Drug Exposure in Chronic Kidney Disease: It Is Not Just About the Glomerular Filtration Rate.

Chronic kidney disease (CKD) affects over 10% of the world's population and is associated with high morbidity and mortality rates. The management of CKD is complex; CKD alters drug pharmacokinetics and pharmacodynamics and further complicates therapeutic strategies regimens. Uremic toxins accumulate in patients with CKD and significantly impact drug pharmacokinetics and drug responses. These toxins modify drug pharmacokinetics. Indeed, uremic toxins can alter intestinal absorption by affecting drug transporters, such as P-glycoprotein and multidrug resistance-associated proteins. These changes

Change in Dyslipidemia with Declining Glomerular Filtration Rate and Increasing Proteinuria in Children with CKD.

BACKGROUND AND OBJECTIVES: Dyslipidemia, a risk factor for cardiovascular disease, is common in CKD but its change over time and how that change is influenced by concurrent progression of CKD have not been previously described. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: In the CKD in Children study we prospectively followed children with progressive CKD and utilized multivariable, linear mixed-effects models to quantify the longitudinal relationship between within-subject changes in lipid measures (HDL cholesterol, non-HDL cholesterol, triglycerides) and within-subject changes in GFR, pro

Water-soluble vitamins in people with low glomerular filtration rate or on dialysis: a review.

People with low glomerular filtration rate and people on dialysis are spontaneously at risk for vitamin deficiency because of the potential for problems with decreased appetite and decreased sense of smell and taste, leading to decreased intake, and because decreased energy or decreased cognitive ability results in difficulties in shopping and cooking. Imposed dietary restrictions because of their renal dysfunction and because of comorbidities such as hypertension and diabetes exacerbate this problem. Finally, particularly for water-soluble vitamins, loss may occur into the dialysate. We did n

The effects of delapril in combination with indapamide on glomerular filtration rate in elderly hypertensive patients.

We performed a placebo-controlled trial on the effects of a combined antihypertensive treatment with delapril, a new nonsulfhydryl angiotensin-converting enzyme inhibitor, and indapamide, a sulfonamide diuretic. We studied 28 elderly patients aged 65-85 years (mean age, 69 +/- 1 years) who took 30 mg delapril in combination with 1.25 mg indapamide once daily for 24 weeks. In the present study (performed simultaneously with our trial on the effects of delapril/indapamide on left ventricular mass in elderly patients with hypertension and on the same patients), we report the effects of this drug

Serum creatinine is a poor marker of glomerular filtration rate in patients with spina bifida.

Serum creatinine is used to estimate the glomerular filtration rate (GFR). The serum creatinine, however, may not accurately reflect the GFR in spina bifida patients, who often have decreased overall muscle mass resulting from spinal cord abnormalities. The relationship between the serum creatinine and GFR (obtained by [(125)I]iothalamate clearance) was examined in a population of spina bifida patients. Age-matched patients without spina bifida were used as controls. Results demonstrate that, for serum creatinines above 0.5 mg/dL, serum creatinine is a very poor predictor of GFR. Two patients

Effect of cyclosporin A on glomerular filtration rate in children with minimal change nephrotic syndrome.

Cyclosporin A (CyA) is now commonly used in the management of children with steroid-dependent nephrotic syndrome. In order to assess nephrotoxicity related to CyA therapy, we measured glomerular filtration rate (GFR) on 123 occasions in 24 children with minimal change nephrotic syndrome receiving CyA. GFR was estimated from the plasma clearance of 51chromium-EDTA every 3 months during CyA therapy of up to 27 months duration. There was a significant reduction in GFR after 3 months of CyA therapy [118 +/- 33 (SD) to 93 +/- 24 ml/min per 1.73 m2] but no further fall thereafter, although the reduc

The syndrome of hypertension and hyperkalaemia with normal glomerular filtration rate: is there a deficiency in vasodilator prostaglandins?

