Fatty Infiltration of Mitral Valve: A Rare Case Report and Review of Literature.

Adipose tissue is a normal anatomical finding in the heart but fat infiltration in cardiac valves is extremely rare with very few cases reported in the literature. We report a case of fatty infiltration in a mitral valve replaced for mitral valve prolapse causing severe mitral regurgitation in a 65-year-old male who presented with shortness of breath and upper respiratory tract infection. The histopathological examination of the mitral valve revealed sheets of mature adipocytes in the spongiosa layer leading to replacement of collagen and elastic fibers. The presence of adipocytes in the prola

Arrhythmias in patients with mitral valve prolapse. Types, evaluation, and therapy.

A wide spectrum of cardiac rhythm and conduction disorders occur in patients with all types of valvular heart disease. However, certain types of valvular disease have a special predilection for arrhythmias, including atrial and ventricular tachyarrhythmias as well as bradyarrhythmias, inherent to the etiology of the condition itself. Most notable in this regard is mitral valve prolapse, in which cardiac dysrhythmia is now recognized as the complication of highest frequency. The principal purpose of this article is the delineation of the characteristics and management of rhythm disorders in the

Aortic laceration due to prolapse of the bicuspid aortic valve: case report.

Reports of aortic regurgitation due to rupture of the aortic valve commissures are rare. Prompt surgical intervention is necessary, as the condition results in rapid, progressive heart failure and subsequent death. We report the case of a 78-year-old man who presented with aortic laceration and cardiac tamponade that was probably induced by prolapse of the bicuspid aortic valve. We speculate that prompt initial surgery may have prevented aortic laceration and cardiac tamponade in this patient. Thus, in order to optimize clinical outcome, clinicians must consider early, precautionary surgical m

Valvular heart disease, infected valves and prosthetic heart valves.

The major causes of systemic embolism from valvular heart disease (mitral, aortic and mitral valve prolapse), prosthetic valves (both mechanical and tissue valves) and infected valves (endocarditis) are reviewed from the standpoint of their incidence and complications. Recommendations for therapy with anticoagulants or other antithrombotic therapy are set forth following the guidelines recently provided by the Second American College of Chest Physicians Conference on Antithrombotic Therapy. Adherence to these recommendations can significantly decrease the risk of systemic embolism in patients

Epidemiology of Valvular Heart Disease.

Acquired diseases of the aortic and mitral valves are the most common cause of morbidity and mortality among Valvular heart diseases. Aortic stenosis (AS) is increasing in incidence in the United States (4,43 US), driven largely by an aging demographic. Aortic valve replacement is the only effective treatment of AS and has a dramatic mortality benefit. Mitral valve regurgitation (MR) is the most common form of valvular heart disease (VHD) in the US, whereby MR is most often the result of mitral valve prolapse; rheumatic heart disease (RHD) is a more common etiology of MR in underdeveloped coun

[Aortic Valve Regurgitation with Dove-coo Murmur: Report of Two Cases].

We performed aortic valve replacement in 2 patients with aortic regurgitation (AR) in whom a "dovecoo" murmur was heard. Here we discuss the valve shape and the origin of the heart murmur. In the 1st patient, echocardiography revealed prolapse of the right coronary valve cusp toward the left ventricular outflow tract during the diastolic phase and fibrillation of the valve leaflet due to the AR jet. In the 2nd patient, surgical findings revealed thickening of the middle part of the leaflet of the right coronary cusp, which caused prolapse of the right coronary valve cusp toward the left ventri

The pathology of mitral valve prolapse.

The gross criteria for diagnosing prolapsing mitral valve are: 1. interchordal hooding of the involved leaflets, 2. hooding or doming of leaflets towards the left atrium, 3. elongation of the involved leaflets resulting in an increase in valve area, 4. dilatation of the valve annulus in patients with severe mitral regurgitation. The posterior leaflet is most frequently affected. The involved leaflets, in general, are thickened, soft, greyish white and have a smooth atrial surface. Chordae tendineae are described as elongated, tortuous and attenuated or thinned. Deviations from normal chordal i

Aberrant, Elevated Papillary Muscle and LV Wall Forces Associated With Mitral Valve Prolapse Are Normalized by Surgical Neochordal Repair but Not by Transcatheter Edge-to-Edge Repair.

BACKGROUND: Recent studies have linked mitral valve prolapse to increased papillary muscle (PM) traction in mitral valve prolapse and MitraClip patients. We aimed to biomechanically compare edge-to-edge repair via MitraClip implantation and neochordal repair for mitral valve prolapse on PM forces ex vivo. METHODS: We developed a force transduction system to measure the forces exerted on the PMs throughout the cardiac cycle in a left heart simulator. Using a randomized, paired procedure with porcine mitral valves (n=8), we measured the hemodynamic parameters and PM forces for control, prolapse

Systolic anterior motion in the absence of asymmetric septal hypertrophy. A buckling phenomenon of the chordae tendineae.

Systolic anterior motion (SAM) of the mitral valve in the absence of asymmetric septal hypertrophy or concentric left ventricular hypertrophy has been reported in several conditions. In this report we describe the clinical and echocardiographic findings in 15 patients who demonstrated SAM without associated organic heart disease (group 1, 10 patients) or in association with mitral valve prolapse (group 2, five patients). Cross-sectional echocardiography revealed the etiology of SAM in both groups to be early systolic anterior angular motion ("buckling") of mitral chordal structures, rather tha

Mitral valve prolapse in the elderly.

