Previous activation of the soleus Ia afferents causes a depression in the amplitude of the H-reflex. This mechanism is referred to as postactivation depression (PAD) and is suggested to be presynaptically mediated. With the use of a paired reflex depression paradigm (eliciting two H-reflexes with conditioning-test intervals from 80 ms to 300 ms), PAD was examined in a group of healthy individuals and a group of hemiplegic patients. Healthy individuals showed substantial depression of the test H-reflex at all intervals. Although the patient group showed substantially less depression at all inte

The purpose of this investigation was to compare three methods of assessing the excitability of lower motoneurones--TA-reflex, H-reflex and F-wave--in 120 patients with spastic hemiparesis following a stroke. The H-reflex was recorded from the soleus muscle after submaximal electrostimulation of the tibial nerve. The T-Achilles (TA) reflex was recorded from the soleus muscle after percussion of the Achilles tendon. The F-wave was recorded in the distal limb muscles after supramaximal electrostimulation of the median, ulnar, fibular and tibial nerves. The patient's healthy side was used as a co

In normal subjects it was possible to evoke tendon and Hoffman reflexes which were followed by late EMG responses with a latency of 150-250 ms after the reflex stimuli. Analysis of the covariations of reflexes and late responses revealed that muscle spindle sensitivity and strength of the preceding twitch are not essential factors in determining the occurrence of the late responses as opposed to excitability changes within the spinal cord. Inhibition of monosynaptic reflexes and facilitation of late EMG responses to vibration indicate a difference in central pathways. A polysynaptic pathway ma

Ageing is accompanied by numerous changes within the sensory and motor components of the muscle spindle pathway. To further document these age-related changes, this study compared the characteristics of the Hoffmann (H) reflex and M wave, evoked with several pulse durations, between young and old adults. The H-reflex and M-wave recruitment curves were recorded at rest in the flexor carpi radialis of 12 young (21-36 years) and 12 older adults (62-80 years). For each pulse duration (0.05, 0.2 and 1 ms), the maximal M-wave (MMAX ) and H-reflex (HMAX ) amplitude, the M-wave amplitude associated wi

OBJECTIVES: To examine the basis for delay in the excitatory effects of transcranial magnetic stimulation (TMS) of motor cortex on motoneuron pools of muscles left partially-paralyzed by traumatic spinal cord injury (SCI).
METHODS: The effect of subthreshold transcranial magnetic stimulation (TMS) on just-suprathreshold H-reflex amplitude was examined in subjects (n = 10) with incomplete cervical SCI, and in able-bodied (AB) subjects (n = 20) for comparison. EMG activity was recorded from the soleus and the abductor hallucis muscles, and H-reflex was elicited by stimulation of the tibial nerv

Action observation (AO) and motor imagery (MI) are used for motor impairment rehabilitation after neurological disorders, such as spinal cord injuries or strokes. Clinical studies have reported that rehabilitation using AO and MI is effective in restoring motor function. Our previous study showed a difference in the modulation of corticospinal excitability only during action observation (AO) of walking and AO of walking combined with motor imagery (AO + MI). However, it is unclear whether AO and AO + MI can modulate spinal reflex excitability as well as corticospinal excitability. Thus, the pu

OBJECTIVE: To determine the incidence of radiculopathy in diabetic patients neurophysiologically.
METHODS: The H-reflexes were elicited by magnetic stimulation to S1 nerve root at the first sacral foramen (S1 H-reflex) in 40 normal subjects and 41 diabetic patients. The action potentials of the H-reflex were recorded from the soleus muscle. For comparison, a popliteal H-reflex (stimulated at popliteal fossa), and a regular distal sensory and motor conduction of the tibial nerves were simultaneously evaluated.
RESULTS: A significant correlation of the peak interval of H wave and M wave (HMI)

In a recent human study, we found that adaptive changes in step length asymmetry (SLA) are correlated with similar changes in the H-reflex gains of the leg muscles during split-belt treadmill locomotion. Although this observation indicated a closer link between gait asymmetry and spinal reflex adaptation, it did not reveal their causal relationship. To better understand this relationship, here we use a neuromuscular model of human walking whose control relies primarily on spinal reflexes. Subjecting the model to split-belt treadmill locomotion with different combinations of belt speed and refl

A predominance and/or an atrophy of type I fibers and a loss of deep tendon reflexes are often observed in different types of congenital myopathy. Various data indicate that both findings can be linked: dysfunction of the myotatic reflex can induce predominant involvement of type I fibers. In order to specify the mechanism of the loss of tendon reflex, an investigation of the Hoffmann reflex (H reflex) was performed in one case of centronuclear myopathy and in one case of congenital type fiber disproportion with type I hypertrophy. The Achilles tendon reflex was absent but the H reflex showed

