Congenital rupture of tricuspid chordae tendinea leading to severe tricuspid insufficiency is an extremely rare pathology associated with signs and symptoms of congestive heart failure presenting at birth. If the diagnosis and treatment of this pathology are not made early in life fetal demise may become inevitable. We herein present a neonate with central cyanosis and congestive heart failure due to rupture of an anterior leaflet chordae resulting in severe insufficiency of the tricuspid valve who was treated with appropriate surgery.

Left ventricular free wall rupture is a lethal complication of myocardial infarction. Although emergent surgical repair is the treatment of choice, the method of repair remains highly individualized. This report presents a case of spontaneous coronary artery dissection in a patient with Turner syndrome that led to left ventricular free wall rupture and was successfully repaired on cardiopulmonary bypass using a suture-free technique with the EVARREST Fibrin Sealant Patch.

In a case of anterior septal rupture complicating a transeptal myocardial infarction, the diagnosis of IVC is immediately confirmed by bidimensional Doppler with color coding. The abnormal flow and its three components are studied in multiple section planes. Incidences permitting the best visualization of the shunt are the longitudinal sections: left parasternal and apical-long axis. The parasternal short axis section only visualizes an abnormal intraventricular flow. Sweeping of the septum by caudal tilting of the probe, from an apical section of the 4 cavities, successively studies the three

BACKGROUND: Acute heart failure caused by severe mitral regurgitation (MR) due to papillary muscle rupture has been described in the puerperium by case reports; however, the majority of cases of papillary muscle rupture are caused by myocardial infarction. We describe papillary muscle rupture occurring in the postpartum period in a patient with systemic lupus erythematosus (SLE), antiphospholipid syndrome (APLS), and chronic Libman-Sacks endocarditis and explore the multifactorial nature of the papillary muscle infarction and rupture in the setting of postpartum fluid shifts, chronic myocardia

Congestive heart failure (CHF) with high cardiac output is an uncommon, yet attributable result of non-hemodialysis arteriovenous malformations. While the prevalence of high output heart failure has yet to be determined, it is observably low - specifically when looking at cases of high output heart failure as a result of ruptured abdominal aortic aneurysms (AAA) with fistula formation, an entity that carries a reported incidence of <1% of all complications of AAA. In this report, we present a 64-year-old male with high output heart failure secondary to a ruptured right common iliac aneurysm ca

Aneurysms of sinuses of Valsalva (ASOV) are thin-walled saccular or tubular outpouchings of the aortic sinuses, which can be either congenital or acquired. They can rupture into heart chambers, the pulmonary artery, or the pericardial space (Perloff, Clinical recognition of congenital heart disease, [8]). This report presents a rare case of a patient with treated infective endocarditis who had a patent ductus arteriosus (PDA), a coronary cameral fistula, and a ruptured ASOV (RASOV) into the left ventricle (LV). Successful transcatheter closure of the ruptured ASOV and the other two lesions was

Cardiac injury, including myocardial contusion and valvular damage, is a common complication of blunt chest trauma; however, traumatic ventricular septal rupture is a rare complication. We encountered a rare case of ventricular septal rupture following blunt chest trauma that was successfully repaired by emergency surgery. The mechanism underlying rupture may involve acute compression of the heart between the sternum and the vertebral column when the ventricle is filled, thereby causing a sudden increase in intraventricular pressure and leading to septal rupture. Emergency operation should be

We applied biventricular assist using centrifugal pumps on a 64-year-old male who has undergone an operation against post-myocardial infarction with ventricular septal perforation. The pump flow was maintained at the level of 2.5 l/m2 in CI, and the pulse pressure 30 to 50 mmHg with IAPB. Complete hemostasis has been achieved prior to use heparin to control the patient's ACT around 200 sec. on the third pumping day. The patient's cardiac function improved in a few days at the beginning, but did not recover enough for weaning from the assist devices. He died on the 8th pumping day, and the auto

A 56-year old woman was admitted to the emergency ward after suffering a blunt thoraco-abdominal high-velocity trauma as she hit the wheel when her car drove into a tree. A laparotomy was performed because of haemodynamic instability and radiographic suspicion of a diaphragmatic rupture. Besides haemorrhage from liver and spleen injuries, an abdominal herniation of the heart through a ruptured pericardium and diaphragm was found. Haemostasis of liver, splenectomy and suturing of defects in pericardium and diaphragm resulted in a haemodynamically stable situation. A high index of suspicion of r

Non-penetrating cardiac traumatisms can cause cardiac rupture, myocardial contusion or, rarely, commotio cordis. In cases of rupture of a cardiac cavity, acute and severe cardiac tamponade almost invariably occurs. This paper presents an exceptionally unusual case of non-penetrating cardiac trauma resulting in right atrium rupture contained by the pericardial cavity. A situation of exceptional hemodynamic balance was established with subacute, progressive cardiac tamponade that evolved during three months, presenting gradual right-heart failure instead of the expected acute and severe cardiac