1. In Gordon's syndrome (GS; a syndrome of hypertension and hyperkalaemia with normal glomerular filtration rate), excessive proximal sodium reabsorption leads to suppression of renin and aldosterone, hyperkalaemia and hyperchloraemic acidosis. 2. Low urinary levels of vasodilator prostaglandins (PG) have been reported in GS, suggesting renal hypoprostaglandinism as a pathophysiological mechanism. 3. In four cases of GS, levels of vasodilator prostaglandins PGE2 and 6-keto-PGF1 alpha were low. 4. In one case of GS, low PGE2 levels were normalized by dietary salt restriction or diuretic therapy

Effect of papillary exposure on intrarenal distribution of glomerular filtration rate and of plasma flow.

Removal of the renal pelvis in order to expose the renal papilla has been shown to cause impairment of the renal concentrating ability by an unknown mechanism. To study this phenomenon, urine osmolality (Uosm), single-nephron glomerular filtration rate (SNGFR) in outer and inner cortical nephrons and the inner and outer medullary plasma flow were determined. Measurements were performed on groups of rats before (control) and 15, 45 and 90 min after exposure of the left renal papilla. Rats with an intact ureter were studied in parallel to see whether the variables varied within the 90-min period

The effect of renal revascularization on decreased glomerular filtration rate in patients with renovascular hypertension.

The effect of revascularization on decreased glomerular filtration rate (GFR) was studied in 18 patients with renovascular hypertension (RVH) for a period of 5 to 74 months after operation. Renal artery stenosis (SRA) was present unilaterally in 3 patients, bilaterally in 13 patients, and in two patients with one kidney. Following surgical correction, the GFR increased or normalized in 6 patients, decreased in 2, and did not change in 10 patients. The blood pressure of almost 90% of the operated patients normalized or improved. The results support the assumption that severe hypertension is ind

Inhibitory effect of methylxanthines on feedback control of glomerular filtration rate in the rat kidney.

Microperfusion experiments were performed in rats to assess the effect of luminal application of theophylline and the more lipophilic 3-isobutyl-1-methylxanthine (IBMX) on feedback regulation of glomerular filtration rate. Elevation of loop of Henle flow from 0 to 40 nl/min caused a 20.3 +/- 4.5% reduction of stop flow pressure (SFP) and a 32.2 +/- 3.7% reduction of early proximal flow rate (EPFR) when the perfusate was a 140 mM NaCl solution. When theophylline was added in a concentration of 5mM SFP fell by only 5.3 +/- 1.8% and EPFR by 7.9 +/- 33%, changes which were significantly smaller th

Development and validation of a new equation to estimate glomerular filtration rate in Argentinian adults.

Latin Americans are underrepresented in the current equations used to estimate glomerular filtration rate (GFR), and the applicability of their predictions to this population may therefore be questionable. The objective of this study was to develop a new equation to estimate GFR based on data from Argentina. A cross-sectional study was conducted. We included 583 Argentinian adults in development sample (DS) and 78 in temporary validation sample (TVS). Urinary iothalamate clearance (reference method), and different predictive variables were used to develop candidate equations to estimate GFR. T

Estimated Glomerular Filtration Rate is not Associated with Alzheimer's Disease in a Northern Ireland Cohort.

BACKGROUND: Alzheimer's disease (AD) prevalence is increasing globally and typically progresses for several years prior to clinical presentation of dementia. Renal dysfunction and vascular disease have been reported in association with dementia in several cross-sectional and longitudinal studies, and may contribute to AD risk. Experimental and observational studies suggest amyloid-β (Aβ) clearance may be impaired in chronic kidney disease (CKD) indicating a mechanism for increased AD risk. OBJECTIVE: The objective of this study was to compare estimated glomerular filtration rate (eGFR) betwee

Demographic and clinical characteristics associated with glomerular filtration rates in living kidney donors.

