Hematemesis From Downhill Esophageal Varices Secondary to Superior Vena Cava Occlusion in a Patient With a Hemodialysis Catheter: A Case Report.

Upper gastrointestinal bleeding in patients with end-stage renal disease (ESRD) on hemodialysis is a diagnostic dilemma, especially when the cause is unrelated to portal hypertension. One rare but critical etiology is the development of proximal or downhill esophageal varices secondary to superior vena cava (SVC) occlusion. This condition, usually associated with central venous catheters, can lead to severe hematemesis if not properly managed. Although downhill varices are rare, they have been increasingly recognized in dialysis patients, particularly those with long-standing catheters. A 49-y

A rare case of gastro-intestinal stromal tumor presented with hematemesis and severe anemia from a low and middle-income country.

INTRODUCTION AND IMPORTANCE: Gastrointestinal stromal tumors (GISTs) are rare mesenchymal tumors with varied clinical presentations. This case report highlights the significance of recognizing atypical GIST presentations, such as hematemesis, especially in resource-limited settings. CASE PRESENTATION: A 52-year-old male from an economically disadvantaged background presented with hematemesis and severe anemia. Initial investigations suggested iron deficiency anemia, but further evaluation revealed a gastric mass, raising suspicion of GIST. Despite limited resources, a distal radical gastrecto

[Massive hematemesis resulting from stomach rupture caused by pseudoaneurysm of the splenic artery in a patient with chronic pancreatitis. A case report].

The authors report a case of a pancreatic pseudocyst, due to alcoholic chronic pancreatitis, that was transformed into a pseudoaneurysm of the splenic artery as a result of vascular erosion and that manifested itself with massive haematemesis due to spontaneous fistulisation in the stomach. After defining the incidence of the pancreatic disease and of this unusual form of gastric bleeding, particular attention is devoted to the clinical data and to the aetiopathogenic and physiopathological mechanisms involved in the vascular glandular and periglandular damage, outlining the sources and sites

Cryopreserved aortic homograft for in situ replacement of infected thoracic stent graft associated with distal aortic arch rupture and hematemesis.

Endograft infections present a potentially lethal complication of thoracic endovascular aortic repair (TEVAR). We report a case of a young male patient who was referred to our institution because of a stent graft infection that occurred 10 months after TEVAR. Contained distal aortic arch rupture and hematemesis were associated with the endograft infection. Emergent open surgical repair was undertaken with deep hypothermic circulatory arrest. After the removal of the infected endograft, the distal aortic arch and proximal descending thoracic aorta were replaced with a cryopreserved aortic homog

[Case of miliary tuberculosis with esophageal perforation and a tracheal inflammatory polyp secondary to mediastinal lymphadenitis causing massive hematemesis and hemosputum].

A 62-year-old man with a history of left nephrectomy due to tuberculosis was referred to our hospital, because chest radiography showed diffuse miliary shadows in the bilateral lung fields, and acid-fast bacilli were detected from his hemosputum after steroid therapy for fever of unknown origin. Chest computed tomography showed mediastinal lymph node enlargement with partial calcification of these lymph nodes together with the presence of air. He was diagnosed with miliary tuberculosis and tuberculous mediastinal lymphadenitis and anti-tuberculosis drug therapy was started. Massive hematemesis

A Low-Cost Cocoa-Opacified Hematemesis Simulant for Manikin-Based Airway and Endoscopic Visualization Training: Formulation Rationale, Targeted Review, and Cost Analysis.

High-fidelity manikin scenarios for contaminated airway management and upper gastrointestinal bleeding (UGIB) depend on blood or hematemesis simulants that reliably occlude optics and reproduce the visual gestalt of real blood under high-intensity video laryngoscopy and flexible endoscopy lighting. Commercially available simulated blood products are frequently optimized for reusability, stain resistance, and compatibility with procedural trainers, and can be cost-prohibitive for repeated high-volume "soiling" curricula. However, many simulated blood products do not report optical opacity or ca

Association between duration of alcohol consumption and risk of hematemesis in patients with alcoholic cirrhosis complicated by esophagogastric variceal bleeding: a case-control study.

