Idiopathic humeral head osteonecrosis mimicking rotator cuff disorders: Two challenging diagnostic case reports.

RATIONALE: Shoulder pain is a common complaint among patients, and rotator cuff disorders are the most common diagnoses. Humeral head osteonecrosis is easily masked by other more common diagnoses and concomitant conditions. PATIENT CONCERNS: This challenging diagnostic report consists of 2 cases. Case 1 was that of a 59-year-old man who presented with right shoulder pain that had lasted for >1 year. Case 2 was that of a 52-year-old man who complained of right shoulder pain lasting for 6 months. They both presented with chronic right shoulder pain without relevant trauma history, and the physi

Structural glenoid grafting during primary reverse total shoulder arthroplasty using humeral head autograft.

BACKGROUND: Large glenoid bone defects in the setting of glenohumeral arthritis can present a challenge to the shoulder arthroplasty surgeon. The results of large structural autografting at the time of reverse total shoulder arthroplasty (RTSA) are relatively unknown. METHODS: This retrospective case series describes the clinical and radiographic results of large structural autografting from the humeral head to the glenoid during primary RTSA. RESULTS: Of 17 patients who met inclusion criteria, 14 (82% follow-up) were evaluated postoperatively at a mean of 2.6 years (range, 2.0-5.4 years). M

Atraumatic osteonecrosis of the humeral head after influenza A-(H1N1) v-2009 vaccination.

In the recent pandemic influenza A-(H1N1) v-2009 vaccination campaign, adjuvanted vaccines have been used because of their antigen-sparing effect. According to available reports, the rate of severe vaccination reactions has not increased, as compared with previous seasonal influenza vaccinations. Here we describe an adult female patient who was vaccinated with an AS03 adjuvanted split-virus vaccine injected into the left arm. She experienced a prolonged and painful local reaction for 4 weeks. During this time, persistent incapacitating pain shifted into the left shoulder. Magnetic resonance im

Missed intrathoracically displaced humeral head in proximal humerus fracture dislocation: A case report.

CASE: We report a case of a 44-year-old female with intrathoracic displacement of the humeral head, which was not only missed by the initial trauma team while receiving the patient but also missed by multiple senior surgeons during primary management in the operating room. CONCLUSION: Diagnostic accuracy hinges on clinical presentation and detailed imaging locating fragments of the fractured head. Interdisciplinary collaboration, particularly with thoracic and trauma surgeons, is crucial for early diagnosis and timely recovery. The possibility of intrathoracic dislodged head should not be rul

A Rare Case of Intramediastinal Humeral Head Fracture Dislocation Through the Thoracic Inlet.

Intramediastinal fracture dislocation of the humeral head due to a blunt trauma is very rare. It may be accompanied by local and systemic injuries associated with high-energy trauma. Only one case has been reported in the literature; therefore, an appropriate treatment modality remains unclear. We present a case of a 71-year-old woman involved in an accidental fall who sustained an intramediastinal humeral head fracture dislocation through the thoracic inlet, without damaging the chest wall, causing the thoracic duct rupture and requiring three consecutive surgical procedures as treatment.

Massive rotator cuff tears repaired on top of humeral head by McLaughlin's procedure.

The clinical and radiographic outcomes of McLaughlin's procedure for massive rotator cuff tears were investigated in 25 shoulders, in which the cuff tears were so severe that the tendons were sutured on the top of the humeral head. The mean age at surgery was 62.2 years (range, 39-74 years). The mean follow-up period was 50 months (range, 24-80 months). The University of California, Los Angeles score significantly improved from 10.9 to 31.8 points postoperatively; the postoperative result was classified as excellent in 11 shoulders, good in 11, and poor in 3. Osteoarthritis progressed postoper

Spontaneous bilateral humeral head fractures occurring simultaneously in a woman with rheumatoid arthritis.

A 67-year-old woman with a 15-year history of seropositive rheumatoid arthritis developed acute night pain in both shoulders. Examination revealed bilateral swelling and joint mobility impairment. Ultrasonography disclosed joint effusions which were hemorrhagic on aspiration. Radiographs showed the presence of a four-part fracture of both humeral heads. Bilateral humeral head fractures can occur spontaneously in patients with rheumatoid arthritis and osteoporosis. Repeated use of crutches could be a predisposing factor.

Internal fixation of valgus-impacted humeral head fractures using the screw-wiring technique.

Between February 1996 and July 2000 eight patients (ages 35-74 years) with nine severely impacted humeral head fractures were operated on with the aim of preserving the humeral heads. Patients were treated surgically with open reduction, autologous bone grafting, and internal fixation with a screw-wiring technique. There were no complications and all patients were satisfied with the results. After 10-63 months there was no evidence of nonunion, arthritis, avascular necrosis, or loosening of implants. The mean Constant score was 83 (66-97) points. Results of this study support the observation t

Bilateral aseptic necrosis of the humeral head following combined therapy for Hodgkin's lymphoma.

