Osteitis Condensans Ilii: A Case Series.

Osteitis condensans ilii (OCI) is a rare self-limiting low back pain syndrome. It is an incidental imaging discovery around sacroiliac joints with distinctive sclerotic lesions. We present three case reports as a series. In the first case, a 38-year-old female presented with unresolved chronic low back pain for eight years. She had bilateral triangular sclerosis of the ilium abutting sacroiliac joints and other causes of back pain were ruled out. Non-operative management was successful. In the second case, a 38-year-old female presented with acute exacerbation of low back pain for one year. Sh

Sacral agenesis combined with spinopelvic dissociation: A case report and literature review.

INTRODUCTION: Sacral agenesis is a rare congenital disease with radiologic manifestation of sacrum deformity. Its clinical manifestations include spinopelvic instability due to sacroiliac joint deformity, spinal rotation, scoliosis, difficulties in walking, and claudication. Surgical intervention aims to prevent further deformity progression and to improve the patients' walking function. It is challenging to achieve solid arthrodesis for this congenital disease, and fusion failure could aggravate deformity. CASE PRESENTATION: We retrospectively studied one case of a 12-year-old girl with sacr

CT-Based Evaluation of Volumetric Posterior Pelvic Bone Density with Implications for the Percutaneous Screw Fixation of the Sacroiliac Joint.

Background: Operative treatment of fragility fractures of the pelvis has become a gold standard. Preoperative planning, including the assessment of the pathway for iliosacral screws, is crucial. The anchorage of the screw depends on the bone quality. Some recent studies have concentrated on assessing bone mineral density (BMD) with the use of Hounsfield unit (HU) values obtained from CT scans. The aim of the present study is to determine the best sacral levels of S1-S3 on the pathway of iliosacral screws for sacroiliac joint fixation. Methods: Patients admitted to the Independent Public Health

Cervical compensatory alignment changes following correction of adult thoracic deformity: a multicenter experience in 57 patients with a 2-year follow-up.

OBJECT Alignment changes in the cervical spine that occur following surgical correction for thoracic deformity remain poorly understood. The purpose of this study was to evaluate such changes in a cohort of adults with thoracic deformity treated surgically. METHODS The authors conducted a multicenter retrospective analysis of consecutive patients with thoracic deformity. Inclusion criteria for this study were as follows: corrective osteotomy for thoracic deformity, upper-most instrumented vertebra (UIV) between T-1 and T-4, lower-most instrumented vertebra (LIV) at or above L-5 (LIV ≥ L-5) or

Burkitt's Lymphoma/Leukemia in a 15-Year-Old Male.

We present the case of a 15-year-old male with sporadic Burkitt's lymphoma/leukemia. The patient presented with right lower quadrant abdominal pain and masses in the terminal ilium and pelvis, and was subsequently demonstrated to have involvement of the bone marrow. We discuss differential diagnoses and approach to diagnose and stage this disease. A review of the clinical, radiologic, and pathologic features of Burkitt's lymphoma/leukemia are also presented.

[Isthmic spondylolisthesis with spontaneous development in adults. Apropos of 8 cases].

Eight cases of progressive deterioration in spondylolisthesis in adults have been observed. In 7 cases, the displacement was between the 4th and 5th lumbar vertebra and only once between the 5th lumbar vertebra and the sacrum. The mean age of the patients was 37 years. Several factors leading to this deterioration are considered - disc degeneration, antero-posterior imbalance of the spine and inadequate fixation of the 4th lumbar vertebra to the ilium.

Osteitis condensans of the clavicle: does fibrocartilage play a role? A report of two cases with spontaneous clinical and roentgenographic resolution.

Two cases of osteitis condensans of the medial clavicle with an unusual course toward spontaneous resolution of clinical and roentgenographic manifestations are reported. The relevant literature is reviewed. A pathogenic hypothesis for condensing osteitis of the clavicle, ilium, and pubis is suggested. At all these sites, the sclerosis occurs in bone overlaid by fibrocartilage. In contrast, joint aspects spared by the sclerosis are covered with hyaline cartilage. We report data documenting this fact at the sternoclavicular joint.

