Journal of cosmetic and laser therapy : official publication of the European Society for Laser Dermatology2018eng
BACKGROUND: Drug-induced gingival overgrowth (DIGO) is attributed mainly to the prolonged use of calcium channel blocking agents, anticonvulsants, and anti-calcineurin inhibitors . The management of DIGO depends on the severity of the disease and includes surgical intervention and plaque control. Compared to conventional surgical treatment, the recent literature data have shown that gingivectomy using a high-power laser (HPL) is a short and easy procedure, which results in minimal postoperative discomfort and greater patient accep- tance.

Hereditary gingival fibromatosis (HGF) is a rare condition characterised by severe gingival hyperplasia, which could result in serious aesthetic and emotional problems and functional impairment. Here the present authors report a case of a 28-year-old female patient with generalised severe gingival enlargement covering almost all of the teeth and diagnosed as HGF. Her family history was of significance, since her father and 3-year-old daughter suffered from the same symptoms. Many studies have agreed that surgical removal should be used in the treatment of HGF, and gingivectomy is the most comm

The progressive overgrowth of the gingiva is the hallmark of idiopathic gingival fibromatosis (IGF). Excess gingival tissue can obscure the crown of a tooth, resulting in spaces between teeth, displacement, retention of primary or permanent teeth, and difficulties with feeding, speaking, and appearance. The diagnosis and management of inherited gingival fibromatosis are the focus of this case report. A 12-year-old girl was referred from the Department of Orthodontics to Oral Medicine as a result of progressive gingival enlargement, which impeded orthodontic treatment for misaligned lower front

One of the most challenging problems for orthodontists is that of multiple missing maxillary teeth in a growing patient. In many patients, a good treatment option is autotransplantation. This case report describes the multidisciplinary treatment of an 11-year-old girl with regional odontodysplasia affecting the maxillary right and left central incisors, and congenitally missing maxillary left lateral incisor and canine. Autotransplantation of the mandibular second premolars to the affected area was combined with orthodontic space closure, and the transplanted premolars were reshaped and restor

Gingival enlargement can be multifactorial; it can be due to nutritional deficiency or can be because of several drugs. Poor oral hygiene may further worsen the gingival hyperplasia, because of which the patient may have difficulty in chewing the food, and the speech may get affected. It may end up in psychosocial stigma for the patient as the patient is even neglected by friends or relatives due to poor oral hygiene and horrifying look of gingival overgrowth. Here, we report a case of gingival enlargement in a 22-year-old married female. The enlargement was massive, such that the teeth were b

Background. Hereditary gingival fibromatosis is a fibrotic enlargement of the gingiva. It may exist as an isolated abnormality or as part of multisystem syndrome. This paper reports a case of 16-year-old male with generalized severe gingival overgrowth, involving the maxillary and mandibular arches and covering almost all teeth. Methods. Periodontal management of gingival enlargement included gingivectomy in both arches except in the lower right molar region where flap surgery was done under general anesthesia. After a 2-month followup period, orthodontic treatment was started with fixed appli

BACKGROUND: Drug-induced gingival overgrowth (DIGO) is a well-recognized adverse effect associated with several systemic medications, including calcium channel blockers such as amlodipine. Management becomes particularly challenging in medically complex patients requiring continuous anticoagulant and antiplatelet therapy, in which conventional surgical approaches may increase the risk of perioperative bleeding.
CASE PRESENTATION: We report the case of a 66-year-old male patient with severe, multifocal amlodipine-induced gingival overgrowth associated with generalized severe periodontitis (Sta

Gingival hyperpigmentation in the anterior region in people with a high smile line is an esthetic concern. Gingival depigmentation can be achieved through various procedures. Most techniques have shown successful short-term results; however, recurrence is observed in 50% of patients after 2 to 4 years. A 46-year-old woman presenting with a large black zone of gingival hyperpigmentation in the anterior maxilla was treated with scalpel blade excision. At 25 years after the depigmentation procedure, the patient displayed no recurrence.

