Microscopic clearance with ivermectin-permethrin combination vs. permethrin alone in noncrusted scabies: a retrospective cohort study.

BACKGROUND: Scabies is a common problem in dermatology clinics, with an increasing burden in long-term care hospitals (LTCHs). Recent guidelines recognize both topical 5% permethrin and oral ivermectin as first-line treatment options for noncrusted scabies, whereas combination therapy is primarily recommended for crusted scabies. Management in routine practice remains challenging, particularly in patients with a heavier infestation, and evidence supporting combination strategies in noncrusted scabies remains limited. OBJECTIVES: To evaluate real-world microscopic treatment outcomes of oral iv

Barriers and Facilitators to Clinician Readiness to Provide Emergency Department-Initiated Buprenorphine.

IMPORTANCE: Treatment of opioid use disorder (OUD) with buprenorphine decreases opioid use and prevents morbidity and mortality. Emergency departments (EDs) are an important setting for buprenorphine initiation for patients with untreated OUD; however, readiness varies among ED clinicians. OBJECTIVE: To characterize barriers and facilitators of readiness to initiate buprenorphine for the treatment of OUD in the ED and identify opportunities to promote readiness across multiple clinician types. DESIGN, SETTING, AND PARTICIPANTS: Using data collected from April 1, 2018, to January 11, 2019, th

The Critical Care Society of Southern Africa Consensus Guideline on ICU Triage and Rationing (ConICTri).

Background. In South Africa (SA), administrators and intensive care practitioners are faced with the challenge of resource scarcity as well as an increasing demand for intensive care unit (ICU) services. ICU services are expensive, and practitioners in low- to middle-income countries experience the consequences of limited resources daily. Critically limited resources necessitate that rationing and triage (prioritisation) decisions are routinely necessary in SA, particularly in the publicly funded health sector. Purpose. The purpose of this guideline is to utilise the relevant recommendations o

Management of barium enema-induced colorectal perforation.

Unless recognized and treated promptly, colorectal perforation induced by barium enema examination is a life-threatening complication. Between 1977 and 1986, 13,000 barium enemas were performed at the Mayo Medical Center. Colorectal perforation occurred in five patients (overall incidence: 0.04%). The two colonic perforations were managed by immediate celiotomy with resection in one and primary repair in the other. The three rectal perforations were managed conservatively in two patients and by proximal diversion in one. All patients recovered. Perforations were believed to be related to the t

How do General Practice trainees experience the transition from hospital to General Practice: a qualitative study.

BACKGROUND: Specialist training in General Practice (GP) incorporates hospital and community-based training. The transition from hospital to General Practice can be a challenging experience. A negative experience of this transition has been shown to impact the trainee's desire to remain in General Practice. AIM: The aim of this study was to explore GP Trainees' experiences of moving from hospital medicine to General Practice in Ireland. METHODS: This qualitative study involved an initial focus group with GP trainees. The results of this focus group informed the development of a topic guide f

Determinants of sustainable quality improvement for operating room clean air systems: a two-phase sequential qualitative study in Dutch hospitals.

BACKGROUND: To address healthcare’s polluting environmental impacts, environmental sustainability is increasingly recognised as a key domain of healthcare quality improvement. Heating, ventilation, and air conditioning (HVAC) systems are important targets for environmental sustainability-oriented quality improvement, causing 70–90% of operating room (OR) energy consumption and incurring substantial amounts of greenhouse gas emissions. Safe and effective energy-saving measures exist yet are infrequently implemented in practice. This study aims to identify key implementation determinants for HVA

Perspectives and practices of counselling, consent and debriefing for cesarean sections: a mixed-methods study in two Indian states.

BACKGROUND: Effective counselling, consent, and debriefing (CCD) are essential for respectful, quality cesarean section (CS) services but limited evidence exists. We explored providers and clients' perceptions and practices regarding CCD for CS in India. METHODS: A mixed-methods study was conducted in seven purposively selected secondary level hospitals (five public, two private) across two states. Key-informant interviews and in-depth interviews (N = 108) with providers, women and their relatives, and community workers were conducted in all seven hospitals. Focused ethnography and record rev

Integration of Cirrhosis Best Practices Into Electronic Medical Record Documentation Associated With Reduction in 30-Day Mortality Following Hospitalization.

BACKGROUND: Hospital admissions for patients with cirrhosis continue to increase. In New York City, 25% to 30% of hospitalized cirrhotics are readmitted within 30 days. Rehospitalization is associated with increased mortality, poor quality of life, and financial burden to patients, hospitals, and payers. Preventable readmissions are partially accounted for by a well-documented quality gap between evidence-based guidelines for cirrhosis management and real-world adherence to these recommendations. METHODS: We performed a prospective cohort study that compared outcomes among cirrhotic patients

Consensus document on the radial approach in percutaneous cardiovascular interventions: position paper by the European Association of Percutaneous Cardiovascular Interventions and Working Groups on Acute Cardiac Care** and Thrombosis of the European Society of Cardiology.

Radial access use has been growing steadily but, despite encouraging results, still varies greatly among operators, hospitals, countries and continents. Twenty years from its introduction, it was felt that the time had come to develop a common evidence-based view on the technical, clinical and organisational implications of using the radial approach for coronary angiography and interventions. The European Association of Percutaneous Cardiovascular Interventions (EAPCI) has, therefore, appointed a core group of European and non-European experts, including pioneers of radial angioplasty and oper