A young woman treated with combined therapy (chemo- and radiotherapy) for Hodgkin's disease later developed avascular necrosis of the humeral heads. The unusual location of the aseptic necrosis, and particularly the fact that it occurred bilaterally, made the case exceptional. The hypothesis is that the radiotherapy was a contributory factor.

Relationship Between the Axis of the Humeral Heads and the Spinal Sagittal Alignment in Asymptomatic Volunteers.

Background Currently, there is no known correlation between relevant parameters of respective non-adjacent spinal segments, which suggests the possibility of a third external factor. To our knowledge, no study has examined the humeral head position in consideration of spinal sagittal alignment. This study aims to establish new parameters related to the humeral head axis and evaluate the relationship between the axis of the humeral head and spinal sagittal alignment in asymptomatic volunteers. Methods Standing posteroanterior and lateral radiographs of the entire spine were obtained from 62 asym

Incidence and risk factors for pseudosubluxation of the humeral head following proximal humerus fracture.

BACKGROUND: Humeral head pseudosubluxation (HHPS) in relation to the glenohumeral joint is a common finding following fractures of the proximal humerus. The temporary inferior subluxation of the humeral head may be secondary to a transient axillary nerve neuropraxia, pain inhibition of the deltoid, or hemarthrosis or capsular disruption that alters the physiologically negative pressure in the glenohumeral joint. Despite the frequency of this finding, it is not well described in the literature. This study sought to describe the incidence, risk factors, and rate of resolution of HHPS following p

Treatment of avascular necrosis of the humeral head - Postoperative results and a proposed modification of the classification.

BACKGROUND: Avascular necrosis of the humeral head after proximal humeral fracture i.e. type 1 fracture sequelae (FS) according to the Boileau classification is a rare, often painful condition and treatment still remains a challenge. This study evaluates the treatment of FS type 1 with anatomic and reverse shoulder arthroplasty and a new subclassification is proposed. METHODS: This single-center, retrospective, comparative study, included all consecutive patients with a proximal humeral FS type 1 treated surgically in a four-year period. All patients were classified according to the proposed

Autologous chondrocyte implantation for treatment of focal articular cartilage defects of the humeral head.

BACKGROUND: Autologous chondrocyte implantation (ACI) constitutes an established treatment option for cartilage defects of the knee joint. Experience in the shoulder, however, is limited, and the management of cartilage defects remains a challenge. The purpose of this study was to evaluate the results after ACI with 3-dimensional spheroids of human autologous matrix-associated chondrocytes in the shoulder. METHODS: Seven male patients (median age, 42.8 years [range, 18-55 years]) underwent ACI for symptomatic focal grade IV cartilage lesions of the humeral head by an open or arthroscopic appr

Classification of humeral head pathomorphology in primary osteoarthritis: a radiographic and in vivo photographic analysis.

BACKGROUND: The purpose of this study was to characterize the pathologic changes of the osteoarthritic humeral head. METHODS: The study included 55 patients with primary osteoarthritis who underwent anatomic shoulder arthroplasty. Several radiologic parameters (radiography, magnetic resonance imaging) were assessed. Humeral head deformity in the transverse plane and humeral cartilage erosion in the coronal plane were chosen for photographic measurements from the resected humeral heads. RESULTS: In the coronal plane, 82% of patients presented with an aspherical humeral head shape with a signi

Stability of the Glenohumeral Joint With Combined Humeral Head and Glenoid Defects: A Cadaveric Study.

BACKGROUND: Shoulders with recurrent anterior instability often have combined bony defects of the humeral head and glenoid. Previous studies have looked at only isolated humeral head or glenoid defects. PURPOSE/HYPOTHESIS: The aim of this study was to define the relationship of combined humeral head and glenoid defects on anterior shoulder instability. Combined bony defects will lead to increased instability compared with an isolated defect, and the "critical" size of humeral head and glenoid defects that need to be addressed to restore stability will be smaller when combined rather than isol

[Destruction of the Humeral Head in Sensory Deficit due to the Spinal Hemorhagia. Case Report].

Degenerative changes of the shoulder are a common complication in patients after spinal cord injury. The main cause is chronic overload to the shoulder joint due to manual wheelchair propulsion and transfers. Reduced shoulder function has a significant impact on all aspects of daily life. Shoulder arthroplasty in this group of patients is a challenging procedure because of the unique demand on the shoulder. This report presents the case of a wheelchair user who additionally experienced a complete loss of sensation around her shoulder. As a result of a repetitive strain during transfers from th
