[Multiple brown tumours in a patient with tertiary hyperparathyroidism].

We describe the case of a 49-year-old female with chronic renal failure and secondary hyperparathyroidism that not dissapeared after kidney transplant and turned into an autonomous form (tertiary hyperparathyroidism). The xRays and CT showed the presence of multiple brown tumors in iliac bones and tibia. We performed a parathyroidectomy removing a lower left adenoma confirmed in the histological exam the renal. After the surgery the osteoarticular manifestations improved and lithiasis dissapeared. There were not any postoperative complications such as hypocalcemia or recurrential palsy althoug

X-ray microanalytical detection of iron in pigmentation of oral mucosa.

A patient was referred to the Charles Clifford Dental Hospital with a grey metallic pigmentation of the hard palate. Conventional histopathological examination was inconclusive, suggesting the presence of either an ephelis (freckle) or pigmentation resulting from a stainless steel upper denture. Material from the pathological specimen was dewaxed and reembedded in Spurr's resin. Examination in the TEM revealed electron dense deposits in the cytoplasm of macrophages. Energy dispersive X-ray microanalysis demonstrated that these inclusions contained iron. The results suggested that the iron was

Renal hemosiderosis and rapidly progressive glomerulonephritis associated with primary hemochromatosis.

Hereditary hemochromatosis leads to the accumulation of iron in many organs including the liver, spleen and heart and results in injury and dysfunction of these organs. On the other hand, iron accumulation and functional impairment in kidney is extremely rare. We report a 61-year-old male patient with hereditary hemochromatosis, in whom the renal function was deteriorated rapidly. Renal biopsy revealed crescentic glomeruli and hemosiderin accumulation in tubular epithelial cells.

Histopathologic findings in surgical specimens of endolymphatic sac in Meniere's disease.

Two types of abnormality in the endolymphatic sac acquired at shunt surgery are presented. One was deep brown pigmentation in the subepithelial connective tissue. The pigment did not react to Prussian blue, thus ruling out hemosiderin. The other, in seven out of eight specimens, was the perisaccular fibrosis. The fibrotic foci in four cases were assumed, from the findings under ordinary illumination and polarized light microscopy, to be reorganized scar tissue.

Comparison of the bone marrow composition in samples from rib, ilium, sternum, and humerus of dogs.

Bone marrow (BM) samples in dogs are typically acquired from hematopoietically active trabecular BM at appendicular skeletal sites, such as the proximal humerus and femur, or axial skeletal sites, such as pelvis, sternum, and rib. Suitability for in vivo biopsy varies by site, but equivalency of BM composition at different sites is uncertain. We sought to determine whether the composition of BM is comparable in humerus, ilium, rib, and sternum. Samples from each site were obtained within 24 h after death from 30 dogs with spontaneous disease submitted for postmortem examination. Tissues were f

BILATERAL LOCALIZED PIGMENTED VILLONODULAR SYNOVITIS OF THE KNEE: CASE REPORT AND REVIEW.

OBJECTIVES: Several cases of bilateral diffuse pigmented villonodular synovitis (PVNS) or tenosynovial giant cell tumor have been described in the literature. Nevertheless, some presentations are rare and differential diagnoses are necessary. METHODS: The purpose of this study was to perform a systematic review of the literature related to PVNS and to report a rare supra-patellar bilateral and focal presentation. We performed a systematic data review in the Pubmed Clinical Queries database using MeSH and keywords related to PVNS and tenosynovial giant cell tumor. RESULTS: Two cases of bilate

Hemochromatosis in Salers cattle.

Two 2-year-old Salers cattle from different herds raised on pasture were evaluated for retarded growth and diarrhea. Increase of liver enzyme activities and prolonged sulfobromophothalein (BSP) half life (T1/2) indicated liver disease with impaired liver function. Histopathologic examination of liver biopsies revealed a micronodular cirrhosis with marked deposition of hemosiderin in hepatocytes, Kupffer cells, and arterioles. Transferrin saturation (TS) and liver iron content were markedly increased, consistent with a diagnosis of hemochromatosis. Both animals were euthanatized due to deterior

Immunohistochemical evidence for a lack of ferritin in duodenal absorptive epithelial cells in idiopathic hemochromatosis.

Patients with idiopathic hemochromatosis exhibit an unexplained increase in intestinal iron absorption. The aim of this work was to study immunohistochemical H- and L-ferritin distribution in duodenal mucosal cells of patients with idiopathic hemochromatosis, and of subjects with various degrees of iron loading. Biopsy sections of gastrointestinal mucosa from 24 patients with idiopathic hemochromatosis, 10 patients with secondary iron overload, 6 normal subjects, and 13 iron-deficient subjects were analyzed with monoclonal antibodies for the presence of immunohistochemical H and L ferritin typ

[Localized pigmented vellonodular sunovitis, cause of knee hemarthrosis].