This study included 40 patients over 60 years of age with echocardiographic findings of mitral valve prolapse (MVP). Most of these patients were unaware of any cardiac disorder until the time of echocardiography. In the majority, the clinical manifestations were benign, and the duration of symptoms variable. Congestive heart failure (CHF) was noted in 10 patients (25 percent) who were unaware of having any cardiac disorders until the onset of their symptoms. In 5 patients (4 with CHF and 1 with endocarditis), surgical replacement of the prolapsed mitral valve was necessary. Endocarditis was pr

[Serial clinical and echocardiographic evaluation in children with Marfan syndrome].

OBJECTIVE: To describe the clinical cardiac manifestations and temporal evolution of Marfan syndrome in children; to estimate the incidence of annuloaortic ectasia and mitral valve prolapse; and to evaluate tolerability and efficacy of beta-blockers in these patients. METHODS: During one year, 21 children with Marfan syndrome underwent serial clinical and echocardiographic examinations. Echocardiograms assessed: the presence of mitral valve prolapse, aortic root diameter, mitral and aortic valves regurgitation, and aortic enlargement during beta-blocker therapy. Eleven patients had two measur

Frequency and significance of M-mode echocardiographic evidence of mitral valve prolapse in clinically isolated pure mitral regurgitation: analysis of 65 patients having mitral valve replacement.

To assess the reliability of M-mode echocardiographic patterns of mitral valve prolapse (MVP) (echo MVP) in detection of morphologic evidence of MVP (morphologic MVP), operatively excised mitral valves and corresponding M-mode echocardiograms from 65 patients with chronic, severe, isolated, pure mitral regurgitation (MR) were studied. Of the 65 patients, 45 (69%) had echo MVP (either holosystolic or mid-to-late systolic prolapse patterns on preoperative M-mode echograms) and 42 (93%) of them had morphologic MVP; of the 3 without morphologic MVP, 2 had ruptured chordae tendineae from infective

The dynamic cardiac biosimulator: A method for training physicians in beating-heart mitral valve repair procedures.

OBJECTIVE: Previously, cardiac surgeons and cardiologists learned to operate new clinical devices for the first time in the operating room or catheterization laboratory. We describe a biosimulator that recapitulates normal heart valve physiology with associated real-time hemodynamic performance. METHODS: To highlight the advantages of this simulation platform, transventricular extruded polytetrafluoroethylene artificial chordae were attached to repair flail or prolapsing mitral valve leaflets. Guidance for key repair steps was by 2-dimensional/3-dimensional echocardiography and simultaneous i

A Novel Aortic Regurgitation Model from Cusp Prolapse with Hemodynamic Validation Using an Ex Vivo Left Heart Simulator.

Although ex vivo simulation is a valuable tool for surgical optimization, a disease model that mimics human aortic regurgitation (AR) from cusp prolapse is needed to accurately examine valve biomechanics. To simulate AR, four porcine aortic valves were explanted, and the commissure between the two largest leaflets was detached and re-implanted 5 mm lower to induce cusp prolapse. Four additional valves were tested in their native state as controls. All valves were tested in a heart simulator while hemodynamics, high-speed videography, and echocardiography data were collected. Our AR model succe

Mitral valve prolapse in a runner.

In the past decade mitral valve prolapse has become a popular diagnosis. This may be related to the increased utilisation of cardiac ultrasound diagnostic techniques. Although many initial medical articles highlighted patients with complications of mitral valve prolapse, the natural history of isolated mitral systolic click is benign. Complications are sudden cardiac death, symptomatic cardiac arrhythmias, endocarditis, ruptured chordea tendinae, progressive mitral regurgitation, and cerebral ischemic events. However, these occur more likely in patients with MVP and associated mitral regurgita

[Mitral valve plasty for a 91-year-old woman with mitral valve prolapse].

We reported a successful mitral valve plasty for a 91-year-old woman with mitral valve prolapse. She has lived healthfully and independently without a big problem. She was admitted to another hospital for acute heart failure. Echo cardiography revealed prolapse of posterior mitral valve leaflet and severe mitral regurgitation. Drug therapy was not enough to control her complaint In spite of her age, the patient was able to support herself, and she and her family desired to have a surgical treatment. Therefore she referred to our hospital and underwent mitral valve plasty. Post operative course

Aortic insufficiency due to a partial left-coronary aortic valve prolapse and the detection of two small interatrial jets after blunt thoracic trauma.

Aortic valve regurgitation due to blunt thoracic trauma is a rare complication. Autopsy studies have been shown that the aortic valve is the most often lacerated one among the heart valves. Actually, we describe a case of a 47 year old man with the signs of heart failure after a blunt thoracic trauma 2 months before caused by aortic insufficiency due to a partial left-coronary aortic valve prolapse. Furthermore, transthoracic and transesophageal echocardiography revealed two small jets between the left and the right atrium.

[Severe mitral insufficiency caused by the rupture of the chordae tendinae in acromegaly. Report of a case].

Cardiovascular disease is a common finding in patients with acromegaly. In such patients, heart failure frequently leads to death. Cardiovascular manifestations of acromegaly include cardiomegaly and very often hypertension, coronary atherosclerosis, and diabetes. Primary valvular disease is less commonly observed. Because it is not clear whether acromegaly-related cardiomyopathy is a specific entity and since there are not many necropsy reports regarding mitral valve prolapse in acromegalic patients, we report the case of severe mitral regurgitation due to rupture of the chordae tendinae in a