OBJECTIVE: To investigate the correlation between drinking patterns and the occurrence of hematemesis in patients with alcoholic cirrhosis complicated by esophagogastric variceal bleeding (EVB). METHODS: This retrospective study included 182 patients with alcoholic cirrhosis complicated by EVB, admitted to the General Hospital of Northern Theater Command of the Chinese People's Liberation Army, from January 2013 to January 2023. Patients were classified into the hematemesis group (n = 130) and the non-hematemesis group (n = 52). Baseline characteristics, drinking patterns, and laboratory para

Squamous cell carcinoma-induced gastric conduit-thoracic aorta fistula presenting with massive hematemesis 4 years after radical esophagectomy: a CARE guidelines-compliant case report.

INTRODUCTION: Gastric squamous cell carcinoma (GSCC) arising within a gastric conduit is an exceedingly rare phenomenon, and its presentation as a gastro-aortic fistula has never been documented. This case highlights the diagnostic challenges and life-threatening potential of delayed aortoenteric complications after esophagectomy, and underscores the evolving role of endovascular therapy in emergent hemorrhage control. PATIENT CONCERNS AND CLINICAL FINDINGS: A 75-year-old male presented with 6 h of recurrent, high-volume hematemesis and presyncope. On arrival, he was hypotensive (74/50 mmHg),

Bleeding pattern before admission as guideline for emergency endoscopy.

In a prospective study of 539 patients admitted because of hematemesis and melena the bleeding pattern before admission was compared with the findings obtained on emergency endoscopy and the subsequent clinical course. Ranked in order of prognostic importance, red hematemesis with melena, black hematemesis with melena, and red hematemesis alone increased the probability of massive hemorrhage. Moreover, black hematemesis with melena was the superior predictor of bleeding ulcer, the commonest lesion carrying the risk of massive hemorrhage. In contrast, in patients with melena or black hematemesi

An 18-Year-Old Man with Idiopathic Non-cirrhotic Portal Hypertension.

Non-Cirrhotic Portal Hypertension (NCPH) is a rare cause of hematemesis and melena. Like in cirrhotic patient, hematemesis in NCPH patient was caused by rupture of esophageal varices. But unlike in cirrhotic patient, in NCPH there are no sign of liver failure, because liver physiology is still normal. We reported case of male patient with NCPH that had hematemesis because of rupture of esophageal varices.

Life-threatening bleeding from gastrointestinal stromal tumor of the stomach.

Here, we report on two patients with hemorrhagic shock due to hematemesis from a gastrointestinal stromal tumor (GIST) of the stomach. Patient 1 was a 64-year-old woman who was admitted to our hospital because of syncope due to hemorrhagic shock resulting from massive hematemesis. Emergent upper gastrointestinal (GI) endoscopy revealed a 5-cm-diameter submucosal tumor on the lesser curvature of the lower gastric body. In addition to the central ulceration of the tumor, a Dieulafoy-like lesion was present. Neither lesions showed active bleeding at the time of observation. Because the patient co

Postmortem diagnosis of massive gastrointestinal bleeding in a patient with aberrant right subclavian artery-esophageal fistula.

CASE: Aberrant right subclavian artery-esophageal fistula is a rare, but fatal, complication. A 55-year-old febrile man with a nasogastric feeding tube developed sudden massive hematemesis and shock. OUTCOME: Upper endoscopy revealed an intragastric hematoma with no active bleeding observed except for oozing from an esophageal tear. Enhanced computed tomography scan detected aberrant right subclavian artery but was unable to determine the bleeding source. Repeat endoscopy carried out on day 2 confirmed hemostasis and the disappearance of the intragastric hematoma. However, the patient suddenl
