BACKGROUND: Treatment strategies for talar avascular necrosis range from nonsurgical management to surgical interventions, including joint-sparing and joint-sacrificing procedures. However, no consensus exists on the optimal treatment approach, particularly in cases requiring ankle arthrodesis.
PURPOSE: This study evaluated whether open ankle arthrodesis and arthroscopic ankle arthrodesis differ in complication rates when used for the treatment of talar avascular necrosis.
STUDY DESIGN: Patient outcomes following ankle arthrodesis procedures were analyzed retrospectively using a national hea

BACKGROUND: Despite carrying significant complication risk, titanium plates remain the standard-of-care in the reconstruction of segmental mandibulectomy defects. Radiation therapy (RT) is known to impair wound healing and cause dose perturbation at the metal-tissue interface. In this study, we sought to investigate the relationship between RT dosimetry and hardware-related complications.
METHODS: A retrospective review of 99 patients who underwent segmental mandibulectomy with osseous free flap and titanium plate reconstruction between 2013 and 2022 was performed. Multivariable logistic and

BACKGROUND: Kirschner wires (K-wires) remain a simple and cost-effective option for clavicle fixation, especially in resource-limited settings. However, migration of K-wires is a rare but potentially fatal complication that can involve critical structures in the thorax, mediastinum, or neck. Preventive measures such as bending, burying, and early removal are essential but often overlooked. CASE PRESENTATION: A 35-year-old male underwent K-wire fixation for a displaced left clavicle fracture. Three months postoperatively, he presented with progressive shoulder pain and restricted motion. Imagin

This case report describes a patient who underwent an ankle fusion using a lateral approach and fibular-onlay strut 14 years ago. She presented with increasing lateral ankle pain and giving way. Despite bracing and steroid injections, she had continued symptoms and surgery for hardware removal was recommended. While under anesthesia, she was found to have subluxation of her peroneal tendons. Operative findings included impingement of the peroneal brevis on the screw head from her fibular-onlay strut and a peroneal tendon tear. Hardware removal, a peroneal tenodesis, and reconstruction of the p

We conducted a study to determine the incidence of syndesmotic injury and stabilization within various ankle fracture patterns along with the rate of syndesmotic implant removal. All data were obtained from a publicly available for-fee healthcare database, the PearlDiver Patient Records Database. Current Procedural Terminology (CPT) codes were used to search the database for cases of fixation of lateral malleolus, bimalleolar, and trimalleolar fractures in addition to syndesmotic fixation and implant removal. Open reduction and internal fixation (ORIF) was performed for an ankle injury in 54,7

BACKGROUND: Pectus bar removal following minimally invasive repair of pectus excavatum (MIRPE) is often considered a minor procedure, but operative difficulty can vary substantially due to fibrotic adhesion and osseous formation at the bar-chest wall interface. The influence of bar configuration on these factors remains insufficiently evaluated.
METHOD: A retrospective cohort study was conducted in patients under 20 years old who underwent elective pectus bar removal between January 2024 and February 2026 in our institution. Patients were categorized into coaxial double bar and non-double bar

STUDY DESIGN: In this retrospective study, the incidence of anterior cervical hardware failure was reviewed in 109 patients with degenerative disorders treated by one surgeon.
OBJECTIVES: To evaluate the risk of injury caused by hardware failure in anterior cervical spine reconstruction.
SUMMARY OF BACKGROUND DATA: Anterior plating is used for stabilization after cervical spine trauma and other conditions of instability. There has been a concern among surgeons about the risks involved when anterior cervical plating fails (fracturing or loosening of the construct).
METHODS: The series includ

BACKGROUND: Posterolateral antiglide plating of unstable AO-type B lateral malleolar fractures is biomechanically stronger than lateral plating and causes less wound healing problems and less frequent hardware removal. However, the distal end of the plate or the screws may cause peroneal tendinitis. The limits of safe hardware placement have not been established.
METHOD: A retrospective analysis of 70 patients was done to determine hardware position and identify peroneal tendon lesions. An adjunct study involved dissection of the retromalleolar region in 10 embalmed cadaver specimens to study

UNLABELLED: This pictorial review presents basic principles of the types of hardware extraction commonly encountered in foot and ankle surgical practice. We review the indications, contraindications and complications of hardware removal including pain, intra-articular fixation, and carcinogenesis, as well as special considerations in pediatric patients and in the setting of infection. Figures are then used to describe the appropriate techniques for use of the screwdriver shafts, conical extraction screws, extraction bolts, hollow reamers, and other instruments found in most hardware extraction

