Intestinal metabolomic profiling provides insights into the molecular mechanisms for hyperuricemia-induced intestinal barrier dysfunctions in a hyperuricemia mouse model.

Cadmium exposure could damage the liver, which is suggested to be associated with the hyperuricemia (HUA)-induced intestinal barrier injury. To reveal the mechanism for HUA-induced intestinal barrier injury, the HUA mice constructed by knockout (Ko) of the urate oxidase (Uox) gene and their corresponding controls were used for the metabolomics analysis. Clinical biochemistry from the plasma was assessed, and the histopathological changes of the intestines were evaluated. Metabolomics was performed to explore the intestinal metabolomic profiles from the Uox-Ko mice, and the potential metabolic

Perspectives on the Management of Hyperuricemia and Gout in Older Adults.

Various comorbidities and extensive medication regimens often complicate the management of gout in older adults. Even in the general population, managing gout can be challenging, as clinicians must develop strategies for acute gout treatment, assess the need for chronic management, and optimize prophylaxis for gout flares. Several factors influence the choice of pharmacologic therapy, including the patient's medical history, current medications, social and family history, ethnicity, age, and care preferences. This article reviews gout management with a focus on older adults through a case stud

Gout, Hyperuricemia and Psoriatic Arthritis: An Evolving Conundrum.

PURPOSE OF REVIEW: The co-existence of gout and psoriatic disease (PD) is long standing but more recently frequently encountered in clinical settings due to increased awareness of their shared comorbidities and clinical phenotypes, often posing diagnostic and management challenges. Here we review the overlap in gout and PD focusing on shared clinical features, common inflammatory pathophysiology and comorbidities which may prompt a diagnosis of 'Psout' and lead to changes in management. RECENT FINDINGS: Several epidemiological studies have highlighted the increased incidence of hyperuricemia

Perspectives on the Epidemiology of Hyperuricemia and Gout in Older Adults.

As life expectancy increases, the older adult population is expected to grow significantly. As a result, gout cases continue to escalate in both incidence and prevalence. Gout is a chronic inflammatory condition that disproportionately affects older adults, certain racial and ethnic groups, and underserved populations. Risk factors for gout include renal impairment, genetics, select medications, obesity, and lifestyle factors. It is also associated with numerous comorbidities, such as cardiovascular disease, metabolic syndromes, chronic kidney disease (CKD), depression, and anxiety. This artic

Effect of Dietary Counseling on Patients with Asymptomatic Hyperuricemia.

In Japan, hyperuricemia is on the rise. The guideline for the management of hyperuricemia and gout recommends lifestyle changes before beginning drug therapy. This study aimed to evaluate the effectiveness of dietary counseling following the guideline. Thirty-three subjects (24 men and 9 women) with asymptomatic hyperuricemia underwent dietary counseling for 6 months based on the following recommendations : (1) prevent excessive purine intake, (2) prevent excessive fructose intake, (3) limit alcohol drinking, and (4) drink sufficient water. Obese subjects were counseled on adequate energy inta

The biomarkers discovery of hyperuricemia and gout: proteomics and metabolomics.

BACKGROUND: Hyperuricemia and gout are a group of disorders of purine metabolism. In recent years, the incidence of hyperuricemia and gout has been increasing, which is a severe threat to people's health. Several studies on hyperuricemia and gout in proteomics and metabolomics have been conducted recently. Some literature has identified biomarkers that distinguish asymptomatic hyperuricemia from acute gout or remission of gout. We summarize the physiological processes in which these biomarkers may be involved and their role in disease progression. METHODOLOGY: We used professional databases i

[The role of hyperuricemia in the development of cognitive changes in the elderly.].

