Predictors of maternal health services utilization by poor, rural women: a comparative study in Indian States of Gujarat and Tamil Nadu.

BACKGROUND: India leads all nations in numbers of maternal deaths, with poor, rural women contributing disproportionately to the high maternal mortality ratio. In 2005, India launched the world's largest conditional cash transfer scheme, Janani Suraksha Yojana (JSY), to increase poor women's access to institutional delivery, anticipating that facility-based birthing would decrease deaths. Indian states have taken different approaches to implementing JSY. Tamil Nadu adopted JSY with a reorganization of its public health system, and Gujarat augmented JSY with the state-funded Chiranjeevi Yojana

Integrating oral health into planetary health governance: a framework for resilient and equitable oral healthcare.

Oral health is largely absent from climate-adaptation and planetary-health governance, even though oral health and oral healthcare depend on supplies of clean water, stable electricity, secure supply chains, and a retained workforce, which climate change is destabilizing. In this Perspective, we argue that this omission is a strategic failure for health security, and draw on the contrasting experiences of seven climate-exposed settings (Australia, Fiji, Indonesia, New Zealand, the Solomon Islands, South Africa, and the United Kingdom) to show how environmental stressors disrupt progress in pro

Optimizing Diagnosis and Management of Ventilator-Associated Pneumonia: A Systematic Evaluation of Biofilm Detection Methods and Bacterial Colonization on Endotracheal Tubes.

Healthcare-associated infections, such as ventilator-associated pneumonia and biofilm formation on intubation cannulas, impose significant burdens on hospitals, affecting staffing, finances, and patient wellbeing, while also increasing the risk of patient mortality. We propose a research study aimed at exploring various methodologies for detecting these infections, discovered in the biofilm on medical devices, particularly tracheal cannulas, and understanding the role of each method in comprehending these infections from an etiological perspective. Our investigation also involves an analysis o

[Metabolic imaging--PET, PET/CT--in oncologic surgery].

The availability of PET/CT examinations in Hungary has significantly improved in the past few years. In the three Hungarian centres approximately 10,000 examinations can be performed yearly, which are financed by the national healthcare system. The PET/CT technique using FDG has become an efficient tumour diagnostic method due to its outstanding sensitivity and specificity. Similarly to other medical imaging techniques, PET/CT is a useful and cost-beneficial diagnostic modality in the hand of those clinicians, who clearly know its advantages and limitations. In the present paper the most frequ

From France to Rijeka: The Course and Legacy of First Renal Transplantation in Yugoslavia.

The article analyses the course and legacy of the first successful kidney transplantation in socialist Yugoslavia, performed in Rijeka on 30 January 1971 under the leadership of Prof. Vinko Frančišković. It is based on a combination of secondary literature, newspaper sources, and limited archival documents, thereby approaching this medical event from a historiographical perspective as well. Particular emphasis is placed on the preparations of the Rijeka medical team, which involved international cooperation, especially with France. The procedure itself, carried out as a kidney transplant from

Atrial fibrillation, transient ischaemic attack and stroke in older people. A themed collection in age and ageing journal.

With an increase in the ageing population, there is a rise in the burden of cardiovascular disease. Age and Ageing have compiled collections of their key cardiovascular themed papers. The first Age and Ageing Cardiovascular Collection focussed on blood pressure, coronary heart disease and heart failure. In this second collection, publications since 2011 were selected with emphasis on atrial fibrillation, transient ischaemic attack (TIA) and stroke. The prevalence of TIA and stroke increases as people get older. In this commentary we summarise studies published in Age and Ageing that bring to t

Individual and Societal Economic Burden of Chronic Rhinosinusitis with or Without Nasal Polyps.

Background/Objectives: Chronic rhinosinusitis (CRS) with nasal polyps (CRSwNP) or without NP (CRSsNP) are common upper airway diseases with major impact on healthcare utilization. Little is known about the overall national financial burden of CRS. We aimed to assess the excess financial burden of CRS from a countrywide perspective. Methods: Annual expenditure from healthcare, welfare transfers and foregone income was retrieved from national databases, annualized and compared to matched healthy comparators. Results: Of the 303,475 patients included with CRS (mean age 51, 55% female), 18,142 wer

Cost-effectiveness of Financial Incentives for Patients and Physicians to Manage Low-Density Lipoprotein Cholesterol Levels.

