Congenital diaphragmatic hernia: advanced physiology and care concepts.

Congenital diaphragmatic hernia (CDH) is a defect in the formation of the diaphragm of the fetus. The diaphragm is the muscle and tissue that separate the chest and the abdominal cavities. In CDH, abdominal organs push into the chest cavity through the defect or herniation, compressing the developing lungs. During the past 10 years, significant changes have occurred in the diagnosis and management of CDH. Despite advances in postnatal care, infants born with a CDH continue to suffer substantial morbidity and mortality. Healthcare providers continue to research therapeutic approaches that will

Airway Management by Laryngeal Mask Airways for Cervical Tracheal Resection and Reconstruction: A Single-Center Retrospective Analysis.

BACKGROUND: Supraglottic airway devices (SADs) may have advantages over endotracheal intubation for tracheal resection and reconstruction in cases of severe and proximally located subglottic stenosis. This retrospective case series examines the feasibility of using SADs as a novel approach to airway management in tracheal resections. METHODS: All patients who were managed with SADs for cervical tracheal resection and reconstruction during the study period (2010-2015) in our university hospital were included. To examine the feasibility of airway management with SADs for tracheal resection, medi

Partial liquid ventilation: a comparison using conventional and high-frequency techniques in an animal model of acute respiratory failure.

OBJECTIVE: To test the hypothesis that high-frequency ventilation (HFV), when compared with conventional techniques, enhances respiratory gas exchange during partial liquid ventilation (PLV). DESIGN: A four-period crossover design. SETTING: Animal research laboratory of Children's Health Care-St. Paul. SUBJECTS: Thirty-two newborn piglets, weighing 1.40 +/- 0.39 kg. INTERVENTIONS: Animals were divided into four groups of eight animals: a) PLV with high-frequency jet ventilation; b) PLV with jet ventilation using a background intermittent mandatory ventilation (IMV) rate; c) PLV with high-f

Volume Targeted Ventilation and High Frequency Ventilation as the Primary Modes of Respiratory Support for ELBW Babies: What Does the Evidence Say?

Respiratory management of the extremely low birth weight (ELBW) newborn has evolved over time. Although non-invasive ventilation is being increasingly used for respiratory support in these ELBW infants, invasive ventilation still remains the primary mode in this population. Current ventilators are microprocessor driven and have revolutionized the respiratory support for these neonates synchronizing the baby's breath to ventilator breaths. High frequency ventilators with the delivery of tidal volumes less than the dead space have been introduced to minimize barotrauma and chronic lung disease.

Application of SHFJV for surgical procedures.

Using SHFJV in combination with the jet laryngoscope it is possible to ventilate patients for laryngeal surgery with out the necessity for any kind of endotracheal tube or catheters. This technique can also safely be applied in patients with underlying pulmonary or cardiac disease or in obese patients. Patients with laryngeal stenosis can be ventilated from above the stenosis eliminating the danger of barotrauma. The laser can be used at any time the surgeon desires without any additional protective measures. The SHFJV can be used for tracheobronchial stent insertion. Only in cases were the gl

Jet ventilation and hemodynamic stability during pulmonary vein isolation: ventilatory and hemodynamic outcomes from a randomized controlled trial (Secondary analysis under EEG-Guided Total Intravenous Anesthesia).

BACKGROUND: High-Frequency Jet Ventilation (HFJV) improves catheter stability and shortens procedure time during pulmonary vein isolation (PVI) for atrial fibrillation. This study reports predefined secondary outcomes assessing the ventilatory and hemodynamic safety of HFJV under total intravenous anesthesia (TIVA). METHODS: In this prospective, single-blind randomized controlled trial, 149 adult patients undergoing catheter-based PVI were assigned to HFJV or conventional volume-controlled ventilation. All patients received a standardized TIVA protocol guided by EEG-based depth monitoring. Se

Effect of supraglottic jet oxygenation and ventilation on hypoxemia in patients undergoing endoscopic surgery with sedation: A meta-analysis of randomized controlled trials.

BACKGROUND: Nasal cannulas and face masks are common oxygenation tools used in conventional oxygen therapy for patients undergoing endoscopic surgery with sedation. However, as a novel supraglottic ventilation technique, the application of supraglottic jet oxygenation and ventilation (SJOV) in endoscopic surgery has not been well established. METHOD: We searched six electronic databases from inception to January 16, 2024, to assess the oxygenation/ventilation efficacy and side effects of the of SJOV in endoscopic surgery. The primary outcome was the incidence of hypoxemia. The secondary outco

Rigid bronchoscopy and jet ventilation in an extremely low birthweight infant.