The pigmented vellonodular sinovitis (PVNS) is benign neoplasm with synovial proliferation and hemosiderin deposit, characterized by large compromising joints, especially the knee. At present, two variants of clinics, the diffuse form (PVNSD) and the localized (PVNSL) are described. Arthroscopic synovectomy and radiosynoviorthesis (RSO) is the treatment that has shown the best functional results. Nuclear magnetic resonance is an appropriate method for conducting the diagnosis of PVNSL.

Evidence for preganglionic pupillary involvement in superficial siderosis.

A 36-year-old man presented with spinal myoclonus, ataxia, hearing loss, and unilateral pupillary dilation. MRI demonstrated hemosiderin deposition along the superficial surfaces of the brain, brainstem, cerebellum, and spine. The pupillary changes were localized to the preganglionic oculomotor nerve. In contrast to vasculopathic oculomotor nerve palsies, superficial siderosis may cause selective involvement of the superficially located pupillary fibers.

Cutaneous idiopathic hyperchromia of the orbital region (CIHOR): a histopathological study.

BACKGROUND: Dark circles under or around the eyes is a cosmetic problem for a large number of people. It is a condition of unknown etiology characterized by the darkening of the eyelids and periorbital skin. The aim of this study was to determine histopathological changes associated with cutaneous idiopathic hyperchromia of the orbital region (CIHOR). METHODS: Twenty-eight adult patients with CIHOR were consecutively selected for the study. Biopsy specimens were taken from the darkened skin of the eyelid and from the normal retroauricular skin as a control. RESULTS: Hemosiderin was absent in

Histologic evidence of repetitive blunt force abdominal trauma in four pediatric fatalities.

In cases of acute fatal child abuse, certain injuries, including cutaneous blunt force trauma, skull fractures, subdural hematomas, intra-abdominal hemorrhage, and retinal hemorrhages are common and well described in the pediatric and forensic literature. These gross findings at autopsy, when taken into consideration with scene investigation and interviews with caregivers, may indicate both a clear manner and cause of death. In such cases, the discovery of additional pathologic changes attributable to older abusive injuries helps support a conclusion of death due to inflicted trauma. We discus

Correlations between extent and spread of adenomyosis and clinical symptoms.

OBJECTIVE: To correlate the histopathology of adenomyosis particularly the depth and spread of adenomyosis and symptomatology. STUDY DESIGN: Medical records of 94 patients who had undergone a hysterectomy and who were found to have adenomyosis on histopathologic examination were reviewed and histopathologic slides were reexamined. The symptoms were correlated with the presence of adenomyosis, the depth of penetration, and the spread of adenomyosis foci. RESULTS: Specimens were stratified according to the degree of adenomyosis penetration into 4 groups: group A consisted of specimens with ade

Atypical low-signal-intensity renal parenchyma: causes and patterns.

Certain renal diseases manifest as low signal intensity of the renal parenchyma on magnetic resonance images. Sometimes, the appearance is sufficiently characteristic to allow a specific radiologic diagnosis to be made. The causes of this finding can be classified into three main categories on the basis of the pathophysiology: hemolysis, infection, and vascular disease. The first category includes paroxysmal nocturnal hemoglobinuria (PNH), hemosiderin deposition in the renal cortex from mechanical hemolysis, and sickle cell disease. The second category includes hemorrhagic fever with renal syn

Superficial siderosis of the central nervous system: MR findings with susceptibility-weighted imaging.

We report three cases of superficial siderosis of the central nervous system examined with MR susceptibility-weighted imaging (SWI) technique additional to the conventional SE sequence. Deposition of hemosiderin on SWI images is thicker and far more conspicuous compare with conventional T2-weighted images. Additional intracerebral bleeds are evidenced on susceptibility-weighted images in all three cases, which provide possible clues to the underlining etiology of superficial siderosis.

An autopsy case of Marchiafava-Bignami disease with peculiar chronological CT changes in the corpus callosum: neuroradiopathological correlations.

We conducted a comparative analysis of the autopsy findings in a case of Marchiafava-Bignami disease with the X-ray computed tomography (CT) and magnetic resonance imaging (MRI) findings obtained while the patient was alive. Histologically, massive deposits of hemosiderin were demonstrated in the astrocytes and macrophages located in the walls of the callosal cystic lesion, suggesting that the transient increase in density during the subacute stage observed on X-ray CT is secondary to significant hemorrhage.
