Autoimmune hepatitis is a chronic liver disease, and both genetic background and environmental factors are related to its pathogenesis. Here, we report that out of five members of a family with similar human leukocyte antigen haplotypes, two developed autoimmune hepatitis, one was positive for antinuclear antibody, and the remaining two had no features of autoimmunity. The two patients with autoimmune hepatitis had a history of medication use, whereas the other family members did not. Our familial study suggests that in addition to genetic background, medication use and other environmental fac

INTRODUCTION: The annual incidence of acute hepatitis C virus (HCV) has fallen in recent years, primarily because of effective blood screening efforts and increased education on the dangers of needle sharing. However, hepatitis C infection is still relatively frequent in certain populations. Most patients infected with HCV are unaware of their exposure and remain asymptomatic during the initial stages of the infection, making early diagnosis during the acute phase (first 6 months after infection) unlikely. While some of those infections will have a spontaneous resolution, the majority will pro

Neonatal hemochromatosis (NH) is a rare and enigmatic disease that has been clinically defined as severe neonatal liver disease in association with extrahepatic siderosis. It recurs at an alarming rate in the offspring of certain women; the rate and pattern of recurrence led us to hypothesize that maternal alloimmunity is the likely cause at least of recurrent cases. This hypothesis led to a trial of gestational treatment to prevent the recurrence of severe NH, which has been highly successful adding strength to the alloimmune hypothesis. Laboratory proof of an alloimmune mechanism has been ga

Although acute hepatitis C virus (HCV) infection is a rare disease and typically not associated with severe clinical symptoms, it has become a disease of significant interest for clinical investigators, virologists, and immunologists alike. In the same way that acute hepatitis C provided a window of opportunity for understanding the clinical and virological aspects of HCV infection as the field was being established, it is now clear that it can provide a window into further understanding the early interaction of the virus with the host immune response. The acute phase of infection is usually c

BACKGROUND: There is a paucity of academic literature that describes patients' experiences of receiving a hepatitis C-positive diagnosis, doctors' explanations of the infection and referral to support services.
AIM: The aim of the present study was to report findings on receiving a positive diagnosis from a study of people with hepatitis C infection.
METHOD: A cross-sectional study of people with hepatitis C infection living in New South Wales, Australia, was carried out (n=504).
RESULTS: Of participants diagnosed with hepatitis C infection from 1989 to 2001 (n= 17), just under one-third (2

46-year-old Mexican-born who lived in Mexico City was admitted to the hospital for evaluation with a two-week history of fever, jaundice, and malaise. Physical examination he had cardiac murmurs. The liver was palpable 2 cm below the costal margins. Liver-function tests showed hypertransaminasemia. The patient had a high titer of anti-EBV IgM antibodies, but tests for all other antiviral antibodies were negative. The liver biopsy shows EBV latent membrane protein.

During recent 10 years (1990-1999) essential changes occurred the epidemiology of viral hepatitis A (VHA) in Estonia: simultaneously with a decreased level of morbidity (morbidity rate per 100,000 of the population was 78.4 in 1990 and 7.7 in 1996, or 1,241 and 112 cases, respectively), a shift in the age structure of patients from children of preschool age to older age groups took place. Everyday contacts were the main established route of VHA transmission in recent years, but in more than 80% of cases the risk factors of the virus transmission remained obscure. During the regional outbreak o

Hepatitis E is an infectious viral disease with clinical and morphologic features of acute hepatitis. Although HEV infection is endemic in the Indian subcontinent, Southeast and Central Asia, a large outbreak of hepatitis E was identified in China. Smaller outbreaks have been observed in the Middle East, northern and western parts of Africa, and Mexico. Sporadic hepatitis E also has been observed in several countries. In nonendemic regions, the sporadic cases of hepatitis E are almost always associated with travel to HEV-endemic regions. In Korea, there has been no report on hepatitis E. Recen

We report a case of hepatitis in a 58-year-old woman being treated with thalidomide for end-stage plasma cell leukemia. The patient had a medical history including chronic stable hepatitis C infection. At diagnosis there was a severe anemia, thrombocytopenia, hypercalcemia, IgG paraproteinemia, peripheral blood myeloma cells, and a marrow plasmacytosis with lytic bony lesion. The disease was refractory to standard chemotherapy, and she was treated with oral thalidomide. Within 1 week she became jaundiced and developed a marked transaminitis. This promptly resolved upon cessation of thalidomide