Until recently, rigid bronchoscopy was considered too risky for premature infants. We report a 658 g infant with life-threatening mediastinal shift due to right pulmonary atelectasis. Rigid bronchoscopy revealed tenacious mucous plugs in the right main bronchus. The collapsed lung was successfully reinflated after bronchial lavage with a rigid bronchoscope, with the help of intermittent high-frequency jet ventilation (HFJV). The infant's mediastinal shift was reversed and the vital signs became stable. In our experience, rigid bronchoscopy combined with HFJV is a valuable diagnostic and therap

[Ventilation with metallic tubes in CO2 laser surgery of the larynx].

One of the risks of laryngeal laser surgery is the ignition of the disposable plastic endotracheal tubes used for ventilation. In 1985, Hunton and Oswal (Anaesthesia, 40: 1210, 1985) developed a metallic tube which is now available in France. In this study, the tube has been tested on 30 patients undergoing this type of surgery. The tube was sterilized in glutaraldehyde before re-using it. There never was any problem with placing the tube and surgical conditions were satisfactory. There was neither incident due to the laser, nor any difficulties with the use of high-frequency jet-ventilation.

Congenital diaphragmatic hernia: impact of preoperative stabilization. A prospective pilot study in 13 patients.

In case of congenital diaphragmatic hernia (CDH), survival generally depends not on prenatal diagnosis, planned delivery, and immediate postnatal operation, but on the gravity of pulmonary hypoplasia and persistent hypertension (PPH). Many vasoactive drugs have become available for lowering PPH, but the mortality rate for CDH still amounts to 40% to 70%. Preoperative stabilization might prevent or at least reduce the risk of PPH. This method was evaluated in a pilot study lasting 15 months and involving 13 patients. All were admitted to the pediatric surgical intensive care unit within six hou

Emergency tracheal catheterization for jet ventilation: a role for the ENT surgeon?

Stridor causing respiratory failure is an ENT and anaesthetic emergency requiring prompt management to secure a clear airway. We describe a case of subacute partial upper airway obstruction due to a large laryngeal carcinoma in an 81-year-old male resulting in respiratory failure. The patient became apnoeic after gaseous induction of general anaesthesia, and after two failed intubation attempts an emergency transtracheal airway catheter was placed by the surgeon under direct vision below the cricothyroid membrane, as this had tumour involvement. The patient was subsequently manually jet-ventil

High frequency ventilation in the neonatal period.

There are three forms of high frequency ventilation, high frequency jet ventilation (HFJV, up to 400/min), high frequency oscillation (HFO, up to 40 Hz), and high frequency positive pressure ventilation (HFPPV, rates between 60 and 150/min). The first two forms of ventilation are still experimental and have been used only in critically ill children where respiratory failure has been unresponsive to more conventional therapy. Unfortunately, however, HFJV has already been associated with a high incidence of tracheal lesions. High-frequency positive pressure ventilation, on the other hand, using

Ventilator Management in Extremely Preterm Infants.

Advances in ventilation strategies for infants in the NICU have led to increased survival of extremely preterm infants. More than 75% of infants born at less than or equal to 27 weeks' gestation require initial mechanical ventilation for survival due to developmental immaturity of their lungs and respiratory drive. Various ventilators using different technologies and involving multiple management strategies are available for use in this population. Centers across the world have successfully used conventional, high-frequency oscillatory and high-frequency jet ventilation to manage respiratory f

Percutaneous transtracheal emergency ventilation during respiratory arrest: comparison of the oxygen flow modulator with a hand-triggered emergency jet injector in an animal model.

The oxygen flow modulator is a device for percutaneous transtracheal emergency ventilation. Simulating a respiratory arrest situation, we studied the effects of this device in comparison with a hand-triggered emergency jet injector during pulmonary resuscitation. Nine pigs were anesthetized and mechanically ventilated. After surgical exposure, an emergency transtracheal airway catheter was inserted into the trachea. Ventilation was stopped until SpO2 was below 70%. Each animal was subsequently randomly ventilated via the transtracheal airway catheter with either the hand-triggered emergency je

Alternatives to ECMO.

The past decade has witnessed technological advancements which are unparalleled in neonatology. ECMO has been demonstrated to be a powerful rescue treatment, but has perhaps been overutilised and is not universally available. Alternative treatments have been shown to be both safe and efficacious in the management of infants with respiratory failure. Direct head to head clinical trials will probably be necessary to establish appropriate criteria and indications for use, given the wide diversity of pathophysiology these unique patients present.

Effect of ventilator modes on neonatal cerebral and peripheral oxygenation using near-infrared spectroscopy.

AIM: The effect of ventilator modes on regional tissue oxygenation in premature neonates with respiratory distress syndrome (RDS) has yet to be delineated. Previous studies have looked at global oxygen delivery and have not assessed the effects on regional tissue oxygenation. Our aim in this study was to assess such tissue oxygenation in premature babies with RDS in relation to differing modes of ventilation using near-infrared spectroscopy (NIRS). METHODS: In 24 stable preterm infants with RDS, undergoing elective wean in ventilator mode, cerebral and muscle tissue oxygenation were assessed