The management of retained orthopedic hardware during amputation procedures presents significant challenges. A case utilizing a modified Gritti-Stokes through-knee amputation to address a periprosthetic fracture with an exposed intramedullary nail (IMN) in a patient with multiple comorbidities is reported. A 47-year-old male with a history of human immunodeficiency virus, hepatitis C virus, and type 2 diabetes mellitus presented with a left femoral periprosthetic supracondylar fracture. The patient had an existing IMN from a previous femoral shaft fracture 20 years prior, along with a left bel

Open reduction with internal fixation (ORIF) of Lisfranc injuries are associated with an increased risk for secondary surgery including hardware removal and salvage arthrodesis. In the current literature, rates of salvage arthrodesis vary due to small sample sizes and a low incidence of Lisfranc injuries. There is little evidence to identify specific surgical and patient-related variables that may result in later arthrodesis. The purpose of this study is to determine the rate of tarsometatarsal joint arthrodesis following Lisfranc ORIF in a relatively large sample size. This retrospective revi

Presence of hardware in juxta-articular location poses challenge during total knee arthroplasty (TKA). When present in distal femur, it precludes the use of an intramedullary femoral jig during TKA often necessitating removal of hardware leading to prolonged surgery, higher risk of complications, and inferior results. We conducted a case-control study to assess the outcome of TKA among patients with post-traumatic arthritis using a handheld navigation system to perform bone cuts allowing retention of hardware in situ. In 15 patients with post-traumatic arthritis and hardware around the knee (G

The treatment of proximal humerus fractures is complex. Factors like fracture pattern, patient age, pre-injury activity level, soft tissue status, and comorbidities play a role in decision-making for non-operative versus operative management. Complications of non-operative vs operative management of proximal humerus fractures include but are not limited to arthrofibrosis, fracture nonunion, and avascular necrosis. We report an unusual case of a 64-year-old female presenting with a three-part proximal humerus fracture dislocation. The patient underwent primary open reduction internal fixation.

The complication rate for an arthrodesis of the first metatarsophalangeal joint remains high. To improve results, we used a complete intraosseous fixation device (IOFIX) with proposed biomechanical advantages. Our hypothesis is that this technique has at least an equal union rate and less hardware irritation compared to other techniques. Seventy procedures were performed in 55 patients. Average follow-up was 24.5 months. All patients returned to follow-up after 6 weeks and were evaluated for union. Fifty-nine feet (84%) completed full follow- up. Union occurred in 62 of 70 feet (88.5%). Eight

INTRODUCTION: Deep brain stimulation has emerged as an effective treatment for movement disorders such as Parkinson's disease, dystonia, and essential tremor with estimates of >100,000 deep brain stimulators (DBSs) implanted worldwide since 1980s. Infections rates vary widely in the literature with rates as high as 25%. Traditional management of infection after deep brain stimulation is systemic antibiotic therapy with wound incision and debridement (I&D) and removal of implanted DBS hardware. The aim of this study is to evaluate the infections occurring after DBS placement and implantable gen

CASE: A 58-year-old woman underwent delayed fixation of an unstable distal radius fracture. At staged hardware removal, an isolated rupture of extensor carpi radialis brevis (ECRB) was discovered and tenodesed to extensor carpi radialis longus acutely. The surgical intervention allowed her to return to gardening at 2 weeks after the hardware removal with preserved function and pain control at 13-month follow-up, respectively.
CONCLUSION: We present a rare complication of isolated abrasive ECRB rupture with an intra-articular distal radius fracture treated with a late decision for surgery. Our

The suture button (SB) device was introduced to negate the need for routine hardware removal in the treatment of syndesmosis injuries. However, a considerable SB removal rate has been reported, and the impact of removal is unknown. This study aimed to evaluate the radiographic and clinical outcomes after removal of SB for syndesmosis fixation. A total of 36 patients who underwent removal surgery after syndesmosis fixation using SB were identified. The mean postoperative time to removal was 12.2 months. On a plain radiograph, tibiofibular clear space (TFCS) was measured and compared at three fo

PURPOSE: Failed femoral neck fracture (FNF) fixation with in situ pinning presents a surgical challenge. Osteoporotic bone, retained hardware, and a typically elderly population magnify the risks of surgery. Here, outcomes of conversion total hip arthroplasty (THA) using two separate incisions in these high-risk patients were examined.
MATERIALS AND METHODS: Medical records for 42 patients with a prior history of FNF fixation who underwent conversion THA with hardware removal between 2009 and 2019 were retrospectively reviewed. Surgery was performed by a single surgeon at a single institution

Surgical treatment usually is indicated for the management of acromioclavicular fracture-dislocations. These are unstable injuries and have shown a high rate of nonunion when managed conservatively. However, surgical strategies often require a second surgery for hardware removal. We describe an arthroscopic-assisted technique to repair the acromioclavicular fracture-dislocation without implants, using a double cerclage and osteosutures. This technique does not require specific instrumentation, avoids clavicle/coracoid drilling, and minimizes secondary irritation related to hardware. This can b
