In contrast to distal humeral fractures, humeral head fractures represent a common injury to the old patient. For both fracture localizations osteoporosis and multi-morbidity are of significant importance. The classification for humeral head fractures in one-, two-, three- and four-part fractures is generally accepted. Valgus impacted head fractures as well as head-splitting fractures are considered a separate entity. In none or minimally displaced fractures good functional results are achieved by conservative means. Although numerous therapeutical procedure are offered in the treatment of dis

Adult distal humerus fractures are infrequent, yet they account for one-third of all humerus fractures. For the treatment of comminuted and osteoporotic fractures, locking plates are claimed to be biomechanically superior to alternative internal fixing techniques. Treatment remains difficult despite recent advancements and the use of locking plates due to frequent comminution, low bone quality, and limited healing ability in osteoporotic bone. An optimal design of the newly constructed plate and the control model were selected. The biomechanical characteristics of non-osteoporotic and osteopor

INTRODUCTION AND IMPORTANCE: A re-nonunion after failed intramedullary nailing for a nonunion of the humeral shaft and a peri-implant distal humeral fracture with an ununited humeral shaft are rare cases. Therefore, no consensus has been established regarding the treatment strategies.
CASE PRESENTATION: Case 1: An 84-year-old woman presented with humeral shaft re-nonunion after intramedullary nailing. The images revealed callus formation and loosening of the implant, suggesting a lack of mechanical stability. Nail removal and helical plating were performed. One year postoperatively, bony unio

Nonunion is one of the most common and challenging complications of distal humeral fractures. Newer internal fixation principles and techniques have improved the ability of orthopaedic surgeons to achieve stable fixation of complex distal humeral fractures. However, some fractures will fail to unite, leaving the patient with an unstable, dysfunctional, and often painful upper extremity that requires additional surgery.

BACKGROUND: Humeral fractures are common, but the association between the patho-anatomical fracture pattern and patient characteristics has been inadequately studied and epidemiological knowledge is scarce. Following the introduction of the Swedish Fracture Register (SFR), risk factors for various fractures can be studied, as well as the outcome of different treatments. The objective of this study was to analyse adult humeral fractures in Gothenburg from a descriptive epidemiological perspective.
METHODS: All humeral fractures registered in the SFR at Sahlgrenska University Hospital in 2011-2

PURPOSE: Lateral humeral condyle fracture (HLCF) is a common pediatric elbow injury. For minimally displaced fractures (≤ 2 mm), the optimal management strategy remains controversial. This systematic review and meta-analysis aimed to evaluate the feasibility and safety of conservative treatment for this specific subtype. METHODS: A systematic search of multiple databases was conducted to compare conservative and surgical management of minimally displaced (≤ 2 mm) pediatric HLCF. The primary outcome was fracture healing, defined by the absence of secondary displacement during follow-up. Seconda

Gartland type IV supracondylar humerus fractures in children are characterized by complete periosteal disruption and multidirectional instability, frequently resulting in failure of standard closed reduction maneuvers. Persistent rotational instability is the primary factor leading to loss of reduction and the subsequent need for open reduction. Although open reduction is effective, it is associated with increased soft-tissue dissection, longer operative times, and a higher risk of postoperative stiffness and neurovascular complications. We describe a temporary lateral cortical pin-assisted cl

Distal femoral metaphyseal fractures are common injuries in children. Multiple treatment options have been described for this type of injury. For older children with distal metaphyseal fracture, there is still no optimal method of fixation. We propose that the commonly used proximal humeral plate can provide good method of fixation for this fracture in adolescents. Two children (12 and 14 years old) with distal metaphyseal femoral fracture were treated with proximal humeral plate. We describe the surgical technique and postoperative management. The two children healed with good alignment and f

