Improving access to pulmonary rehabilitation for patients with COPD treated for substance misuse in the London Borough of Islington.

Chronic obstructive pulmonary disease (COPD) is a collection of conditions that cause permanent damage to the lungs. Among a range of treatment options, patients can benefit from pulmonary rehabilitation (PR) programmes involving physical exercises and education. The risk of developing COPD is higher for substance misusers than the general population. Substance misusers with COPD have more severe symptoms and poorer health outcomes than other COPD patients, and experience inequalities in accessing PR services. This project aimed to work with a local substance misuse service to increase the refer

The health of homeless people in high-income countries: descriptive epidemiology, health consequences, and clinical and policy recommendations.

In the European Union, more than 400,000 individuals are homeless on any one night and more than 600,000 are homeless in the USA. The causes of homelessness are an interaction between individual and structural factors. Individual factors include poverty, family problems, and mental health and substance misuse problems. The availability of low-cost housing is thought to be the most important structural determinant for homelessness. Homeless people have higher rates of premature mortality than the rest of the population, especially from suicide and unintentional injuries, and an increased preval

Novel psychoactive substances as a novel challenge for health professionals: results from an Italian survey.

INTRODUCTION: Novel psychoactive substances (NPS; a.k.a. 'legal highs' or 'smart drugs') are advertised online as 'safe' and 'legal' natural/synthetic analogues of controlled illicit drugs. However, only little research has been carried out in identifying the health professionals' knowledge and expertise relating to the intake of these compounds. METHODS: Data presented here refer to the Italian component of the European Union-wide, European Commission-funded, ReDNet project survey. An ad-hoc questionnaire was administered to professionals from the departments of Addiction, Psychiatry, Paedia

Reducing duplicate hand and wrist series in the emergency room: experience at Madigan Army Medical Center.

OBJECTIVE: To demonstrate that repetitive hand or wrist series could be minimized with minor refinements of triage. POPULATION: 2,119 consecutive trauma patients requiring hand or wrist series. METHOD: In baseline phase I, radiographs were ordered by triage corpsmen. In phase II, RNs or physicians performed triage and ordered films. Phase III followed inservice training on strategies to minimize duplicate studies. Phase IV required the RN or physician to provide written justification for duplicate studies. The number of duplications in each phase were compared to baseline using the chi-squar

Opioid Use and Misuse and Suicidal Ideation Among Persons With Spinal Cord Injury: Differential Risk for Former Service Members and Non-Service Members.

INTRODUCTION: People with spinal cord injury (SCI) have an elevated risk of secondary health conditions, including pain, which is often treated with prescription opioids, depression, and suicide. Our purpose was to compare opioid use and misuse between Former Service Members (FSMs) and non-FSMs with SCI, identify risk factors, and assess the differential relationships of opioid use and misuse with suicide ideation (SI). MATERIALS AND METHODS: Data were collected from 1,253 participants with SCI. Using logistic regression, risk factors for SI were evaluated, including interactions between FSM

Characteristics of clients using a community-based drug treatment service ('CAPS-AD') in Brazil: An exploratory study.

BACKGROUND: Substance use is common in Brazil. In order to improve availability of substance misuse care services, over 400 Psycho-Social Care Centres for Alcohol and Drugs (CAPS-AD) - providing community-based care - have been established following mental health care reform (2001). Information on CAPS-AD clients and outcomes is limited. The present study examined select characteristics of local CAPS-AD clients. METHODS: N=143 adult CAPS-AD clients in Ceilândia (suburb of Brasília, Federal District) participated in a 1-week 'snapshot' assessment of service users (February 2015). Following con

Responding to out-of-hours demand: the extent and nature of urgent need.

BACKGROUND: Little research has been undertaken concerning GPs' perceptions about urgent or 'appropriate' out-of-hours demand. OBJECTIVE: We aimed to measure GPs' perceptions about patients' need for urgent out-of-hours general medical help according to indicators of physical, psychological/emotional and social need, and the medical necessity of a home visit. METHODS: Twenty-five practices participated in an audit and research study whereby GPs completed an audit form for all contacts during November/December 1995 and February/March 1996. Each contact was assessed according to the indicators

Victimization experiences, substance misuse, and mental health problems in relation to risk for lethality among African American and African Caribbean women.

