Strategies To Empower Communities To Reduce Health Disparities.

Community-based participatory research is a promising approach to reducing health disparities. It empowers individuals and communities to become the major players in solving their own health problems. We discuss the use of community-based participatory research and other strategies to enhance empowerment. We also discuss projects from the Centers for Population Health and Health Disparities that have empowered communities to achieve positive health outcomes aimed at reducing disparities. We offer recommendations to policy makers for involving residents in efforts to achieve health equity.

Vaccinations in prisons: A shot in the arm for community health.

From the first day of imprisonment, prisoners are exposed to and expose other prisoners to various communicable diseases, many of which are vaccine-preventable. The risk of acquiring these diseases during the prison sentence exceeds that of the general population. This excess risk may be explained by various causes; some due to the structural and logistical problems of prisons and others to habitual or acquired behaviors during imprisonment. Prison is, for many inmates, an opportunity to access health care, and is therefore an ideal opportunity to update adult vaccination schedules. The tradit

Historical overview of community health practitioners in Korea.

The Korean government introduced CHPs (Community Health Practitioners) as front-line primary health care providers to address the health disparity between urban and rural areas. Through their dedicated contribution over last 30 years, the CHPs have improved Korea's public health through the successful control of high birth rates, a lowered maternal and infant mortality rate in the 1980s, eradication of parasitic infection, and containing many communicable diseases including hepatitis B. However, rapid changes in the health care environment and demands for health care among rural residents have

Community violence and pregnancy health behaviors and outcomes.

OBJECTIVES: Community violence is associated with health-compromising behaviors (HCBs) and adverse general health and pregnancy outcomes. It is unknown whether these effects persist after adjusting for health behaviors. METHODS: Retrospective cohort study of 36,637 pregnancies, 2005-2009, using birth certificate/hospital discharge databases. Community violence rate = (nonaccidental injuries/total injuries) × 100%. ZIP codes were categorized into levels (CVL) by quartiles (lowest, 1, to highest, 4). Outcomes included HCBs (tobacco, alcohol, or drugs), fetal death, preterm birth, and infant sma

District of Columbia Community Health Needs Assessment.

The District of Columbia Healthy Communities Collaborative (DCHCC) represents a unique collaboration among D.C.-area hospitals and federally qualified health centers. In response to its community commitment and Affordable Care Act requirements, DCHCC set forth to conduct a community health needs assessment (CHNA) that can guide decisions about where and how to allocate resources and implement appropriate health interventions for the population it serves. The CHNA described in this article includes analysis of existing demographic, health status, and hospital service use data, as well as hospit

Community health centers: medical homes for children?

OBJECTIVE: To explore medical home attributes of community health centers (CHCs) that provide care to low-income children nationwide compared to other providers for the poor. METHODS: Cross-sectional study of children aged 0 to 17 years in the Medical Expenditure Panel Survey (MEPS; 2003 to 2009) who resided in families living at <200% of the federal poverty level (FPL) and had visits to a primary care setting. CHC visits were defined as a visit to a neighborhood/family health center, rural health clinic, or community health center. Independent measures included provider type, age, gender, ra

Psychosocial factors and health: community and workplace study.

The effects of psychosocial factors on health have drawn growing attention. An important prerequisite for epidemiologic studies is that instruments to measure psychosocial factors be reliable and valid based on psychometric properties. The introduction of occupational stress models has made breakthroughs in conceptualizing real-life complex phenomena in the workplace. This article describes some trials that explore the associations between psychosocial factors and health in the community and workplace. Scales for measuring social support and psychosocial job characteristics were developed, and

Traumatic events and physical health in a community sample.

The relationship of traumatic events to physical health was examined in a randomized community survey (N = 2,364) of Los Angeles residents, 16% of whom had experienced a lifetime traumatic event. This study tested hypotheses that individuals experiencing traumatic events have poorer physical health and that the negative impact of traumatic events on physical health is greater among disadvantaged sociodemographic groups. Regression analyses showed that respondents with a traumatic event history indicated poorer perceptions of their physical health, more chronic limitations in physical functioni

Growth monitoring: the role of community health volunteers.

A community volunteer programme was initiated in rural Jamaica in May 1990. The main aim of the programme was to monitor the growth of children less than 36 months of age through community health volunteers (CHVs) and improve their nutritional status. At the end of the second year the programme was evaluated to determine its effectiveness. The results of the evaluation indicated that almost all (95.6%) of the children were covered by the CHVs. In addition the participation rate was high (78.5%). However, only 50% of the children were adequately covered. Nonetheless, 81% of them gained adequate

Communicating with adolescents in alternative health care sites.

Many adolescents have needs that are not always met in traditional medical settings. A number of sociomedical problems known as the new morbidities have presently emerged as national health-related problems among adolescents. However, in a recent national review, the 15 most frequent reasons for adolescent visits to a hospital or practitioner's office did not include these problems. We know that the major causes of death among adolescents are accidents, homicide, and suicide, and they experience untold mortalities related to teenage pregnancy, alcohol abuse, and drug abuse. Special programs ar

The respiratory health of school children in smelter communities.

This study compares the pulmonary function and prevalence of respiratory symptoms and disorders among school children living in smelter towns and other communities of Arizona. The rate of asthma was found to be significantly higher in whites than Mexican-Americans, although differences in related problems, such as wheezing, were not significant. When the subjects were grouped by area of residence, the children living in areas with relatively high particulate levels had significantly lower pulmonary function than children living in areas with low particulate levels. No relation between symptoms

Air pollution and health in three urban communities.

A health study was conducted in three communities in the greater Montreal region; the first two were characterized by relatively high particulate and sulfur dioxide levels, respectively, and a third community without major industrial pollution. In each community, 300 men and women 45 to 64 yr of age were studied. Residents of the two more polluted communities exhibited a higher prevalence of respiratory symptoms and mean lung function evaluated by various tests was lower than in residents of the less polluted community. When intercity differences in age and smoking were accounted for, no stati

The military health system: a community of solutions for medical education, health care delivery, and public health.

Multiple strategies have been proposed to improve health care in the United States. These include the development of communities of solution (COSs), implementation of patient-centered medical homes (PCMHs), and lengthening family medicine residency training. There is scant literature on how to build and integrate these ideal models of care, and no literature about how to build a model of care integrating all 3 strategies is available. The Military Health System has adopted the PCMH model and will offer some 4-year family medicine residency positions starting in 2013. Lengthening residency trai

Communication partner training for student health and social care professionals engaging with people with stroke acquired communication difficulties: A protocol for a realist review.

BACKGROUND: Stroke acquired communication impairments impede effective communication. Consequently, in stroke care, communicative interactions can be challenging for both patients and staff and can predispose patients to increased risk of preventable adverse events. Communication partner training (CPT) can mitigate such negative outcomes by optimising communicative interactions. Providing CPT to student health and social care professionals (SH&SCPs) has the potential to enhance their clinical expertise and experiences and enhance the future clinical care of patients with stroke acquired commun
