Comparison of two systems for the video head impulse test (vHIT) for the lateral semicircular canal: description of results from normal and pathological subjects.

BACKGROUND: The video head impulse test (vHIT) is a recent technique for functional evaluation of semicircular canals (SSCs). The vHIT examines eye movements at high frequencies of stimulation and provides an objective assessment of the functioning of the high-frequency domain of the vestibular system. OBJECTIVE: To describe the results from vHIT performed using two systems. METHODS: All subjects were evaluated through an audiological and otoneurological battery of tests and were diagnosed as normal or abnormal by an otorhinolaryngologist. The results from two systems: 1. ICS Impulse (Otomet

Assessment of vestibulo-ocular reflex function in people with Parkinson's disease: a cross-sectional study in a rehabilitation setting using the video head impulse test.

BACKGROUND: Parkinson's disease (PD) is a neurodegenerative, progressive disorder known for motor and non-motor symptoms. The vestibular system, via the vestibulo-ocular reflex (VOR), is crucial for maintaining dynamic gaze stability, and its role in PD is raising interest among researchers. Indeed, vestibular dysfunction in PD may exacerbate postural instability and gait disturbances; however, the prevalence of vestibular dysfunctions remains unclear. This study aims to objectively investigate the VOR function in people with PD using the video head impulse test. METHODS: This is a cross-sect

Real-world use and diagnostic performance of the head impulse, nystagmus, and test of skew examination in acute vestibular syndrome: an Australian tertiary hospital study.

BACKGROUND: The HINTS (head impulse, nystagmus, and test of skew) examination can surpass early magnetic resonance imaging (MRI) in detecting central causes of acute vestibular syndrome (AVS) when performed by neurologists, but its accuracy in routine hospital practice by non-neurologists is uncertain. AIMS: To evaluate the utilisation, documentation, diagnostic accuracy and clinical influence of HINTS in AVS cases at a tertiary Australian hospital and whether combining HINTS with the ABCD2 (age, blood pressure, clinical features, duration, diabetes (clinical risk score)) score improves diagn

Validation of a portable video head impulse test using an iPod touch and an oral stabilization three-dimensional (3D) - printed mount: a method-comparison study.

BACKGROUND: Although video head impulse testing (vHIT) is widely used to assess semicircular canal function, access to quantitative vestibular testing remains limited. Smartphone-based vHIT systems with goggle-based fixation have been described, but larger validation studies and improved fixation stability are needed. We evaluated a portable vHIT application using an iPod touch with an oral stabilization three-dimensional (3D)-printed mount to determine whether it detects laterality in unilateral vestibular hypofunction and is noninferior to a medical-grade device for a patient-level lateralit

Effects of Long-Term Contact Lens Use on Vestibulo-Ocular Reflex Using Rotary Chair and Video Head Impulse Testing.

PURPOSE: This study investigated whether long-term contact lens wear influences vestibulo-ocular reflex (VOR) gain and suppression across a range of stimulus frequencies. Despite frequent use of contact lenses in clinical populations, it remains unclear how optical correction affects VOR assessment outcomes. METHOD: Twelve myopic adults (four men, eight women; 21-25 years) who wore contact lenses at least 12 hr per day, 5 days per week, participated. VOR gain and suppression were measured using sinusoidal harmonic acceleration (SHA) rotary chair testing at 0.04-0.64 Hz, and VOR gain at high f

Associations of video head impulse test and computerized dynamic posturography with self-reported falls and postural control in people with diabetes: an exploratory cross-sectional study.

BACKGROUND: People with diabetes have an increased risk of falls, potentially related to impaired postural control, peripheral neuropathy, and vestibular dysfunction. This secondary fall-focused analysis examined whether video head impulse test (vHIT) and computerized dynamic posturography (CDP) measures were associated with previous self-reported falls in the DIAFALL cohort. METHOD: Fall data were available for 108 participants aged 40-80 years: 11 controls, 49 with type 1 diabetes (T1D), and 48 with type 2 diabetes (T2D). Previously collected vHIT and CDP measures, including Sensory Organis

What does the dissociation between the results of video head impulse versus caloric testing reveal about the vestibular dysfunction in Ménière's disease?

