Predictors and impact of right heart failure severity following left ventricular assist device implantation.

BACKGROUND: Right heart failure (RHF) is a well-known consequence of left ventricular assist device (LVAD) placement, and has been linked to negative surgical outcomes. However, little is known regarding risk factors associated with RHF. This article delineates pre- and intra-operative risk factors for RHF following LVAD implantation and demonstrates the effect of RHF severity on key surgical outcomes. METHODS: We performed a retrospective analysis of consecutive LVAD patients treated at our center between 2008 and 2016. RHF was categorized using the Interagency Registry for Mechanically Assi

Cost comparison of heart transplant vs. left ventricular assist device therapy at one year.

With the worldwide shortage of donor organs, use of ventricular assist device (VAD) therapy is rapidly increasing in both the bridge to transplant and destination therapy settings. However, the high cost of VADs and VAD care is a cause for concern for policy makers who have relied on the limited supply of donor hearts to naturally cap health expenditure on heart transplantation (HTx). We sought to compare costs of the first 12 months of care of VADs vs. HTx. Single center retrospective study utilizing real generated costs over a three yr span from 2010-2012. Only patients with 12 months of cos

The association of pretransplant HeartMate II left ventricular assist device placement and heart transplantation mortality.

Previous United Network for Organ Sharing (UNOS) analysis has shown an increase in posttransplant mortality with pretransplant pulsatile-flow left ventricular assist device (LVAD). Recent studies evaluating continuous-flow LVAD demonstrated improved durability, excellent survival, and improved quality of life. This study investigates the association of preheart transplant continuous-flow LVAD placement and posttransplant mortality using the UNOS database. Heart transplant patients listed after April 2004 (N = 48,090) during the era of HeartMate (HM) II LVAD usage were investigated. Patients wi

Institutional volume of heart transplantation with left ventricular assist device explantation influences graft survival.

BACKGROUND: There are increasing numbers of patients undergoing orthotopic heart transplantation (OHT) with left ventricular assist device (LVAD) explantation (LVAD explant-OHT). We hypothesized that LVAD explant-OHT is a more challenging surgical procedure compared to OHT without LVAD explantation and that institutional LVAD explant-OHT procedural volume would be associated with post-transplant graft survival. We sought to assess the impact of institutional volume of LVAD explant-OHT on post-transplant graft survival. METHODS: This is a retrospective analysis of the Scientific Registry of Tr

Septuagenarians bridged to heart transplantation with a ventricular assist device have outcomes similar to younger patients.

BACKGROUND: Although orthotopic heart transplantation (OHT) is increasingly being offered to older patients, few studies have evaluated outcomes in patients older than 70 years of age. We undertook this study to characterize the outcomes of septuagenarians bridged to transplantation (BTT) in the modern era. METHODS: We conducted a retrospective cohort study of all adult OHT in the United Network for Organ Sharing database from 2005 to 2011. Primary stratification was by age 70 years or older. Subgroup analysis evaluated patients who received BTT. The primary outcome was survival as determined

Preliminary experience with the LionHeart left ventricular assist device in patients with end-stage heart failure.

BACKGROUND: The Arrow LionHeart LVD 2000 left ventricular assist device is the first fully implantable system designed for destination therapy. We report on 2 years of experience with this device, which we implanted for the first time in October 1999. METHODS: Since October 1999, 6 male patients between 55 and 69 years of age (mean 65 +/- 6 years) have received the device at our center; all were in New York Heart Association functional class IV and ineligible for heart transplantation. RESULTS: All surgical procedures were uneventful, with a timely extubation in 5 of 6 patients. Duration of

Short-term bridge to heart transplant using the BVS 5000 external ventricular assist device.

From January 1995 to April 2001, 71 patients with cardiogenic shock using the BVS 5000 were treated or accepted in transfer. Of the 24 transplanted, nine had dilated cardiomyopathy, ischemic cardiomyopathy, acute myocardial infarction, giant cell myocarditis and previous Fontan procedure (group I, n = 13). The others had postcardiotomy shock (group II, n = 11); seven were transferred to our center after device implantation. Age ranged from 8 to 67 years. Ten (77%) patients in group I were implanted without cardiopulmonary bypass. The mean duration of support was 6.7 (2-24) d. Twelve patients w

Reversibility of precapillary pulmonary hypertension and outcomes after heart transplantation bridged with left ventricular assist devices: Insight from the United Network for Organ Sharing.

BACKGROUND: In light of the updated lowered threshold for diagnosing pulmonary hypertension (PH), the reversibility of precapillary PH with left ventricular assist device (LVAD) and the associated post-heart transplantation (HT) outcomes remain unclear. METHODS: Using data from the United Network for Organ Sharing database, we aimed to investigate predictors of persistent precapillary PH in HT recipients bridged with LVAD and examine the interrelated post-HT survival using the updated pulmonary vascular resistance (PVR) cutoff of >2 Wood units for precapillary PH. RESULTS: Among 2169 HT reci

Around and around the merry-go-round: multiple implantations of short- and long-term ventricular assist devices in a patient with severe heart failure.