IMPORTANCE: Financial incentives shared between physicians and patients were shown to significantly reduce low-density lipoprotein cholesterol (LDL-C) levels in a randomized clinical trial, but it is not known whether these health benefits are worth the added incentive and utilization costs required to achieve them. OBJECTIVE: To evaluate the long-term cost-effectiveness of financial incentives on LDL-C level control. Infection prevention and control in low- and middle-income countries: policy, practice, and implementation challenges in sub-Saharan Africa.

SUMMARYInfection prevention and control (IPC) programs are essential for safe healthcare; yet, implementation across sub-Saharan Africa remains constrained by chronic underfunding, weak water, sanitation, and hygiene (WASH) systems, limited laboratory capacity, and shortages of trained staff. Healthcare-associated infections (HCAIs) are presumed common, underreported, and often severe, with a disproportionate burden among hospitalized neonates and postpartum women, and they pose substantial risks to healthcare workers during outbreaks. Gram-negative pathogens such as Klebsiella, Escherichia co

Healthcare provider's perspectives on home blood pressure management in Peru and Cameroon: Findings from the BPMONITOR study.

Home blood pressure management, including self-monitoring and medication self-titration, is an efficient and cost-effective tool. Although its use is increasing globally, little is known about the feasibility of such interventions in low and middle-income countries. Further, the perspectives and experiences of healthcare providers who play a big role in ensuring the success of home blood pressure management interventions have not been documented. This qualitative study was conducted with a total of 35 healthcare providers (60% female, mean [SD] age = 37.3 [6.9 years] years), through 4 in-depth

[Geriatric medicine shapes the Future : Recommendations for modern Geriatric Medicine that places older people and their needs at the centre].

Geriatric medicine is becoming increasingly more important in the context of demographic change. Older people frequently suffer from multimorbidity, functional impairments and complex psychosocial conditions that are often insufficiently addressed by a purely diagnosis-oriented medical approach. The primary aim of geriatrics is therefore not only the treatment of diseases but also with respect to individual goals, the preservation of independence, quality of life and social participation. This expert paper outlines the current state of geriatric care in Germany, its scientific evidence base and

Physician maldistribution persisted despite health system capacity expansion in Türkiye, 2002-23.

BACKGROUND: Equitable access is a core dimension of healthcare quality. Türkiye expanded health system financing, workforce, and infrastructure after the early 2000s, but whether national capacity growth was accompanied by more equitable subnational resource distribution remains unclear. This study examined national trends in health system capacity and financing between 2002 and 2023 and compared provincial resource distribution by Human Development Index group. METHODS: We conducted a retrospective observational health services study using aggregated national and provincial indicators from T

Artificial Intelligence: A Mighty Adjunct for Caries Detection.

Articifial intelligence (AI) is impacting many aspects of people's lives today. In fields such as finance, manufacturing, agriculture, insurance, education, and healthcare, AI has been commissioned to cut costs, increase efficiency, and improve accuracy. Like many innovations, the use of AI can have both positive and potentially negative consequences. As an example, if used in dentistry to detect interproximal carious lesions, AI technology does not have the ability (at least not yet) to recognize the individual uniqueness of each patient. For instance, some patients are highly caries prone an

Expanding adult immunization in low- and middle-income countries through pharmacist-provided vaccination.

Adult immunization is increasingly recognized as an essential component of life-course vaccination strategies that improve population health, promote healthy aging, and strengthen pandemic preparedness. Despite recommendations from the World Health Organization (WHO) to expand seasonal influenza vaccination and other adult immunization programs, vaccine coverage among adults in low- and middle-income countries (LMICs) remains substantially below global targets. This shortfall is largely due to insufficient healthcare infrastructure, shortages of trained immunization providers, inadequate finan

Perspectives of cardiac care providers on the continuum of care among women with cardiovascular disease in a tertiary care hospital of South India.

BACKGROUND: Cardiovascular disease (CVD) is a leading cause of mortality among women, and the COVID-19 pandemic disrupted healthcare continuity. This research explores healthcare providers' (HCPs) perspectives on challenges in providing cardiac care to women during the pandemic. METHODS: A qualitative inquiry was conducted through in-depth interviews among 11 HCPs at a tertiary care hospital in South India. RESULTS: The thematic analysis revealed several concepts. Women with CVD were less health literate and often presented late. They relied on family support for care continuity. Family deci
