an under normally developed intellect, dysarthria, epileptic crises. Her speaking practice is bad, she also has tropical disturbances, genu valgum, shortening of the fingers of the right hand, kyphoscoliosis. The clinical signs integrate the observations with the Marinescu-Sjogren syndrome. The case is interesting because of the association of the epilepsy with this syndrome.

This, to our knowledge, is the first report of homocystinuria in the South African Negro. Two siblings are presented who developed normally until the age of 5 years but then showed mental retardation. Both were tall and thin, had genu valgum and arachnodactyly. The elder sib also had kyphoscoliosis, pes cavus and bilateral ectopia lentis. The diagnosis was confirmed by the presence of homocystine in the urine, elevated plasma homocystine and methionine and diminished plasma cystine levels.

Nineteen knees were treated for recurrent subluxation or dislocation of the patella by lateral and distal transfer of the vastus medialis with or without release of the lateral retinaculum. Fifty-eight per cent has good or excellent and 42 per cent had poor results after a mean follow-up of twenty-nine months. The factors that predisposed to failure were genu valgum greater than 15 degrees and, to a lesser extent, palpable synovitis associated with symptomatic chondromalacia patellae.

Growth plate stapling using Blount's technique is still under discussion in its application to adolescent's lower limbs axial deformities. To establish its validity, we present a review of 18 stapling cases, 15 for valgus knees and 3 for genu varum. Mean follow up is 5 years 9 months. Focal fibrocartilaginous dysplasia (FFCD) is an uncommon, yet well documented, cause of long-bone angular deformities in children. We report two unusual cases of FFCD involving the tibia: one within the proximal tibia inducing genu valgum and one apparent FFCD lesion of the distal tibia that generated a varus deformity of the distal tibia. To our knowledge, this is the first reported case of FFCD in the distal tibia. Each lesion led to an angular deformity that required surgical management.

A newborn, who had the congenital anomalies including protuberance on the right lower leg, bilateral equinovarus deformity of the feet, genu valgum with knee flexion deformity, syndactyly between the first and the second digit on the right, in addition with the absence of the fifth digit. Echocardiography revealed a secundum type atrial septal defect. The combination of these congenital defects associated with developmental anomalies of lower extremities. The aneurysmal bone cyst (ABC) is well-known for its local expansile growth form and for the high risk of recurrence after treatment. We present the case of a 6‑year-old patient with a large ABC in the metaphysis of the proximal tibia, which showed recurrence only five months after treatment with curettage and autologous bone graft. By using six percutaneous intralesional injections of polidocanol the patient was successfully treated with no recurrence. After 18 months, a mild genu valgum was clinically noted.

A 6 1/2 year old male presented with acrocephaly, brachydactyly, clinodactyly, mild syndactyly of the hands and feet, genu valgum, and marked obesity. Roentgenograms of the hands revealed hypoplasia or aplasia of the middle phalanges. Roentgenograms of the feet revealed hypoplasia of the middle phalanges and deformity of the proximal phalangeal epiphyses of the great toes. Chromosomes studies revealed a normal 46,XY karyotype, and psychological testing revealed low normal intelligence. Current data support autosomal recessive inheritance, although X-linkage cannot be excluded.

One hundred and fourteen patients with primary knee deformity either genu varum, genu valgum or combined varus and valgus deformities were treated with serial plaster cylinder casts and wedging for six to nine months.
