A Preclinical Model of Exertional Heat Stroke in Mice.

Heat stroke is the most severe manifestation of heat-related illnesses. Classic heat stroke (CHS), also known as passive heat stroke, occurs at rest, whereas exertional heat stroke (EHS) occurs during physical activity. EHS differs from CHS in etiology, clinical presentation, and sequelae of multi-organ dysfunction. Until recently, only models of CHS have been well established. This protocol aims to provide guidelines for a refined preclinical mouse model of EHS that is free from major limiting factors such as the use of anesthesia, restraint, rectal probes, or electric shock. Male and female

The use of rectal temperature to monitor heat stroke.

The diagnosis and treatment of heat stroke has relied on rectal temperature monitoring. A twenty-year retrospective review of heat stroke and heat exhaustion victims reveals a 24% mortality and a 33% incidence of overcooling. An experimental protocol using hot and cool baths demonstrates marked rectal temperature lag during periods of rapid body heating and cooling. Auditory canal temperature measurements provide an attractive alternative to rectal temperature measurement during rapid temperature change. Rectal temperature monitoring of heat stroke victims is not recommended. This work suggest

Effect of dantrolene pretreatment on heat stroke in sheep.

Dantrolene is effective in the management of malignant hyperpyrexia. Due to the similarities of malignant hyperpyrexia and heat stroke, we have investigated the effect of dantrolene on heat stroke in sheep. Our study indicates that dantrolene pretreatment decreases the extent of heat stroke signs and some of the induced changes in enzymes and hormones. These results indicate that dantrolene may be a valuable drug in the prophylaxis against heat stroke attack in susceptible individuals.

Heat acclimation defense against exertional heat stroke by improving the function of preoptic TRPV1 neurons.

Rationale: Record-breaking heatwaves caused by greenhouse effects lead to multiple hyperthermia disorders, the most serious of which is exertional heat stroke (EHS) with the mortality reaching 60 %. Repeat exercise with heat exposure, termed heat acclimation (HA), protects against EHS by fine-tuning feedback control of body temperature (Tb), the mechanism of which is opaque. This study aimed to explore the molecular and neural circuit mechanisms of the HA training against EHS. Methods: Male C57BL/6 mice (6-8 weeks) and male TRPV1-Cre mice (6-8 weeks) were used in our experiments. The EHS model

Association between active cooling and lower mortality among patients with heat stroke and heat exhaustion.

Body cooling is recommended for patients with heat stroke and heat exhaustion. However, differences in the outcomes of patients who do or do not receive active cooling therapy have not been determined. The best available evidence supporting active cooling is based on a case series without comparison groups; thus, the effectiveness of this method in improving patient prognoses cannot be appropriately quantified. Therefore, we compared the outcomes of heat stroke patients receiving active cooling with those of patients receiving rehydration-only therapy. This prospective observational multicente

Appropriate implantable cardioverter-defibrillator therapy triggered by heat stroke.

We present a case of a 20 years old male with a history of syncope and Brugada type 1 pattern confirmed by a pharmacological test with flecainide, inducible ventricular tachycardia on an electrophysiological study, and had undergone placement of implantable cardioverter-defibrillator (ICD). In 12 months of follow-up, the patient was asymptomatic; on a day of extreme heat (46°C), he had a presyncope episode followed by an ICD shock. The patient was brought to the emergency room with heat stroke symptoms. ECG showed a Brugada type 1 pattern, and ICD interrogation showed ventricular fibrillation

Myricetin against myocardial injury in rat heat stroke model.

BACKGROUND: Heat stroke-induced mortality is rising across the globe. So, the design of prophylactic and/or therapeutic modalities for heat stroke is pressing need. The common plant derived flavonoid exhibits strong anti-oxidant and anti-inflammatory activities; however, its effects in heat stroke remain unknown. The study aimed to investigate the cardioprotective effects of myricetin on heat stroke induced acute myocardial injury as well as lethality in rats and to explore the underlying mechanisms. METHODS: Myocardial injury was induced by subjecting the anesthetized rats to a high ambient

Acute phase response to exertional heat stroke in mice.

