Holmium: YAG laser endoureterotomy in the treatment of ureteroenteric strictures following orthotopic urinary diversion.

The management of ureteroenteric strictures in patients who have undergone urinary diversion can be challenging. In those patients with an orthotopic neobladder, anastomotic ureteral strictures can be treated endoscopically using a retrograde or antegrade approach. The availability of small (7.5F) flexible ureteroscopes, as well as the use of the Holmium laser has facilitated the ability to precisely incise the stricture under direct endoscopic visualization (endoureterotomy). We describe our technique for laser endoureterotomy in patients with ureteroenteric strictures following orthotopic ur

Holmium laser enucleation of the prostate in the oldest-old: a comparative analysis across the extremes of age.

INTRODUCTION: Expansion of the "oldest-old" (≥85 years) has increased the prevalence of BPH and the need for surgery. Holmium laser enucleation of the prostate (HoLEP) is a size-independent alternative to TURP, yet age-specific outcomes remain understudied. We compared outcomes, complications, and recovery after HoLEP in men ≥85 years versus ≤55 years. METHODS: We conducted a retrospective analysis of a prospective database of HoLEP procedures performed by a single surgeon at a tertiary academic center (2017-2025). Men aged ≥85 years constituted the study cohort and were compared with men ≤55

En-bloc holmium laser enucleation of the prostate (HoLEP): Initial experience and consolidation of the technique.

INTRODUCTION AND OBJECTIVES: Benign prostatic enlargement has a prevalence of approximately 60% in 60-year-old men. En bloc enucleation and its variants, which emerged in 2010, are methods to enucleate the three lobes of the prostatic capsule completely, maintaining the capsular plane and the urethral sphincter while removing the prostate. The aim of the present study is to analyze the implementation of the surgical technique, considering the learning curve and the experience accumulated in a group of patients before and after its development. MATERIAL AND METHODS: Retrospective study from Ju

The Impact of Anesthesia-induced Intraoperative Hypotension on Hemostasis During Holmium Laser Enucleation of the Prostate.

OBJECTIVE: To investigate the effect of intraoperative hypotension (IOH) on clinically significant hematuria (CSH) following holmium laser enucleation of the prostate (HoLEP). Benign prostatic hyperplasia (BPH) is a common cause of lower urinary tract symptoms (LUTS) in men. HoLEP has emerged as a superior surgical option due to its hemostatic properties, especially in patients at risk of bleeding. IOH, commonly defined as a >20% reduction in systolic blood pressure (BP) from baseline, is frequent under general anesthesia and may obscure intraoperative bleeding-but its role in CSH is unclear.

The Role of Holmium Laser Enucleation of the Prostate in Reducing the Risk of Bladder Cancer Recurrence and Progression.

BACKGROUND: Bladder outlet obstruction (BOO) and urinary retention were recognized contributing factors to bladder cancer (BCa) recurrence and progression due to prolonged urothelial exposure to carcinogens, chronic inflammation, and incomplete bladder emptying. Holmium laser enucleation of prostate (HoLEP) is a minimally invasive and highly effective procedure that completely removes obstructive prostatic tissue, thus resolving BOO. To explore the HoLEP impact on BCa recurrence and progression by improving voiding dynamics and reducing post-void residual (PVR). MATERIALS AND METHODS: We carr

The en bloc method is feasible for beginners learning to perform holmium laser enucleation of the prostate.

BACKGROUND: Holmium laser enucleation of the prostate (HoLEP) is a new alternative method for the treatment of benign prostatic hyperplasia (BPH); however, most surgeons are hesitant to use this new technique due to its steep learning curve. Notably, most beginners start with the traditional multi-incisional method rather than the en bloc method. Thus, we sought to explore the feasibility of beginners adopting the en bloc method in HoLEP. METHODS: A clinical development project was established at Department of Urology, Affiliated Nantong Rehabilitation Hospital of Nantong University in Decemb

Leiomyoma of the Prostate Treated via Holmium Laser Enucleation: A Case Report and Literature Review.

Leiomyoma is a rare tumor that arises from mesenchymal cells, with few reported cases of treatment using holmium laser enucleation of the prostate. A 74-year-old man with dysuria had a mass near the bladder neck in magnetic resonance imaging; this entity was suspected to be a leiomyoma. The patient underwent holmium laser enucleation of the prostate and one lobe was removed. However, the mass was firm and morcellation was difficult to break into small pieces. Therefore, it was fragmented via trans-urethral resection and removed with a curette. The postoperative course was favorable, with a pos

Case report of rescue of a patient with COVID-19 and shock after holmium laser lithotripsy.

Lithotripsy is the primary form of treatment for ureteral calculus. According to clinical data, ureteroscopic lithotripsy (URSL) is characterized by better efficacy, a lower risk of complications, and a quicker postoperative recovery, when compared with open surgery. However, elderly patients often have a variety of chronic diseases that can directly or indirectly influence intraoperative care and postoperative recovery. It is important that medical staff closely observe changes in the postoperative condition of patients and provide them with the best quality care. In order to control the prog

Analysing the learning curve of prostate enucleation with the Holmium laser: A retrospective, single-center experience.

BACKGROUND: Holmium laser enucleation of the prostate (HoLEP) is a proven surgical technique for the treatment of benign prostatic hyperplasia (BPH). However, its challenging learning curve prevents its widespread adoption by urologists. The aim of this study was to analyse the learning curve of HoLEP and to determine the factors accelerating it. METHODS: This was a retrospective, monocentric cohort study of the first 60 cases of HoLEP performed by three operators. The primary outcome measure was operative efficiency, defined as the ratio of preoperative prostate volume estimated by trans-abd