OBJECTIVE: Supracondylar humerus fractures (SHFs) are prevalent injuries in pediatric orthopedics and often necessitate surgical management to restore elbow function and prevent long-term complications. The study aimed to evaluate the biomechanical effects of different fracture levels relative to the olecranon fossa and the efficacy of different pin configurations.
METHODS: Synthetic humerus bone models were used to standardize the fracture patterns and simulate surgical interventions. Pin configurations, including lateral divergent and cross-pin configurations, were applied using 3D-printed

PURPOSE OF THE STUDY: Radiological studies in supracondylar humerus fractures (SCHF) often focus on coronal plane imaging. In our study, we focused on the measurements used in elbow sagittal plane imaging. We investigated the role of humeral shaft condylar angle (SCA) and lateral capitellohumeral angle (LCHA) in determining the reduction quality by comparing them with Baumann angle (BA). İnvestigated its effectiveness in assessing the reduction of the teardrop figure and in the follow-up period.
MATERIAL AND METHODS: Eighty-eight patients with SCHF were included in the study. BA, SCA and LCHA

OBJECTIVE: The trochlea is of great importance for the stability of the elbow and its fixation in low transcondylar fractures of the distal humerus is especially challenging. The aim of this study was to determine the optimal trochlea fixation in double plate osteosynthesis of intraarticular distal humerus fractures.
METHODS: A low transcondylar, C3-type distal humerus fracture was created in 20 fresh-frozen human cadaveric humeri. The samples were then randomly divided into two groups of 10 specimens each. Double plate osteosynthesis was performed in both groups. In group A, the two most dis

The treatment of comminuted distal humeral fractures with free osteochondral fragments is challenging. Osteochondral fragments should be retained whenever possible and secured with implants buried beneath the articular surface to obtain a uniform articular surface. Headless compression screws and K wires are commonly used for this purpose. However, certain complications have been reported with these fixation implants in case of the non-union and osteolysis of the fragments such as migration and cartilage damage. Fixation of osteochondral fractures in distal humeral fractures using bioabsorbabl

CASE: An 82-year-old man sustained a periprosthetic fracture after IlluminOss photodynamic bone stabilization system (PBSS) stabilization for an impending pathologic fracture. Nonoperative management was unsuccessful, and he subsequently underwent operative fixation, which featured lag screw fixation of the spiral distal humeral shaft fracture and osteotomy followed by plate fixation of the pathologic humeral shaft fracture.
CONCLUSION: This is the first article to report this complication with IlluminOss PBSS stabilization and details regarding revision surgery.

From 1992 to 1994, 29 middle and 19 distal humeral shaft fractures (39 acute fractures, six nonunions, and three pathologic fractures) in 48 patients were treated by retrograde locked nailing. The first eight acute fractures were treated with Seidel nails, the other 40 fractures with specially designed humeral locked nails. Nails were inserted from the supracondylar (6) or the olecranon fossa (42) entry portal. With a single operation, all acute fractures and nonunions achieved osseous union without serious complications. The average time to union was 8.2 weeks for acute fractures and 14.2 wee

We report three cases of fracture of the humerus that occurred at the tip of interlocking humeral nails inserted previously for humeral shaft fractures. The fractures occurred at 8, 10, and 26 weeks postoperatively, and all occurred through the distal interlocking screws after the patient sustained a rotational force to the arm. All three patients had further surgery, two subsequently healed their fractures, and one is still in active treatment. Orthopaedists should be aware of this complication when choosing interlocking humeral nails for the treatment of humeral shaft fractures.

INTRODUCTION: Lateral humeral condyle fractures (LHCFs) are the most common intra-articular fracture occurring at the elbow in children. Conventional treatment for displaced pediatric LHCFs is open reduction and percutaneous pinning, and few studies have regarded the efficacy of arthroscopic-assisted techniques. We aimed to evaluate the efficacy of anatomic reduction via elbow arthroscopy for pediatric humeral lateral condyle fractures with displacements >4 mm.
METHODS: A total of 32 children with LHCFs featuring displacements >4 mm were enrolled in this retrospective study. Arthroscopically