Multiple implantations of left ventricular assist devices over a period of three years were performed in a 17-year-old gentleman with non-ischaemic dilated cardiomyopathy and congestive cardiac failure. The first device (HeartMate II) was implanted as a bridge to transplantation. However, after few months of support the patient showed signs of myocardial recovery and the device was successfully explanted. After 2 years of uneventful follow-up, the patient's condition deteriorated requiring further mechanical support. Due to the elevated risk associated with a redo full-support LVAD implantatio

Cost-effectiveness of left ventricular assist devices (LVADs) for patients with advanced heart failure: analysis of the British NHS bridge to transplant (BTT) program.

BACKGROUND: A previous cost-effectiveness analysis showed that bridge to transplant (BTT) with early design left ventricular assist devices (LVADs) for advanced heart failure was more expensive than medical management while appearing less beneficial. Older LVADs were pulsatile, but current second and third generation LVADs are continuous flow pumps. This study aimed to estimate comparative cost-effectiveness of BTT with durable implantable continuous flow LVADs compared to medical management in the British NHS. METHODS AND RESULTS: A semi-Markov multi-state economic model was built using NHS

Clinical Impact of Heart Failure Readmissions in HeartMate 3 Left Ventricular Assist Device Recipients.

Heart failure (HF) readmissions remain common and adversely affect morbidity and quality of life in durable left ventricular assist device (LVAD) recipients. This study sought to describe the clinical impact of HF readmissions following HeartMate 3 (HM3) LVAD implantation. We conducted a single-center analysis of patients undergoing HM3 implantation between January 2015 and December 2021. Cumulative incidence functions were used to estimate HF readmission, and multistate models evaluated transitions from LVAD implantation to recurrent HF readmissions and death. Among 273 patients with HM3 impl

Beating the Odds: Successful Thrombectomy Followed By Heart Transplant in a Pediatric Ventricular Assist Device Patient.

Ventricular assist devices (VADs) are increasingly used in pediatric patients with end-stage heart failure as a bridge to heart transplantation (HT). Thromboembolic stroke remains one of the most devastating complications and may jeopardize transplant candidacy. We report a 13-year-old male with dilated cardiomyopathy supported with an Impella 5.5 left VAD who developed an acute ischemic stroke due to left middle cerebral artery occlusion while waiting for transplant. He underwent emergent mechanical endovascular thrombectomy with good immediate neurological improvement. Given high panel react

Incidence and Outcomes of Late Right Heart Failure Following Left Ventricular Assist Device Implantation.

Late right heart failure (LRHF) following left ventricular assist device (LVAD) implantation remains an under-recognized complication with significant impact on patient outcomes. We conducted a retrospective cohort study of 183 consecutive LVAD patients at our institution between 2010 and 2024. LRHF was defined according to the 2020 Mechanical Circulatory Support Academic Research Consortium criteria. Clinical, echocardiographic, and perioperative parameters were analyzed with group comparisons and logistic regression, whereas outcomes were evaluated using Kaplan-Meier and competing-risk analy

Psychiatric Disorders and Associated Factors in Left Ventricular Assist Device Implantation and Heart Transplant Candidates.

INTRODUCTION: End-stage heart failure (ESHF) remains a significant challenge despite optimal treatment, with heart transplantation (HTx) being the gold standard of care. Mechanical circulatory support (MCS) devices such as left ventricular assist devices (LVADs) are increasingly used for temporary or permanent treatment. Psychiatric comorbidities are common in patients with ESHF and may affect treatment outcomes, but the relationship between sociodemographic, clinical, and psychiatric characteristics remains unclear. METHODS: A medical record based, descriptive cross-sectional study was condu

A single-center experience with recovery of heart function in children following ventricular assist device support.

OBJECTIVE: Ventricular assist device (VAD) implantation is an established treatment for infants with severe heart failure (HF), serving as a bridge to heart transplantation or enabling myocardial recovery. This study aims to evaluate the probability of ventricular recovery following VAD support in infants with severe HF secondary to myocarditis. In addition, we analyze clinical experiences focusing on the weaning process, challenges during explantation, and outcomes, with special attention to a small cohort (n = 4) to highlight specific findings. PATIENTS AND METHODS: From 1988 to 2024, 63 co

Prediction of Right Heart Failure After Left Ventricular Assist Device: Opening Pandora's Box.

Continuous-flow left ventricular assist devices (LVADs) represent a leading option in the treatment of end-stage heart failure (HF), provided that right ventricular (RV) contractile function is sufficiently preserved to ensure cardiac output after LVAD implantation. In this context, evaluating the RV before surgery is crucial, as the onset of early right heart failure (RHF) following LVAD placement is linked to increased mortality and morbidity. Unfortunately, the contractile performance of the RV is a difficult issue to evaluate and requires a multimodal approach based on the application of m
