Trends in procedure choice, cost, and health outcomes in infants undergoing Stage 1 palliation for hypoplastic left heart syndrome: a retrospective analysis of the Paediatric Health Information System.

Management of infants with hypoplastic left heart syndrome is resource-intensive. Trends in initial Stage 1 palliation choice and associated hospital cost and outcomes over time are unclear.Using a retrospective cohort of infants <30 days of age (2004-22) from the Paediatric Health Information Systems database, we analysed the annual prevalence of Stage 1 palliation choice, as well as the association between palliation choice and outcomes and resource use. Prevalence of palliation choice was calculated, and Mann-Kendall tests evaluated linear trends. Study outcomes were pooled across years and

Piloting a participatory, community-based health information system for strengthening community-based health services: findings of a cluster-randomized controlled trial in the slums of Freetown, Sierra Leone.

BACKGROUND: Although community engagement has been promoted as a strategy for health systems strengthening, there is need for more evidence for effectiveness of this approach. We describe an operations research (OR) Study and assessment of one form of community engagement, the development and implementation of a participatory community-based health information system (PCBHIS), in slum communities in Freetown, Sierra Leone. METHODS: A child survival project was implemented in 10 slum communities, which were then randomly allocated to intervention (PCBHIS) and comparison areas. In the 5 PCBHIS

Perceptions regarding the ease of use and usefulness of health information exchange systems among medical providers, case managers and non-clinical staff members working in HIV care and community settings.

PURPOSE: The objective of this paper is to describe how members of HIV patients' care teams perceived the usefulness and ease of use of newly implemented, innovative health information exchange systems (HIEs) in diverse HIV treatment settings. Five settings with existing electronic medical records (EMRs) received special funding to test enhancements to their systems. Participating clinics and community-based organizations added HIEs permitting bi-directional exchange of information across multiple provider sites serving the same HIV patient population. METHODS: We conducted in-depth qualitati

Health, social and economic impacts of the UK Treasury Soft Drinks Industry Levy: a synopsis of a systems-thinking informed multi-method evaluation.

BACKGROUND: In March 2016, a tiered tax was announced on industries manufacturing or importing sugar-sweetened beverages in the United Kingdom. The Soft Drinks Industry Levy was implemented in April 2018 with two tiers: £0.18/l on drinks containing ≥ 5 g/100 ml and < 8 g/100 ml of added sugar, £0.24/l on drinks containing ≥ 8 g/100 ml of added sugar, with no levy on drinks containing < 5 g/100 ml of added sugar. The Soft Drinks Industry Levy is a complex intervention that acts as a disruptor in the commercial food system. As such, it demanded an evaluation that went beyond a narrow focus on a

Enhancing the antenatal care quality through incentivized routine health information system in Northwestern Ethiopia: a quasi-experimental study (2020-2021).

BACKGROUND: Antenatal care (ANC) is essential for maternal and newborn health, yet its utilization and quality remain suboptimal in many low-income settings. Evidence on the effectiveness of non-financial incentives to improve ANC is limited. This study examined the effect of a performance-based non-financial incentive (PBNI) intervention on the quality and optimal utilization of ANC in northwestern Ethiopia. METHODS: A quasi-experimental pre-post study with a comparator group was conducted in 2020-2021 involving 1,927 women. The intervention provided non-financial incentives such as smartpho

Patient-Reported Outcomes Measurement Information System (PROMIS) outcomes: Higher odds of adverse mental health when physical function is impaired.

PurposeBalance and vestibular disorders have a profound impact on quality of life. Anxiety (ANX) and depression (DEP) are common with dizziness, vertigo, or imbalance. It is unclear whether self-reported anxiety or depression depends on perceived physical function for individuals participating in vestibular rehabilitation (VPT). We hypothesized that individuals with worse physical function would be more likely to report abnormally high anxiety and/or depression levels.Participants170 individuals referred for vestibular rehabilitation (113 female, 57 males, and age 63.7 (21)).MethodsA retrospec

Piloting the Inclusion of the Key Populations Unique Identifier Code in the South African Routine Health Information Management System: Protocol for a Multiphased Study.

BACKGROUND: The global community has set an ambitious goal to end HIV/AIDS as a public health threat by 2030. Significant progress has been achieved in pursuing these objectives; however, concerns remain regarding the lack of disaggregated routine data for key populations (KPs) for a targeted HIV response. KPs include female sex workers, transgender populations, gay men and other men who have sex with men, people who are incarcerated, and people who use drugs. From an epidemiological perspective, KPs play a fundamental role in shaping the dynamics of HIV transmission due to specific behaviors.

Situational analysis of health systems for ear and hearing care in the World Health Organization (WHO) Eastern Mediterranean Region: A systematic review and evidence synthesis to inform national policies and strategies.

In the Eastern Mediterranean Region (EMR), 78.1 million people experience hearing loss of any degree, with 22.1 million having disabling hearing loss, projected to reach 51.7 million by 2050. Unless global action is taken, the worldwide burden could reach over 700 million people with disabling hearing loss by 2050. This systematic review presents the first comprehensive health systems analysis of ear and hearing care (EHC) in the region. Following PRISMA guidelines, we analysed 146 articles through the WHO health systems framework to identify systemic barriers to effective EHC integration. Our