A predictable esthetic restoration is not limited to the restored teeth; it has to include the gingival unit and its interface with the teeth involved. Failure to deliver restorations that maintain gingival health jeopardizes the success of any restorative procedure and creates potential for periodontal problems. Perforations, fractures, root resorption, or caries in the cervical area of the tooth, especially in the anterior part of the mouth, present many challenges to the clinician. Failure to place the crown margins on sound tooth material may violate the biologic width and should be consid

A remarkable gingival pigmentation around the restorated teeth was successfully treated by simple surgical removal; incision & coagulasion of deposits by electrosurgery. Before the treatment, a biopsy from the pigmented gingiva was histopathologically examined, and a lot of small pigment deposits were found in the lamina propria with no inflammation, but not in the epitherium. Elements constructing the deposits were also determined by means of an electron probe X-ray microanalyser. Silver was found to be the principal element of the deposits, but not found at all in the crown metal. These resu

Controversy surrounds the importance of keratinized gingiva in maintaining periodontal health. A well-defined animal model system is necessary to evaluate longitudinally the role of keratinized gingiva when plaque control is inadequate or where dental procedures (restorative, prosthetic or orthodontic) alter the periodontal environment. Facial gingiva was excised from eight primary incisors in miniature swine. Contralateral teeth were used as controls. The experimental teeth exhibited mucogingival defects at 3 and 6 month observation periods. The secondary teeth erupting into the experimental

Solid fundamental skills in periodontal diagnosis and treatment constitute the cornerstone of professional development and clinical competence for every dentist, particularly for periodontists. This article highlights the practical significance and urgency of strengthening such fundamental training within the current landscape of oral healthcare. These essential competencies, mandatory for both periodontists and general dentists, include periodontal examination, manual supragingival scaling, subgingival scaling and root planing, ultrasonic instrumentation, as well as basic periodontal surgical

Hereditary gingival fibromatosis (HGF) is an uncommon gingival disease of attached gingiva, which is manifested as localized or generalized form. The HGF inheritance is transmitted through both autosomal dominant and recessive modes. Here, we are discussing a rare case report of an 8-year-old child with gingival fibromatosis in mixed dentition, which caused damage to his speech, mastication, and esthetics and led to significant change in his facial profile. The patient noticed that the gingival enlargement was simultaneous with deciduous dentition eruption and gradually covered entire dentitio

When the clinical crowns of teeth are dimensionally inadequate, esthetically and biologically acceptable restoration of these dental units is difficult. Often an acceptable restoration cannot be accomplished without first surgically increasing the length of the existing clinical crowns; therefore, successful management requires an understanding of both the dental and periodontal parameters of treatment. The complications presented by teeth with short clinical crowns demand a comprehensive treatment plan and proper sequencing of therapy to ensure a satisfactory result. Visualization of the desi

Extensive coronal destruction following trauma or caries presents a recurring challenge in prosthodontic rehabilitation, particularly in the esthetic zone. A multidisciplinary approach combining traditional post-core systems with contemporary surgical adjuncts ensures optimal long-term outcomes. This paper reports the management of a grossly fractured maxillary central incisor using a Richmond crown following diode laser-assisted crown lengthening. A 21-year-old male patient presented with pulpal necrosis and apical periodontitis in tooth 21. After nonsurgical endodontic therapy, post space pr

The main goal of periodontology is to prevent and arrest gingivitis and periodontitis to avoid tooth loss and focal infection of periodontal origin. Periodontal scaling or flap surgery of moderate-to-severe periodontitis have shortcomings, most likely because removal of herpesviruses and bacterial pathogens in deep periodontal lesions and the adjacent inflamed gingiva requires systemic antimicrobial treatment (or gingivectomy). Valacyclovir (1000 mg twice daily on day 1, and 500 mg twice daily on day 2 and on day 3) is a potent anti-herpesvirus agent. Antibiotic combinations against bacterial

BACKGROUND: Traumatic injuries to primary teeth represent one of the most common aetiologic factors of time-related eruption disturbances of permanent teeth. These include premature eruption, delayed eruption or impactions.
CASE REPORT: A case of a 7.5-year-old male with delayed eruption of the maxillary permanent left incisor is presented. The case history revealed a trauma around the age of 3 involving its primary tooth predecessor and its extraction by a dentist about one year later due to frequent abscesses. The clinical examination showed an early mixed dentition and the single presence