[Systematic hip roentgenograms in infants of 4 months of age in the screening for congenital dislocation of the hip (author's transl)].

The authors report the results of a statistical study undertaken for the screening of congenital dislocation of the hip. 961 children born between September 24, 1977 and June 30, 1978 were examined. Congenital dislocation of the hip was diagnosed in 9 (0.9 % of the cases). Out of these, 4 had no clinical sign of dislocatability at birth. The only determination of acetabular angles cannot be relied upon and qualitative criteria such as ilium thickness must be added.

Ilio-femoral impingement angle: a new radiographic measurement.

BACKGROUND: Hip dislocations after total hip arthroplasty (THA) are a debilitating complication, often occurring in deep flexion. Current 3D modelling allows for simulating the flexion angle at which bone-on-bone impingement occurs, but it is resource-intensive and not widely available for routine clinical use. Therefore, we propose the ilio-femoral impingement angle (IFIa) as a novel, simple radiographic measure to help identify patients at higher risk for dislocation using standard radiographs. METHODS: A radiographic study was performed on 117 consecutive patients who underwent preoperativ

Long-Term Survival After Multidisciplinary Treatment Including Surgery for Metachronous Metastases of Small Intestinal Gastrointestinal Stromal Tumors after Curative Resection: A Case Report.

BACKGROUND Currently, 3 molecular targeted drugs are available for the treatment of unresectable and recurrent gastrointestinal stromal tumors (GISTs), and result in improved prognoses and rare occurrence of bone metastases. However, there is no established treatment guideline for bone metastases of GIST. CASE REPORT The patient was a 56-year-old male who was diagnosed with leiomyosarcoma in 1997. Partial resection of the small bowel was performed. As part of post-operative follow-up in 2004, a computed tomography scan showed metastatic lesions in the liver and the right femoral neck. Treatment of massive acetabular bone loss and pelvic discontinuity with a custom triflange component and ilio-sacral fixation based on preoperative CT templating. A report of 2 cases.

Revision rates for total hip arthroplasty are increasing and pelvic discontinuity is estimated to be present in 1% to 5% (Berry). Discontinuity is defined as a separation of the cephalad portion of the pelvis from the caudad portion (AAOS Type IV defects). This results from bone loss secondary to osteolysis, infection, fracture, or mechanical loosening. The goals of revision surgery in this setting are to obtain secure fixation of the acetabular component with or without union of the discontinuity. Many methods exist for treating this problem. Results with allograft and cage fixation have gene

Curved periacetabular osteotomy for treatment of dysplastic hip.

UNLABELLED: The Bernese periacetabular osteotomy has a considerable rate of postoperative complications such as reflex sympathetic dystrophy, motor nerve palsy, heterotopic ossification, and delayed union of the ilium, which are assumed to be caused by extensive exposure or asphericity of the osteotomy surfaces. To address these issues, we developed the curved periacetabular osteotomy, a modification of the Bernese periacetabular osteotomy which limits dissection, prevents the outside of the ilium from being exposed, and produces osteotomy surfaces with the same curvature. Curved periacetabula

Proximal focal femoral deficiency -- a rare entity in the sonographic differential diagnosis of developmental dysplasia of the hip,

In the present study, we will describe the differential diagnosis of the rare hip anomaly of proximal focal femoral deficiency (PFFD), based on an analysis of 12,488 ultrasound images of the hips of 6244 neonates, examined in our orthopaedic clinics between 1988 and 1998. The clinical manifestations and ultrasound features of PFFD will be characterised and compared with those of the normal, the mildly dysplastic, and the severely dysplastic hip. Two cases of PFFD (0.032 %) were detected during the investigation period. Both neonates had been referred to the orthopaedic clinic for further evalu

Growth and development of the acetabulum in the normal child. Anatomical, histological, and roentgenographic studies.