NEW FINDINGS: What is the central question of this study? Exertional heat stroke is accompanied by a marked inflammatory response. In this study, we explored the time course of acute phase proteins during recovery from severe heat stress in mice and the potential role of skeletal muscles as their source. What is the main finding and its importance? Exertional heat stroke transiently increased expression of acute phase proteins in mouse liver and plasma and depleted liver and plasma fibrinogen, a typical response to severe trauma. In contrast, skeletal muscle fibrinogen production was stimulate

Exertional Heat Stroke: Pathophysiology, Epidemiology, Diagnosis, Treatment, and Prevention.

Temperature increases due to climate changes and operations expected to be conducted in hot environments make heat-related injuries a major medical concern for the military. The most serious of heat-related injuries is exertional heat stroke (EHS). EHS generally occurs when health individual perform physical activity in hot environments and the balance between body heat production and heat dissipation is upset resulting in excessive body heat storage. Blood flow to the skin is increased to assist in dissipating heat while gut blood flow is considerably reduced, and this increases the permeabil

Misdiagnosis of exertional heat stroke and improper medical treatment.

Exertional heat stroke (EHS) is the most complicated and dangerous amongst heat injuries that can lead to irreversible injury and even death. Early diagnosis and treatment are therefore crucial determinants of the patient's prognosis. The following case report depicts a soldier who presented primarily with confusion and behavioral changes during physical exercise and later lost consciousness. He was misdiagnosed by the field physician as suffering from supraventricular tachycardia, was treated as such and only diagnosed as suffering from EHS later in the emergency room. Our main aims are: to h

[Psychiatric drugs as risk factor in fatal heat stroke].

Two men aged 33 and 31 years suffered a fatal heat stroke on a warm summer day. One of them used pimozide and clomipramine, the other zuclopenthixol, dexetimide, droperidol, promethazine and propranolol as psychiatric medication. Both of them had a body temperature > 42.3 degrees C, without perspiring. At first only a comatose situation with practically normal laboratory values existed; this was rapidly followed by massive liver damage, disseminated intravascular coagulation, anaemia, thrombopenia and acute renal failure. In spite of adequate and rapid treatment these complications were fatal.

[Febrile coma and disseminated intravascular coagulation following heat stroke].

A clinical case that occurred in a 42 years old female and which fulfils the diagnostic criteria for the entity described as Heat Stroke is presented. In this case, besides the usual manifestations of hyperpyrexia without sweating along with consciousness disorders, features of consumptive coagulopathy, rhabdomyolysis and well marked laboratory liver dysfunction have also been found. The severity of this situation and its less common occurrence in temperate climates, together with the uncertainty in establishing the diagnosis, namely when infection is suspected--in the present case the initial

[Malignant hyperthermia of effort or "heat stroke" (author's transl)].

Heat stroke following effort is not confined to hot regions. The authors have seen five cases in the Paris region between 1967 and 1974. It particularly affects young subjects, in pour training or living away from home. Clinically very similar to anaesthetic malignant hyperthermia, it has the same gravity, with a high mortality rate. It may be characterised by the triad: coma, muscular hypertonicity and hyperthermia of over 40 degrees C. Refrigeration, sedation and rehydration are all the more effective when started early. Improved knowledge of malignant hyperthermia of effort, within the more

What every intensivist should know about exertional heat stroke.

Exertional heat stroke (EHS) is a life-threatening condition caused by strenuous physical activity, leading to severe hyperthermia and central nervous system dysfunction. Unlike classic heat stroke, EHS can occur in young, healthy individuals and in temperate climates, when heat production exceeds the body's ability to dissipate heat. Rising global temperatures and increased participation in endurance events are contributing to a growing incidence of EHS. Early recognition and rapid cooling are essential to prevent multi-organ failure and death. Whole-body cold-water immersion is the gold stan

Prior viral illness increases heat stroke severity in mice.

NEW FINDINGS: What is the central question of this study? We hypothesized that prior illness would increase the susceptibility to and severity of heat stroke (HS). What is the main finding and its importance? We provide the first experimental evidence, using a mouse model of HS, that recent viral illness increases the severity of HS. Our data indicate that this effect is not attributable to the exacerbation of hyperthermia but is a consequence of ongoing coagulation and systemic inflammatory reactions. Our data suggest that measurement of platelets, cytokines and chemokines before heat exposur