Cerebellar transcranial magnetic stimulation (C-TMS) facilitates the ipsilateral soleus H-reflex, which reflects the excitability of the spinal motoneuron pool. This study aimed to investigate whether this facilitation of the spinal motoneuron pool excitability by C-TMS is affected by cerebellar transcranial direct current stimulation (ctDCS) in a polarity-specific manner. Eleven healthy adults participated in this study. The H-reflex was measured from the right soleus muscle by electrical stimulation of the right tibial nerve. The conditioning TMS over the right cerebellum was delivered 110 m

BACKGROUND: Reflex excitability is increased in hemiplegic patients compared to healthy controls. One challenge of stroke rehabilitation is to decrease the effects of hyperreflexia, which may be possible with water immersion. Methods/Aims: The present study examined the effects of acute water immersion on electrically-evoked Hmax:Mmax ratios (a measure of reflex excitability) in 7 hyperreflexive hemiplegic patients and 7 age-matched healthy people. Hmax:Mmax ratios were measured from soleus on dry land (L1), immediately after (W1) and 5 minutes after immersion (W5), and again after five minute

CONTEXT: Irvin M. Korr, PhD, hypothesized that sensitivity of the monosynaptic stretch reflex (ie, deep tendon reflex) plays a major role in the restriction-of-motion characteristic of somatic dysfunction, and that restoration of range of motion through osteopathic manipulative treatment (OMT) could be achieved by resetting of the stretch receptor gain.
OBJECTIVE: To test Korr's hypothesis in the context of Achilles tendinitis, examining whether OMT applied to patients with Achilles tendinitis reduces the strength of the stretch reflex.
METHODS: Subjects were recruited through public adverti

The pathophysiology of postural tremor was studied in 7 patients with cortical cerebellar atrophy, and compared with the responses of 14 healthy control subjects to the same tests. Both tibial nerves were simultaneously and selectively stimulated in the fossa poplitea. EMG was recorded from agonist gastrocnemius muscles and from the antagonistic anterior tibial muscles. Displacement of the centre of foot pressure, inclination of trunk and head in the anteroposterior direction, and the ankle angle were also measured. Patients and controls both exhibit a synchronized discharge in the anterior ti

Changes of the H reflex during sleep were studied in 13 children with cerebral palsy (8 with spastic tetraplegia, 2 with a mixed form of cerebral palsy without spasticity, 3 with hypotonic diplegia or tetraplegia). These modifications were compared with those of 5 normal children of the same age. During repeated night recordings, responses in the calf muscle elicited by electrical stimuli to the posterior tibial nerve were studied at the same time as the EEG, EOG and EMG of the mental muscles. The results show that:--in normal children the max H reflex progressively decreases in amplitude from

OBJECTIVE: Controlled leg-cycling modulates H-reflex activity after spinal cord injury (SCI). Preserved cutaneomuscular reflex activity is also essential for recovery of residual motor function after SCI. Here the effect of a single leg-cycling session was assessed on cutaneomuscular-conditioned H-reflex excitability in relation to residual lower limb muscle function after incomplete SCI (iSCI).
METHODS: Modulation of Soleus H-reflex activity was evaluated following ipsilateral plantar electrical stimulation applied at 25-100ms inter-stimulus intervals (ISI's), before and after leg-cycling in

UNLABELLED: Increasing evidence indicates that the spinal cord is an important site of anesthetic action necessary for surgical immobility. Whether clinical hyper- or hypoventilation affects motor neuron excitability during general anesthesia is unknown. To clarify this issue, we studied seven adult ASA physical status I or II patients undergoing elective surgery. Spinal motor neuron excitability was determined by measuring the posterior tibial nerve H reflex and F wave. The baseline H reflex and F wave were recorded before anesthesia using electrodes placed over the soleus and abductor halluc

Reduced depression of transmitter release from Ia afferents following previous activation (post-activation depression) has been suggested to be involved in the pathophysiology of spasticity. However, the effect of this mechanism on the myotatic reflex and its possible contribution to increased reflex excitability in spastic participants has not been tested. To investigate these effects, we examined post-activation depression in Soleus H-reflex responses and in mechanically evoked Soleus stretch reflex responses. Stretch reflex responses were evoked with consecutive dorsiflexion perturbations d