Postmortem studies of ten normal full-term infants and of three children, seven, nine, and fourteen years old, showed that the acetabular cartilage complex is a unit that is triradiate medially and cup-shaped laterally and is interposed between the ilium, ischium, and pubis. This complex is composed of epiphyseal growth-plate cartilage adjacent to these bones, of articular cartilage adjacent to these bones, of articular cartilage around the acetabular cavity, and, for the most part, of hyaline carilage. Interstitial growth within the triradiate part of the cartilage complex causes the hip sock

[Ileosigmoid volvulus. A case report].

This is a case report of an ileosigmoid volvulus in a 68 year old man. The patient had emergency surgery for an acute abdominal condition. Intraoperative diagnosis of an ileosigmoid knot was made and it was necessary to resect the necrotic portion of the ilium, with ilioiliostomy, and to resect the gangrenous section of the sigmoid colon, end colostomy, and Hartmann pouch. Eleven weeks later, a colosigmoidostomy was realized. The evolution of the patient was satisfactory. The literature about ileosigmoid volvulus is reviewed.

A large iliopsoas abscess due to colon cancer complicated by bowel obstruction: A case report.

INTRODUCTION: Iliopsoas abscesses (IPAs) associated with bowel obstruction due to colon cancer are rare, and there is no consensus regarding treatment strategies. PRESENTATION OF CASE: A 63-year-old man presented with swelling and pain in the right iliac region. Imaging studies revealed an IPA expanding from the psoas major muscle and retroperitoneal space subcutaneously around the right ilium. After percutaneous drainage, the patient developed bowel obstruction secondary to colon cancer. Hemicolectomy and preventive ileostomy were performed at the gastrointestinal surgery department, and che

The indication of fusion to the pelvis in neuromuscular scoliosis is based on the underlying disease rather than on pelvic obliquity.

INTRODUCTION: The decision to instrument to L5 or ilium, in NMS, is usually based on radiologic factors, including pelvic obliquity (PO) > 15°, apex of curvature < L3, and Cobb angle > 60°. Since scoliosis in these patients is caused by a neurologic disease, we based our decision to stop at L5 on the presence of spasticity or flaccidity. PATIENTS & METHODS: The senior author did 109 primary fusions in NMS. Of those with DMD or SMA only 16% were instrumented to the ilium. The main factor for our decision was the correction potential of the truncal shift and PO in the supine traction radiograph

Acute osteomyelitis--special cases with particularities related to specific locations.

In the clinical practice of paediatric orthopaedic surgery, there are a few cases of acute osteomyelitis which are not characterized by general and local infectious syndrome, making the diagnosis difficult and leading to severe complications. The specific locations, seldom affected, are the spine, pubis, ilium, ischium, sacrum and coccyx. Comparing with other regions, spinal osteomyelitis is less frequent (1:80). It affects not only children, sometimes aged under one year, but also adults of 40-50 years old. The infection may be located in the vertebral body or vertebral arch. Osteomyelitis of

Pelvic instability after bone graft harvesting from posterior iliac crest: report of nine patients.

OBJECTIVE: To report the imaging findings in nine patients who developed pelvic instability after bone graft harvest from the posterior aspect of the iliac crest. DESIGN AND PATIENTS: A retrospective study was performed of the imaging studies of nine patients who developed pelvic pain after autologous bone graft was harvested from the posterior aspect of the ilium for spinal arthrodesis. Plain films, bone scans, and CT and MR examinations of the pelvis were reviewed. Pertinent aspects of the clinical history of these patients were noted, including age, gender and clinical symptoms. RESULTS:

Displaced ischial stress fracture following revision total hip arthroplasty.

Stress fractures of the ischium are uncommon and are most likely caused by excessive stretching or contracture of the hamstring muscles. In addition, revision total hip arthroplasty (THA) may weaken the ilium, and metabolic bone disease may also contribute to a fracture. Treatment is usually conservative and prognosis is favorable. We present a rare case of spontaneous displaced fracture of the entire ischium following revision THA that healed without requiring operative intervention.
