Long-term health, neurodevelopmental, and educational outcomes of children born to mothers with hyperemesis gravidarum: a population-based sibling-design record linkage cohort study.

BACKGROUND: Hyperemesis gravidarum (HG), characterized by severe and constant nausea and vomiting in pregnancy, can lead to nutritional deficiencies and other pregnancy complications. In turn, HG has also been linked with adverse long-term health and neurodevelopmental outcomes for the children of women affected by HG. However, previous studies have not accounted for potential confounding due to shared family-level factors. OBJECTIVE: This study aimed to determine whether maternal HG was associated with long-term educational, neurodevelopmental, and health outcomes for children, and whether t

Proposal of a service delivery model for supported living community forensic services.

PURPOSE: Following serious case review, the Transforming Care agenda (DH, 2015) highlights the need for adults with learning disabilities, autism, mental health issues or behaviors that challenge to be supported within communities rather than hospitals. Poor or absent leadership has been identified as contributing to serious cases of abuse in health-care settings [Department of Health (DH), 2012]. This paper aims to focus on identifying the elements required for good leadership and service delivery in community forensic services (CFS). DESIGN/METHODOLOGY/APPROACH: The perspectives of 12 suppo

Task sharing: Development of evidence-based co-management strategy model for screening, detection, and management of diabetic retinopathy.

BACKGROUND: The number of adults with diabetes is increasing worldwide and also the number of people with diabetic retinopathy (DR), a major complication of diabetes. Task sharing in eye care for people with diabetes could address the shortage in the number of ophthalmologists and increase access to eye care services. This study investigated the opinion of eye care professionals for a checklist of tasks, which are involved in DR management, to be possibly shared by optometrists and mid-level eye and health care workers with ophthalmologists. METHODS: The study used a purposive sampling techni

Inequalities in access to and quality of abortion services in Mexico: Can task-sharing be an opportunity to increase legal and safe abortion care?

First-trimester abortion became legal in Mexico City in April 2007. Since then, 216 755 abortions have been provided, initially in hospitals, by specialized physicians using surgical techniques. With time and experience, services were provided increasingly in health centers, by general physicians using medical therapies. Meanwhile, abortion remains legally restricted in the remaining 31/32 Mexican states. Demand and need for abortion care have increased throughout the country, while overall abortion-specific mortality rates have declined. In an effort to ensure universal access to and improved

Nutritional management of the patient with pancreatic cancer: from the diagnostic and therapeutic pathway to an integrated hospital-territory approach.

Pancreatic cancer is associated to a high risk of malnutrition and neoplastic cachexia even at first diagnosis. Malnutrition is a negative prognostic factor for the outcome of surgery or medical oncology treatments. Despite the good awareness of the problem and the knowledge of the guidelines, the early recognition of malnutrition and its management are still uneven, mainly due to the lack of implementation of standardized and shared protocols and the shortage of dedicated clinical nutritionists and dieticians. An early and appropriate nutritional intervention is mandatory to improve the outco

Paediatric Upper Respiratory Infection in Colombia: 10-Year Nationwide Trends in Healthcare Utilization From Administrative Data (2015-2024).

BACKGROUND: Upper respiratory tract infections (URTIs) are amongst the commonest reasons for paediatric care, yet national evidence from Latin American middle-income health systems remains limited. We used the COVID-19 pandemic as a system shock to examine whether paediatric URTI care returned to its pre-pandemic trajectory or settled into a new service configuration. METHODS: We performed a nationwide population-based time-series study using Colombia's administrative healthcare registry (RIPS) linked to official population projections (DANE) for children and adolescents aged 0-17 years from

Palliative care impact on hospital utilization and trends among lung cancer admissions with a high risk of mortality in U.S. hospitals.

BACKGROUND: The top 5% of healthcare utilization accounts for 60% of healthcare expenditure in the U.S., with the majority of the costs occurring in the last year of life for hospital inpatient services for diseases such as lung cancer (LC). Previous years' concentrated healthcare expenditure burdens have been shared globally. The research questions of this study are whether palliative care (PC) is associated with hospital utilization and what the PC trends are in U.S. hospitals. Thus, this study aimed to examine (1) the impact of hospital PC on hospital utilization charges and length of stay

Navigator programme for hospitalised adults experiencing homelessness: protocol for a pragmatic randomised controlled trial.

INTRODUCTION: People experiencing homelessness suffer from poor outcomes after hospitalisation due to systemic barriers to care, suboptimal transitions of care, and intersecting health and social burdens. Case management programmes have been shown to improve housing stability, but their effects on broad posthospital outcomes in this population have not been rigorously evaluated. The Navigator Programme is a Critical Time Intervention case management programme that was developed to help homeless patients with their postdischarge needs and to link them with community-based health and social serv

Emergency department burden of e-scooter injuries in Victoria, Australia: A statewide analysis of injury patterns and prevention implications, 2022-2025.

BACKGROUND: E-scooter use has expanded rapidly in Australia alongside evolving legislation and changes in e-scooter availability. However, the population-level impact on injury burden and emergency care services remains incompletely characterised, particularly in the absence of standardised external cause coding for e-scooter injuries. METHODS: We conducted a retrospective statewide analysis of emergency department (ED) presentations for e-scooter-related injuries in Victoria, Australia, using the Victorian Emergency Minimum Dataset (VEMD). Cases from January 2022 to December 2025 were identi

Designing for Patient-Centered Care and Equity in Virtual Hospital-at-Home Models: Quality Improvement Initiative Using Experience-Based Co-Design.

BACKGROUND: The rapid expansion of virtual care during COVID-19 accelerated the development of virtual hospital-at-home models, which deliver hospital-level care in patients' homes through remote monitoring, virtual communication, and in-person support when required. While the virtual hospital-at-home model offers the potential to improve patient-centered care and health equity, rapid implementation often overlooks culturally diverse and underserved populations, including South Asian communities who experience disproportionate chronic disease burden and barriers to accessing culturally relevan

Community Benefit: Policies, Practices, and Opportunities at the Half-Century Mark.

In the 50 years since the expansion of the legal definition of charity for tax-exempt hospitals, there have been periodic regulatory actions at the municipal, state, and federal level to quantify charitable contributions and justify the deferral of tax revenues. The movement toward risk-based reimbursement in the last decade creates an incentive for a shift in hospital leadership understanding and approach to community benefit programs and services. The historical interpretation of community benefit as an issue of compliance with legal obligations is being questioned by forward-thinking hospit

Real World Experience of Denosumab Treatment in the Belfast Osteoporosis Service.

Osteoporosis is a significant global health and economic burden associated with bone fracture, morbidity and mortality. Denosumab, a novel human monoclonal antibody second-line treatment, inhibits osteoclast-mediated bone resorption and increases bone mineral density (BMD). Treatment achieves reductions in vertebral, non-vertebral and hip fracture risk. We undertook a service evaluation to review clinical outcomes of patients treated with denosumab in an osteoporosis department that provides regional services. We identified 529 patients (95% female; mean age 72.8 years; 35-98 years), who had a