Due to the shortage of organs, living donor acceptance criteria are becoming less stringent. An accurate determination of the glomerular filtration rate (GFR) is critical in the evaluation of living kidney donors and a value exceeding 80 ml/min per 1.73 m(2) is usually considered suitable. To improve strategies for kidney donor screening, an understanding of factors that affect GFR is needed. Here we studied the relationships between donor GFR measured by (125)I-iothalamate clearances (mGFR) and age, gender, race, and decade of care in living kidney donors evaluated at the Cleveland Clinic fro

Paradoxical mortality of high estimated glomerular filtration rate reversed by 24-h urine creatinine excretion rate adjustment: sarcopenia matters.

BACKGROUND: Muscle wasting may explain the paradoxical mortality of patients with high estimated glomerular filtration rates (eGFRs) derived from equation methods. However, empirical evidence and solutions remain insufficient. METHODS: In this retrospective cohort study, we compared the performance of equation methods for predicting all-cause mortality; we used 24-h creatinine clearance (24-h CrCl), equation-based eGFRs, and a new eGFR estimating equation weighting for population 24-h urine creatinine excretion rate (U-CER). From 2003 to 2018, we identified 4986 patients whose data constitute

Pediatric Reference Ranges for Glomerular Filtration Rate Determined by a Single Injection of 99mTc-DTPA.

Determination of glomerular filtration rate (GFR) with 99mTc-labeled diethylenetriaminepentaacetic acid (99mTc-DTPA) is a validated method to assess renal function, but there are no well-established pediatric reference ranges for GFR. The aim of this study was to establish reference GFRs in children and young adults. Methods: GFR was calculated for 235 reference subjects (median age, 4.86 y; range, 20 d to 24.8 y) using 99mTc-DTPA at Boston Children's Hospital between 2000 and 2020. This included 220 patients for prechemotherapy evaluation and 15 potential kidney donors. GFR was determined usi

The linear association between estimated glomerular filtration rate and diabetic retinopathy: a cross-sectional study.

Diabetic retinopathy (DR) is the predominant microvascular complication of diabetes and the main cause of preventable blindness in working-age adults. Because the retina and kidney share similar microvascular architecture, renal dysfunction-routinely expressed as the estimated glomerular filtration rate (eGFR)-is biologically plausible as a marker of DR risk. However, the relationship between eGFR and DR remains controversial. This study aimed to investigate the association between eGFR and DR prevalence in diabetic patients with type 2 diabetes mellitus. This study represents a secondary anal

Which glomerular filtration rate estimation equations should be used in youth with type 1 diabetes?

BACKGROUND: Diabetic kidney disease (DKD) is the primary cause of kidney failure in type 1 diabetes (T1D). Early identification of subclinical DKD is based on albuminuria and glomerular filtration rate (GFR). However, no GFR equations have been validated in youth with T1D. This study evaluates the performance of GFR equations in youth with T1D. METHODS: We compared a direct measure of GFR (mGFR) via plasma clearance of exogenous inulin, with estimated GFR (eGFR). eGFR was calculated with the following equations: Full-Age Spectrum (FAS-height and FAS-age); revised Lund-Malmö (LM-Rev); European

Glomerular filtration rate measurement during platinum treatment for urothelial carcinoma: optimal methods for clinical practice.

BACKGROUND: We assessed the accuracy of four estimated glomerular filtration rate (eGFR) methods: MDRD, Cockcroft-Gault, CKD-EPI, and Wright. METHOD: The four methods were compared to measure GFR (mGFR) in patients with urothelial urinary tract cancer (T2-T4bNxMx) receiving platinum-based chemotherapy at Rigshospitalet, Copenhagen, from January 2019 to December 2021. Using standardized assays, creatinine values were measured, and mGFR was determined using Technetium-99 m diethylenetriaminepentaacetic acid (Tc-99 m-DTPA) or Cr-51-ethylenediaminetetraacetic acid (Cr-51-EDTA) plasma clearance.