CONCLUSION: It is suggested that the different results of rotational (video head impulse - vHIT) and caloric tests in patients with Ménière's disease (MD) may be a consequence of the physical enlargement of the membranous duct in the hydropic labyrinths in MD, causing a reduced response to caloric stimulation. OBJECTIVES: There have been reports that the results of two tests of semicircular canal function, the caloric response and the responses to vHIT do not agree. This retrospective study at a tertiary referral hospital examined this disagreement. METHODS: This study reviewed the data of 2

Case Report: Could Hennebert's Sign Be Evoked Despite Global Vestibular Impairment on Video Head Impulse Test? Considerations Upon Pathomechanisms Underlying Pressure-Induced Nystagmus due to Labyrinthine Fistula.

We describe a case series of labyrinthine fistula, characterized by Hennebert's sign (HS) elicited by tragal compression despite global hypofunction of semicircular canals (SCs) on a video-head impulse test (vHIT), and review the relevant literature. All three patients presented with different amounts of cochleo-vestibular loss, consistent with labyrinthitis likely induced by labyrinthine fistula due to different temporal bone pathologies (squamous cell carcinoma involving the external auditory canal in one case and middle ear cholesteatoma in two cases). Despite global hypofunction on vHIT pr

Are gain values significantly altered by manual data selection when performing the video Head Impulse Test (v-HIT) on all six semicircular canals with two different v-HIT systems.

BACKGROUND: It has not yet been tested whether averaged gain values and the presence of pathological saccades are significantly altered by manual data selection or if data selection only done by the incorporated software detection algorithms provides a reliable data set following v-HIT testing. OBJECTIVE: The primary endpoint was to evaluate whether the averaged gain values of all six SCCs are significantly altered by manual data selection with two different v-HIT systems. METHOD: 120 subjects with previously neither vestibular nor neurological disorders underwent four separate tests of all

The Spectrum of Peripheral-Vestibular Deficits and Their Change Over Time in CANVAS/RFC1-Related Ataxia Systematic Review and Meta-Analysis of Quantitative Head-Impulse Testing.

Cerebellar ataxia, neuropathy, vestibular-areflexia syndrome (CANVAS) has been linked to biallelic intronic repeat-expansions in RFC1. Video-head-impulse testing (vHIT) offers a quantitative assessment of the angular vestibulo-ocular reflex (aVOR) of all three canals. We evaluated patterns of peripheral-vestibular impairment, its change over time and evaluated correlations with other parameters. PubMed/Embase were searched for articles reporting vHIT in patients with CANVAS/RFC1-related ataxia. A multiple linear-regression model was used to analyse relationships between vHIT-gains and clinical

Improvement of the vestibular functions as measured by video head Impulse test in patients undergoing ventriculoperitoneal shunt surgery for Idiopathic normal pressure Hydrocephalus: Moving Forward in a yet unexplored territory.

INTRODUCTION: Idiopathic Normal pressure hydrocephalus (iNPH) is the most common form of reversible dementia in the elderly. Few were investigated about CSF dynamic disturbances and brainstem functions, though it could be a potentially interesting and yet "unexplored" territory. The objective was to evaluate high-frequency VOR through VHIT before and after ventriculoperitoneal (VPS) shunting. MATERIALS AND METHODS: We investigated a total of 9 patients suffering a confirmed from iNPH diagnosis. Patients underwent video Head Impulse Test (vHIT) before and 10 days after VPS. The VOR measurement

The dissociation between pathological caloric testing and a normal video head impulse test helps differentiate between Menière's disease, vestibular migraine, and other vestibular disorders: a confirmatory study in a large cohort of 2,101 patients.

Vestibular migraine (VM) and Menière's disease (MD) are characterized by episodes of vertigo of similar duration. It is well known that differentiation between both diseases is not always possible based only on the patient history, physical examination, and audiological testing. In addition, the quantification of the vestibular function can also be helpful since, among patients with MD, there is often a dissociation between a normal/pseudo-normal video head impulse test (vHIT) and reduced caloric testing. The goal of this confirmatory study was to determine the sensitivity, specificity, and po