The purpose of this study was to investigate the association of intimate partner victimization experiences, mental health (MH), and substance misuse problems with the risk for lethality among women of African descent. Data for this cross-sectional study were derived from a large case-control study examining the relationship between abuse status and health consequences. Women were recruited from primary care, prenatal, or family planning clinics in Baltimore and the U.S. Virgin Islands. Logistic regression was used to generate the study findings. Among 543 abused women, physical and psychologic

Impact of a Medicaid primary care provider and preventive care on pediatric hospitalization.

OBJECTIVE: This study evaluates the impact that a Medicaid managed care program had on avoidable hospitalization, a form of health care misuse that we hypothesize can be reduced by improved access to and quality of primary care in the context of a managed care program. Ambulatory care sensitive (ACS) hospitalizations, a previously defined categorization of hospitalization, as well as all pediatric hospitalizations were also studied. INTERVENTION: The Maryland Access to Care (MAC) was a fee-for-service, gatekeeper, Medicaid managed care program with assigned primary medical providers and requi

Tokenistic co-design: participation without power in Indigenous health evaluations.

OBJECTIVES: Co-design evaluation methodology can be appropriate for Aboriginal and Torres Strait Islander health and wellbeing programs due to its principles of power-sharing and collaboration. We argue, however, that commissioners of evaluations use the term, co-design, uncritically to describe activities that are misaligned with contemporary co-design theory. METHODS: Through qualitative interview data with key stakeholders in the commissioning process (commissioners, evaluators and service providers), we examined perceptions and sentiments towards the use and misuse of co-design in the pro

Frozen section consultation. Utilization patterns and knowledge base of surgical faculty at a university hospital.

The authors studied the knowledge base of surgical faculty concerning frozen section consultations at a university hospital to determine whether it had any relationship to the appropriateness of frozen section requests. To accomplish this, the reasons for performing frozen sections during a 3-month period were analyzed, and those request that seemed ambiguous or inappropriate were identified. Simultaneously a 15-item questionnaire was distributed to faculty and housestaff dealing with factual information concerning the technique and limitations of frozen section diagnosis (Questions 1-8), as w

Delays before Diagnosis and Initiation of Treatment in Patients Presenting to Mental Health Services with Bipolar Disorder.

BACKGROUND: Bipolar disorder is a significant cause of morbidity and mortality. Although existing treatments are effective, there is often a substantial delay before diagnosis and treatment initiation. We sought to investigate factors associated with the delay before diagnosis of bipolar disorder and the onset of treatment in secondary mental healthcare. METHOD: Retrospective cohort study using anonymised electronic mental health record data from the South London and Maudsley NHS Foundation Trust (SLaM) Biomedical Research Centre (BRC) Case Register on 1364 adults diagnosed with bipolar disor

Behaviors and Norms Regarding Sexual Violence and Alcohol Use: How Do Service Members and College Students Compare.

OBJECTIVE: Sexual violence is a significant public health concern in the U.S. and commonly involves alcohol consumption. Risks for both sexual violence and alcohol misuse are elevated among young adults, and integrated prevention interventions are needed to address the intersection of these behaviors. Existing sexual violence and alcohol misuse prevention research often relies on college student samples, but the extent to which results generalize to other at-risk young adult populations remains to be determined. The current study compares sample characteristics, behaviors (sexual violence perp

Development and Evaluation of Machine Learning Models for the Detection of Emergency Department Patients with Opioid Misuse from Clinical Notes.

OBJECTIVES: The accurate identification of Emergency Department (ED) encounters involving opioid misuse is critical for health services, research, and surveillance. We sought to develop natural language processing (NLP)-based models for the detection of ED encounters involving opioid misuse. METHODS: A sample of ED encounters enriched for opioid misuse was manually annotated and clinical notes extracted. We evaluated classic machine learning (ML) methods, fine-tuning of publicly available pretrained language models, and a previously developed convolutional neural network opioid classifier for

Evidence of prescription of antidepressants for non-psychiatric conditions in primary care: an analysis of guidelines and systematic reviews.

BACKGROUND: Antidepressants (ADs) are commonly prescribed in primary care and are mostly indicated for depression. According to the literature, they are now more frequently prescribed for health conditions other than psychiatric ones. Due to their many indications in a wide range of medical fields, assessing the appropriateness of AD prescription seems to be a challenge for GPs. The aim of this study was to review evidence from guidelines for antidepressant prescription for non-psychiatric conditions in Primary Care (PC) settings. METHODS: Data were retrieved from French, English and US guide
