IgA deficiency in children. A clinical study with special reference to intestinal findings.

Clinical, immunological, and intestinal studies on 26 children with IgA deficiency in the age range 2 to 16 years are reported. 9 of these children were suffering from autoimmune disease, namely thyroiditis (5), thyrotoxicosis (1), rheumatoid arthritis (2), and probable Sjögren's syndrome (1). The last-mentioned patient had defective cellular immunity. Altogether 11 patients were subject to recurrent respiratory tract infections. The symptomatology of the remaining patients was variable. In a boy with growth retardation, a chromosome anomaly was found, and endocrinological studies indicated to

The association of IgA deficiency but not IgG or IgM deficiency with a reduced patient and graft survival following liver transplantation.

Recipients of solid organ allografts require lifelong immunosuppression in order to prevent graft rejection and to maintain graft function. In general, such immunosuppression greatly impairs the cellular immune system, as this level of the immune system is principally responsible for self and non-self recognition. The consequences of allograft transplantation in terms of patient and graft survival when transplants are given to individuals who have a preexisting humoral immune deficiency characterized by a deficiency of the serum levels of one or more of the major Ig classes have not yet been r

Insights into the clinical spectrum of selective IgA deficiency: Data from two centers.

INTRODUCTION: Selective IgA deficiency (sIgAD) is the most common primary immunodeficiency, yet its clinical presentation ranges from asymptomatic cases to individuals suffering from recurrent infections, allergic manifestations, and autoimmune disorders. Limited data exist regarding the immunological and clinical profiles of pediatric patients with sIgAD in Türkiye. METHODS: We conducted a retrospective analysis of 45 pediatric patients (20 females and 25 males) diagnosed with sIgAD and followed at two tertiary care centers. Demographic features, allergic and autoimmune comorbidities, and im

Extra X, extra questions: Trisomy X syndrome and IgA deficiency - a case report.

While Trisomy X syndrome is typically characterized by developmental and cognitive variations, it is not commonly associated with immunodeficiencies. We report the unique case of a 6-year-old girl with Trisomy X presenting with selective IgA deficiency, challenging the conventional understanding of this chromosomal condition. The patient exhibited recurrent respiratory infections and gastrointestinal symptoms, evaluated in the context of her genetic background of Trisomy X and significantly low levels of IgA (0.03 g/L), yet normal IgG and IgM levels. Immunological assessment revealed a poor re

Selective IgA Deficiency with Multiple Autoimmune Comorbidities: A Case Report and Literature Review.

Individuals with Selective Immunoglobulin-A Deficiency (SIgAD) are often asymptomatic, and symptomatic SIgAD patients often have autoimmune comorbidities. A 48-year-old Han Chinese man presented with abdominal discomfort, hematochezia, and a large tumor in the anogenital region. The primary diagnosis of SIgAD was based on the patient's age, serum IgA concentration (0.067 g/L), and the evidence of chronic respiratory infection. No other immunoglobulin deficiency or evidence of immunosuppression was present. The primary diagnosis of giant condyloma acuminatum was based on human papilloma virus-6

Anti-tissue transglutaminase (tTG) screening using relative light units to identify IgA deficiency.

IgA deficiency is more common in patients with coeliac disease (CD). Total IgA levels are often recommended as part of first line coeliac testing along with anti-tissue transglutaminase (tTG) IgA, to identify these patients and reduce falsely negative results. This study aimed to identify patients with complete IgA deficiency by determining a cut-off threshold on chemiluminescent tTG IgA testing. A chemiluminescent assay QUANTA Flash h-tTG was reviewed using the BIO-FLASH automated platform. tTG relative light units (RLU) were analysed in relation to total IgA levels. Correlation analysis was

Diagnosing and Monitoring Celiac Patients with Selective IgA Deficiency: Still an Open Issue.

Although, the association between celiac disease (CD) and selective immunoglobulin A deficiency (SIgAD) has been known for more than fifty years, the procedures for diagnosing and monitoring patients with both conditions are still far from definitive. When serological markers were introduced as pre-bioptic investigations, it was immediately clear that searching for specific IgA antibodies without checking total serum IgA could lead to a failure in diagnosing IgA-deficient CD patients, while specific IgG antibodies could be useful as additional tests, because they are frequently found in the se

IgA Deficiency Is Not Systematically Ruled Out in Patients Undergoing Celiac Disease Testing.

BACKGROUND: Guidelines for celiac disease (CD) testing recommend total serum IgA determination alongside anti-transglutaminase IgA antibodies. It is not well known if lack of serum IgA determination is a common finding in clinical practice. AIM: To determine the prevalence of lack of serum IgA determination among patients screened for celiac disease. MATERIALS AND METHODS: We identified all subjects who underwent serum anti-transglutaminase IgA and/or other CD-related antibodies determination at a single teaching hospital in Buenos Aires from October 2019 to February 2020. Medical records we

TNFRSF13B/TACI Alterations in Turkish Patients with Common Variable Immunodeficiency and IgA Deficiency.

