[Significance of proteinuria and hypertension in the prognosis of type 1 diabetes. Results of a 10-year follow-up study on micro- and macrovascular disease mortality].

Among 163 insulin-dependent (type I) diabetics (average age 43.5 years; average duration of diabetes 17.5 years), 40 (24.5%) died within ten years from the consequences of micro- and (or) macro-angiopathies. The death-rate among hypertensives was twice that among normotensives: 21 of 53 patients (39.6%) with blood pressures above 160/95 mmHg, but 19 of 110 patients (17.3%) with normal pressures. Proliferative retinopathy at the onset of the study was also a predictive marker of a poor prognosis. The death-rate increased threefold for patients with retinopathy if they also had hypertension: 13

Factors related to the development and progression of diabetic retinopathy in patients with type 2 diabetes.

This study was intended to clarify the factors associated with the development and progression of diabetic retinopathy in patients with Type 2 diabetes. A total of 107 patients with Type 2 diabetes underwent fundoscopic examination by an ophthalmologist, and the factors that might be associated with the severity of retinopathy were investigated. Analysis of variance and the chi 2 test were performed to determine whether 22 separate factors were associated with the severity of diabetic retinopathy. There were significant associations between retinopathy and duration of disease, systolic blood p

Research progress on diagnosing retinal vascular diseases based on artificial intelligence and fundus images.

As the only blood vessels that can directly be seen in the whole body, pathological changes in retinal vessels are related to the metabolic state of the whole body and many systems, which seriously affect the vision and quality of life of patients. Timely diagnosis and treatment are key to improving vision prognosis. In recent years, with the rapid development of artificial intelligence, the application of artificial intelligence in ophthalmology has become increasingly extensive and in-depth, especially in the field of retinal vascular diseases. Research study results based on artificial inte

Long-term ocular complications after kidney transplantation.

To assess long-term ocular complications and identify factors affecting these complications in pre-emptive and dialysis patients who have undergone kidney transplantation. We included 548 patients who had a complete ophthalmologic examination at least one year after kidney transplantation. The patients were divided into two groups: those who received dialysis before kidney transplantation (group 1) and those who did not (group 2). We recorded the presence of diabetes mellitus (DM) and hypertension (HT), which could contribute to concomitant retinopathy. Data collected included best corrected v

Microvascular disease: what does the UKPDS tell us about diabetic retinopathy?

Background The UK Prospective Diabetes Study (UKPDS) was a randomised controlled clinical study which looked at the effect of improved blood glucose and blood pressure control on macro- and microvascular complications in Type 2 diabetes. Retinopathy was the commonest microvascular outcome and this paper looks at this complication. Methods Newly diagnosed diabetic patients were randomised to intensive or conventional glycaemic control and a subset of hypertensive patients to tight or less tight blood pressure control. Patients were seen every 3 months in study clinics and retinopathy was assess

Pauci-immune crescentic glomerulonephritis masquerading as goodpasture syndrome: ocular clues in a young adult: a case report.

BACKGROUND: Pauci-immune crescentic glomerulonephritis (PICGN) is a major cause of rapidly progressive renal failure and is commonly linked to ANCA-associated vasculitis. While renal and pulmonary manifestations are typical, posterior segment ocular involvement—particularly hypertensive retinopathy with exudative retinal detachment and retinoschisis—is rarely described. Reporting such cases is important, as these features may mimic other inflammatory choroidopathies and can delay appropriate diagnosis and treatment. CASE PRESENTATION: A 21-year-old previously healthy man developed blurred visi

Increased numbers of caveolae in retinal endothelium and pericytes in hypertensive diabetic rats.

PURPOSE: Long-term clinical studies have now shown that tight control of blood pressure in type 2 diabetes reduces the risk of diabetes-related death and common diabetic complications, including diabetic retinopathy. However, the mechanisms by which hypertension enhances diabetic microvascular disease, especially diabetic retinopathy, are poorly understood. We developed an experimental model of hypertension in diabetic rats and studied the early ultrastructural changes in retinal capillaries under these conditions. METHODS: Hypertension was induced in diabetic BioBreeding (BB) rats by unilate

[Diagnostic significance of perimetry and microperimetry in common retinal diseases].