BACKGROUND: Hepatitis A is a common, self-limited disease transmitted by fecal-oral contamination.
CASE: A 23-year-old woman, para 2-0-0-2, developed hepatitis A at 20 weeks' gestation. At 27 weeks, ultrasound revealed polyhydramnios and fetal ascites. Hepatitis A immunoglobulin M antibody in fetal blood obtained by funipuncture confirmed acute hepatitis A infection in the fetus. Polyhydramnios and ascites persisted until 35 weeks, when the fetus developed ultrasound signs of meconium peritonitis, and delivery was accomplished uneventfully. Meconium peritonitis was diagnosed in the neonatal p

Diagnosis of viral hepatitis is based on epidemiologic data (information about travels, blood transfusions, contacts with persons suffering from hepatitis, etc.) and on results of physical examination and laboratory investigations. Usually, serologic and genomic analysis lead to the specific proof of the viral etiology: HAV, HBV/HDV, HCV and HEV. If these infections are not detected, nonviral causes of hepatopathy should be investigated (i.e. toxic, pharmaco-toxic, metabolic, immunologic, etc.).

A 21-year-old man developed acute renal failure early in the course of hepatitis A infection and recovered after 17 days. There was no evidence of pre-renal azotemia, the hepato-renal syndrome, ischemic acute tubular necrosis, rhabdomyolysis, or thrombotic microangiopathy. There was, however, transient proteinuria and hypocomplementemia. It would appear that the renal failure resulted from viral-induced injury, either direct or mediated by immune complexes.

Fibrin ring granuloma is characterized by a fibrinous ring surrounding a central fat vacuole. It has been found in the liver and bone marrow of patients with Q fever, and occasionally with visceral leishmaniasis, cytomegalovirus, Epstein-Barr virus, Staphylococcus epidermidis infections, Hodgkin's lymphoma, and hypersensitivity to allopurinol. We describe a case of serologically confirmed viral hepatitis A with this lesion in the liver biopsy. A false positive anti-hepatitis A virus IgM result has been excluded. This is, to our knowledge, the second reported case of type A hepatitis with hepat

A unique hepatic adenoma developed in a 26-year-old woman who had used oral contraceptives for 10 years and Tolinase (tolazamide sulfonylurea) for adult-onset diabetes mellitus for five years. Clinically, radiographically, and grossly, the neoplasm showed the usual features of a hepatic adenoma, but microscopically it strongly resembled alcoholic hepatitis with steatonecrosis and Mallory bodies. The surrounding hepatic parenchyma was entirely normal. On transmission electron microscopy these Mallory bodies appeared to be tangles of intermediate filaments. They stained readily with antibodies t

Hepatitis A and B markers were determined in 714 patients and in 291 members of the staff of a psychiatric institution for adults. The leading diagnosis were psychosis (71.8%), neurosis (17.7%), and oligophrenia (10.6%). Anti-HAV was found in 84.4% of the patients and in 69.2% of the staff. 26.8% of the patients and 19.9% of the staff had at least one HBV marker. The frequency of HBV markers correlated with the age of the patients but not with the duration of the hospitalization. After the exclusion of IgM-anti-HAV, IgM-anti-CMV and IgM-anti-EBV or possible toxic effects, 2.6% of all patients

A 10-year-old girl initially presented with clinical features and thyroid function tests consistent with hyperthyroidism. She was treated with propylthiouracil, 100 mg, three times a day. She developed jaundice and hepatitis following treatment with propylthiouracil for 40 days. Clinical features of hepatitis improved after the discontinuation of propylthiouracil and she became euthyroid. At this time, an immunofluorescent technique revealed antibodies consistent with autoimmune thyroiditis. From this report, it appears that hepatitis is one of the infrequent complications of treatment with pr

A study has been made of 33 cases of granulomatous hepatitis. Of these, 12 were eventually established as cases of sarcoidosis, nine as cases of tuberculosis, and four as examples of primary biliary cirrhosis. No clinical diagnosis was ever reached in six cases. Analysis of the histological features of these cases shows that the presence of caseation or of tubercle bacilli will only allow for the diagnosis of approximately 50% of cases of tuberculosis. Histological features which are suggestive of tuberculosis are a paucity of granulomata, a scantiness of giant cells, a relatively mild surroun