BACKGROUND: The Transmembrane Activator and Calcium modulator ligand Interactor (TACI), encoded by TNFRSF13B/TACI gene, is mutated in some patients with Common Variable Immunodeficiency (CVID) and IgA Deficiency (IgAD). The purpose of the study was to investigate for the first time in Turkish patients the prevalence of TNFRSF13B alterations in CVID, selective and partial IgAD patients. METHODS: Forty two CVID, 36 selective IgAD, 34 partial IgAD and 25 healthy controls were included. All patients were examined for TNFRSF13B gene mutations by PCR. RESULTS: The percentages of TNFRSF13B mutation

[Salmonella typhi vaccination response study reveals defective antibody production selective IgA deficiency patient].

Selective IgA deficiency (SIgAD) is the most prevalent immunodeficiency worldwide, progressing to common variable immunodeficiency only in few reported cases. We report the case of a Spanish female aged 22 and diagnosed of selective IgA deficiency, a long history of bronchitis, several episodes of pneumonia, bilateral bronchiectasis, normal IgG, IgM, IgG subclasses, and detectable pre-vaccination IgG antibodies against tetanus toxoid and Streptococcus pneumoniae. She was evaluated in our clinic in order to rule out common variable immunodeficiency. We observed good antibody response to tetanus

Association between IgA deficiency & other autoimmune conditions: a population-based matched cohort study.

PURPOSE: To examine autoimmune disorders in patients with IgA deficiency compared with the general population. METHODS: Nationwide prospective population-based cohort study. Through six university hospitals in Sweden we identified 2100 individuals with IgA deficiency (IgA levels < .07 g/L) diagnosed between 1980 and 2011. Each patient with IgA deficiency was matched on age, sex, place of residence, and year of diagnosis with up to 10 general population controls (n = 18,653). Data on nine autoimmune disorders were retrieved from the Swedish National Patient Register (including inpatient and no

Development of IgA nephropathy-like glomerulonephritis associated with Wiskott-Aldrich syndrome protein deficiency.

Wiskott-Aldrich syndrome (WAS) is a rare X-linked disorder caused by mutations in the WAS gene. Glomerulonephritis is a frequent complication, however, histopathological data from affected patients is scarce because the thrombocytopenia that affects most patients is a contraindication to renal biopsies. We found that WASp-deficient mice develop proliferative glomerulonephritis reminiscent of human IgA nephropathy (IgAN). We examined whether increased aberrant IgA production is associated with the development of glomerulonephritis in WASp-deficient mice. Serum IgA and IgA production by splenic

Immunoglobulin-producing cells in secretory immune system in patients with selective IgA deficiency.

This study evaluated the class distribution and J chain-positivity of immunoglobulin-producing cells (Ig-PCs) in gastrointestinal mucosa and salivary glands of Japanese patients with selective IgA deficiency. The proportional patterns of gland-associated Ig-PCs showed an increase in not only IgM cells but also IgG cells, most of which were J chain-positive. This may represent a maturation arrest at B cell differentiation, rather than a compensatory phenomenon. No patients showed an increase of IgD cells in salivary glands. This behavior of IgD cells in Japanese patients may reflect associated

Polyendocrine deficiency syndrome. Occurrence in a patient with depressed IgA titers receiving phenytoin.

A 35-year-old woman had an addisonian crisis and primary adrenal, ovarian, and thyroid failures were detected. Antibodies against the adrenal glands and the thyroid were found, together with consistently low IgA levels. The patient had been receiving phenytoin, a drug known to induce an IgA deficiency, for 20 years. The question arises as to whether this substance was responsible for an immunological defect leading to an autoimmune process that damaged the adrenal glands, the ovaries, and the thyroid.

Genetic and immunologic analysis of a family containing five patients with common-variable immune deficiency or selective IgA deficiency.

A family with 13 members included 2 subjects with selective IgA deficiency (IgA-D) and 3 subjects with common-variable immune deficiency (CVID), diseases which usually occur sporadically. Reciprocal combinations of B and T cells in vitro between one normal and two immune-deficient family members and normal subjects revealed that defective Ig synthesis was determined by the B cells, while the patient T cells functioned normally. Normal T helper and suppressor function was demonstrated even in one patient with CVID who developed a T-cell lymphoproliferative disorder associated with elevated IgM;

Extrahepatic bile duct growth in mice repeatedly injected with human normal serum, IgA-deficient serum or purified secretory IgA.

Another group of researchers has reported that seven intraperitoneal injections into mice of purified human serum IgA or normal human serum--but not IgA-deficient serum, mouse serum or saline--induced considerable growth of the extrahepatic bile duct epithelium. They stated that heterologous IgA was principally responsible for this effect. We have quantitated this growth by another method: enumeration of total and radiolabeled nuclei of the lumenal and glandular epithelium in transverse duct sections after tritiated thymidine injection and autoradiography. Our data show maximal incorporation b

MBL2 deficiency and IgA Nephropathy in transplant patients: Is complement inhibition always beneficial?

IgA nephropathy (IgAN) is a leading cause of End-Stage Renal Disease, with kidney transplantation as the preferred treatment, although with unpredictable and diverse outcomes. Mannose-binding lectin (MBL), a key component of the lectin pathway (LP), is associated with IgAN, and both excessive and deficient levels may exacerbate the disease. Genetic variants in MBL2 affect MBL serum levels and function, potentially influencing graft outcomes. We investigated correlations between MBL2 variants, MBL levels, complement system activation, and graft survival in 64 IgAN transplant recipients. The com