Perimetry with assessment of the light sensitivity threshold and indices of its loss has wide use in ophthalmology. This review considers three diseases in which static perimetry and microperimetry are used and for which outcome data are available - age-related macular degeneration, diabetic retinopathy, and hypertensive retinopathy. Despite differing opinions on the feasibility of using perimetry in the early stages of these diseases, the obtained data differ from normal values. Microperimetry is more often performed in combination with optical coherence tomography of the retina. Since perime

Oral contraceptives in women with diabetes.

We evaluated the association of oral contraceptive use with the presence and severity of diabetic retinopathy, hypertension, and glycosylated hemoglobin in women of childbearing age who have diabetes. Neither current or past use nor number of years of use of oral contraceptives was associated with severity of retinopathy, hypertension, or current glycosylated hemoglobin. In conclusion, further study of various birth control methods in young women of childbearing age should be considered.

Factors influencing the onset and progression of diabetic retinopathy in subjects with insulin-dependent diabetes mellitus.

BACKGROUND: The etiology of diabetic retinopathy is poorly understood. In the current study, factor associated with the onset and the progression or regression of retinopathy are evaluated. METHODS: Two hundred seventy-seven subjects with insulin-dependent (type I) diabetes mellitus (IDDM) were evaluated longitudinally for retinal changes over a mean of 2.7 years. The multistate Markov model was used to analyze the influences of the duration of diabetes, a family history of hypertension, age, sex, cigarette smoking, systolic blood pressure, diastolic blood pressure, cholesterol levels, and lo

Hypertension, orthostatic hypotension and the microvascular complications of diabetes.

Prevalences of hypertension and orthostatic hypotension and their relationship to the microvascular complications of diabetes were assessed in 702 individuals aged 18-74 years, who had been selected as a representative sample of surviving patients with diabetes diagnosed at the Joslin Clinic between 1939 and 1965. In diabetes of short, long and very long duration, hypertension was 1.7, 1.9 and 2.1 times more frequent, respectively, than in the white U.S. population, regardless of gender. The excess frequency of hypertension in short duration diabetes suggests that some etiologic factor is shar

Hyperhomocysteinemia is associated with the presence of retinopathy in type 2 diabetes mellitus: the Hoorn study.

BACKGROUND: Retinopathy is the leading cause of blindness among patients with type 2 diabetes mellitus (DM). Hyperhomocysteinemia is a recently recognized risk factor for cardiovascular disease, independent of established risk factors. OBJECTIVE: To study the association between the homocysteine level and retinopathy among subjects with and without DM. METHODS: We studied an age-, sex-, and glucose tolerance-stratified random sample of a 50- to 75-year-old general white population in the Hoorn Study (N = 625). Retinal vascular changes (retinopathy) were assessed using ophthalmoscopy and/or f

Role of the angiotensin II type 1 receptor in the pathogenesis of diabetic retinopathy: effects of blood pressure control and beyond.

Diabetic retinopathy is characterized by both functional and morphological changes in the retinal microvessels that can lead to macular edema, neovascularization, and vision loss. Hypertension has been identified as a major risk factor for diabetic retinopathy and randomized clinical trials have shown that reduction of blood pressure using angiotensin converting enzyme (ACE) inhibitors reduces the progression of diabetic retinopathy. The major components of the renin-angiotensin system have been identified in ocular tissues. Activation of angiotensin II type 1 (AT1) receptors expressed on reti

Bilateral Hearing Loss and Tinnitus as Primary Manifestations of Pontine Hemorrhage in a Young Man With Refractory Hypertension.

Pontine hemorrhages are typically associated with profound neurological deficits such as coma and quadriparesis, but selective involvement of the dorsal tegmentum can produce more subtle or atypical symptoms. We report the case of a 32-year-old man with longstanding, treatment-noncompliant hypertension who presented with acute-onset bilateral hearing loss and tinnitus. Head CT revealed an acute hemorrhage localized to the pontine tegmentum, yet the patient remained fully awake and alert. Renovascular causes of hypertension were excluded, but the workup revealed evidence of chronic hypertensive