It is known that hyperuricemia is recognized as an independent cardiovascular risk factor. However, uric acid can also perform useful functions due to its antioxidant properties, which may be especially relevant in the context of neurodegenerative diseases. This review examines the currently available data on the relationship between serum uric acid levels and cognitive functions in the elderly, paying special attention to the etiology of cognitive disorders (Alzheimer's disease, Parkinson's dementia and vascular dementia). Despite the high heterogeneity of existing studies due to the differen

Circulating microRNA alternations in primary hyperuricemia and gout.

OBJECTIVES: MicroRNAs (miRNAs) are short single-stranded RNAs that play a role in the post-transcriptional regulation of gene expression. Their deregulation can be associated with various diseases, such as cancer, neurodegenerative, and immune-related diseases. The aim of our study was to compare miRNA levels in plasma that could potentially influence the progression of hyperuricemia to gout, since the mechanism of progression is still unclear. METHODS: Total RNA, including miRNA, was isolated from the plasma of 45 patients with asymptomatic hyperuricemia, 131 patients with primary gout (incl

Long-term prognostic impact of hyperuricemia in community.

The debate whether an elevated level of serum uric acid (SUA) is an independent marker of cardiovascular risk is still going on. We examined morbidity and mortality related to SUA and hyperuricemia in a well-characterized population with very long follow-up. Study included 4696 participants (aged 30-59 years at baseline) of the coronary heart disease (CHD) Study of the Finnish Mobile Clinic Health Examination Survey. Adjusted hazard ratios (HRs) of hyperuricemia (defined as ≥360 µmol/l and ≥420 µmol/l) and SUA quintiles for mortality and adverse cardiovascular outcomes are reported. During the

Hyperuricemia Associated with Thiazide Diuretics in Hypertensive Adults.

Introduction Thiazide diuretics are essential first-line anti-hypertensive drugs which not only maintain blood pressure but also reduce stroke and congestive heart failure associated with morbidity and mortality in hypertensive patients. However, thiazide diuretics are associated with elevated serum uric acid (SUA) levels. This study aimed to evaluate the impact of thiazide diuretic use on their SUA levels among hypertensive individuals of Pakistan. Methods In this cross-sectional, prospective study, adult hypertensive patients were recruited. They were divided into two groups - thiazide diure

Atypical musculoskeletal manifestations of gout in hyperuricemia patients.

Deposition of tophus is a common feature in chronic gout; however, signs and symptoms are not always well-pronounced in cases of uncommon sites. We report a case series of three patients, one with an acute locked knee suspected to be meniscus tear or intra-articular loose body, a second case with a lump on the dorsal hand suspected to be a benign lesion, and a third case with a finger flexion deformity with carpal tunnel syndrome; all three cases were later on found out to be tophaceous gout. White chalky monosodium urate crystal deposition was found to be the culprit of the symptoms of the pa

Investigational drugs for hyperuricemia, an update on recent developments.

INTRODUCTION: The significant proportion of gout patients not reaching serum urate levels below 6.0 mg/dL and the debated pathogenicity of hyperuricemia (HU) itself motivate investigators to develop new drugs to decrease uricemia. AREAS COVERED: This review discusses the drugs considered to be in active development from pre-clinical to phase III studies. This review covers 11 drugs in development, including a xanthine oxidase inhibitor (topiroxostat), uricosurics (verinurad, arhalofenate, UR-1102, tranilast), dual inhibitors (RLBN1001, KUX-1151), a uricase (pergsiticase), an inhibitor of hypo

Value and Progress of Ultrasound in Diagnosis and Treatment of Hyperuricemia.

Uric acid is the final end-product of purine metabolism and 70% of it is excreted through the kidneys. Chronic hyperuricemia can easily cause gout and meanwhile may affect the kidneys and cardiovascular system. In recent years, the incidences of hyperuricemia and gout have gradually increased. This article summarizes the correlation of hyperuricemia with hyperuricemic nephropathy, gouty arthritis,non-alcoholic fatty liver disease, and carotid atherosclerosis and the role of color Doppler ultrasound in the diagnosis of these lesions.