Severe valvulitis owing to acute Kawasaki disease leading to severe mitral regurgitation is a rare event in infants. Further, there is less information about underlying ruptured mitral chordae tendineae causing severe mitral regurgitation. We encountered ruptured mitral chordae tendineae in three female patients after Kawasaki disease. The age at the onset of Kawasaki disease ranged from 3 to 8 months, and detection of ruptured mitral chordae tendineae was from 24 to 90 days. Two patients had acute heart failure, and one was asymptomatic. One patient underwent mitral annuloplasty, and the othe

PURPOSE: Primary mitral regurgitation (MR) resulting from chordal rupture leads to acute volume overload and elevated left atrial pressure. Transcatheter edge-to-edge mitral valve repair (TMVr) devices can effectively reduce MR, but their impact on global cardiac mechanics and diastolic filling remains difficult to predict. This study develops a whole-heart FSI framework for TMVr by extending a previously developed whole-heart model to represent chordal rupture and edge-to-edge repair and to quantify their structural and hemodynamic consequences.
METHODS: A three-dimensional FSI model of the

Myocardial contusion is the most common manifestation of cardiac trauma; the true heart rupture or posttraumatic aneurysms are rare. Pericardial rupture can lead to cardiac strangulation; haemorrhagic pericardial effusion following trauma requires surgical drainage. Constrictive pericarditis occurs rarely after pericardial injury. Valve injury causes always the incompetence: traumatic aortic and mitral incompetence occur with both blunt and penetrating injury. In penetrating heart trauma a surgical revision is almost always necessary, to perform hemostasis and to decompress pericardial tampona

INTRODUCTION: Free wall rupture is a rare complication of myocardial infarction, occurring as early as a few hours after myocardial infarction. It usually happens after transmural myocardial infarction and has a very high mortality rate. The diagnosis of free wall rupture requires echocardiographic evidence of pericardial leakage or pericardial tamponade. The patient's cause of death is often attributed to other causes such as cardiac arrhythmia unless determined by open heart surgery or autopsy. Diagnostic tools for post-infarction cardiac free wall rupture are limited to date.

During 10 years from 1979 to 1988, 99 patients underwent mitral valvular surgery in our clinic. Among 99 patients, we have experienced 3 cases of left ventricular rupture (3.0%). One of 3 patients survived after successful surgical repair. In patient 1, who underwent MVR, type I rupture occurred. The tears were immediately sutured on the beating heart. But 4 hours later, he died due to rerupture. The cause of rupture was suggested the high blood pressure following the trouble of respirator. In patient 2, who underwent OMC and AVR, type II rupture occurred. The cause of rupture was suggested to

OBJECTIVE: Thyroid storm is a life-threatening presentation, with heart failure and tachyarrhythmias being common manifestations. This case highlights that a flail mitral valve from chordae tendineae rupture can be a cause of worsening heart failure and cardiogenic shock in a thyroid storm, albeit a rare complication.
METHODS: We describe a patient who was admitted for a thyroid storm precipitated by pneumonia, who later developed an acute flail mitral valve from chordae tendineae rupture.
RESULTS: A 55-year-old woman with no past medical history was admitted with fever, dyspnea, lower limb

The clinical course and the clinical and pathological factors were studied in a series of 112 in-hospital patients who died of acute myocardial infarction. A necropsy was performed in 101 cases. 44% of the victims were concluded to have been past the possibility of recovery at the time of admission, as the treatment of early ventricular fibrillation or asystole did not lead to a satisfactory result, or as a refractory heart failure or shock dominated from admission to death. Resuscitations due to early ventricular fibrillation were performed in the emergency department mainly on patients livin

Myocardial rupture is the second most common reason for in-hospital mortality in patients with acute myocardial infarction, accounting for 8-17% of deaths. The clinical presentation varies due to the possibility of rupture in three main locations: free left ventricular wall (85%), interventricular septum (10%), and papillary muscle (5%). Hypotension, long persisting or repeated chest pain, syncopes, new heart murmurs or weak action should draw attention to the possibility of myocardial rupture, apart from the classical sign of upper inflow congestion. In about 48% of cases immediate surgical i

Between 1970 and 1981, 12% of patients undergoing mitral valve surgery were found to have chordal rupture. Spontaneous or primary rupture accounted for 74.6% of patients (primary group); in the remainder (secondary group) chordal rupture complicated chronic rheumatic valvular disease (8.9%), bacterial endocarditis on both normal (8.5%) and rheumatic valves (4.7%), ischaemic heart disease (2.3%), acute rheumatic fever (0.5%), and osteogenesis imperfecta (0.5%). Isolated posterior rupture was seen most frequently (54%), with anterior rupture in 36% and rupture of both mitral cusps in 10% of pati
