Association of polypharmacy and potential drug-drug interactions with adverse treatment outcomes in older adults with advanced cancer.

BACKGROUND: Polypharmacy is common in older adults who are starting cancer treatment and is associated with an increased risk of potentially inappropriate medications (PIMs) and potential drug-drug interactions (PDIs). The authors evaluated the association of medication measures with adverse outcomes in older adults with advanced cancer who were receiving systemic therapy. METHODS: This secondary analysis from GAP 70+ Trial (ClinicalTrials.gov identifier NCT02054741; principal investigator, Supriya G. Mohile) enrolled patients aged 70 years and older with advanced cancer who planned to start

Comparative analysis of potentially inappropriate medication use in long-term care facility residents and community-dwelling elders: A matched cohort study.

As the population of the elderly in long-term care facilities has grown, the number of users of potentially inappropriate medication (PIM) is also increasing. With this study, we aimed to investigate the pattern of PIM usage and related factors among the elderly receiving long-term care services. Using the South Korean National Health Insurance Service Elderly Cohort Database, we conducted a retrospective matched cohort study. Elderly residents (n = 1980) in long-term care facilities in 2013 were selected and matched 1:1 with elderly persons living in the community applying propensity score me

Potentially Inappropriate Medication Use in Patients with Dementia.

The objective of this study was to characterize the epidemiology of using potentially inappropriate medications associated with dementia exacerbation (DPIMs) in elderly outpatients with dementia. Electronic medical records were retrospectively reviewed for geriatric patients with dementia who were prescribed at least one medication in 2016 at a tertiary, university-affiliated hospital. The 2015 Beers criteria were used to define DPIMs. Logistic regression was performed to identify factors associated with prescribing DPIMs in patients with dementia. Among 2100 patients included in our study, 98

Medication burden and inappropriate prescription risk among elderly with advanced chronic kidney disease.

BACKGROUND: Elderly patients with chronic kidney disease (CKD) frequently present comorbidities that put them at risk of polypharmacy and medication-related problems. This study aims to describe the overall medication profile of patients aged ≥75 years with advanced CKD from a multicenter French study and specifically the renally (RIMs) and potentially inappropriate-for-the-elderly medications (PIMs) that they take. METHODS: This is a cross-sectional analysis of medication profiles of individuals aged ≥75 years with eGFR < 20 ml/min/1.73 m2 followed by a nephrologist, who collected their acti

Implementing prescriber-pharmacist collaboration to improve evidence-based medication prescribing using asynchronous, non-interruptive electronic health record notifications.

BACKGROUND: Inappropriate prescribing of Direct Oral Anticoagulants (DOACs) is a leading cause of adverse outcomes. Electronic health record (EHR)-based notification strategies may support evidence-based prescribing and reduce adverse events. Engaging clinical pharmacists (vs. prescribers) through EHR-based notifications that review inappropriate DOAC prescribing may be an effective strategy for ensuring evidence-based medication prescribing. METHODS: We conducted a pragmatic, single-center, parallel-group, randomized implementation trial using notifications (asynchronous EHR-based notificati

A Missouri state-wide assessment of antibiotic appropriateness in outpatient treatment of common bacterial and viral infections.

Approximately 30% of U.S. outpatient antibiotic prescriptions are unnecessary. Using 2016-2022 MarketScan data, this study assessed Missouri outpatient prescribing. Among several bacterial infections, 62.4% were inappropriate; antibiotics were prescribed for 18.3% of viral infections. Inappropriate prescribing was higher in rural areas. This method can assess regional condition-specific antibiotic stewardship targets.

Ophthalmic Antimicrobial Prescribing in Australian Healthcare Facilities.

The National Antimicrobial Prescribing Survey (NAPS) is a web-based, standardized tool, widely adopted in Australian healthcare facilities to assess the reasons for, the quantity of, and the quality of antimicrobial prescribing. It consists of multiple modules tailored towards the needs of a variety of healthcare facilities. Data regarding ophthalmological antimicrobial use from Hospital NAPS, Surgical NAPS, and Aged Care NAPS were analysed. In Hospital NAPS, the most common reasons for inappropriate prescribing were incorrect dose or frequency and incorrect duration. Prolonged duration was al

Approach to the Patient Considering Thyroid Hormone Deprescribing.

There is widespread and increasing thyroid hormone overuse. Levothyroxine, a synthetic form of thyroid hormone, is one of the most prescribed medications in the United States. Many patients are started on levothyroxine for the treatment of mild subclinical hypothyroidism or nonevidence-based indications (eg, euthyroid individuals with weight gain or depression). High-certainty evidence does not show a benefit of thyroid hormone initiation in important patient outcomes (eg, quality of life, hypothyroid symptoms) for patients with subclinical hypothyroidism. Moreover, thyroid hormone treatment i

Guidelines and tools to assess appropriateness of diuretic prescribing and aid deprescribing: a systematic review.

Diuretics are widely used as first-line therapy for heart failure and hypertension in older adults, but long-term continuation without a clear indication can increase adverse drug events such as electrolyte disturbances, renal dysfunction, hypotension, syncope, falls, and reduced quality of life-especially in people with multimorbidity, frailty, and polypharmacy. In this systematic review of international clinical practice guidelines and expert tools published up to August, 2025, we identified 41 resources (14 guidelines, 27 tools) and synthesised 184 unique recommendations on inappropriate ch

Deprescribing tools and guidelines in chronic kidney disease: A scoping review.

BACKGROUND: Chronic kidney disease (CKD) is a major global health concern that is associated with multiple complications and comorbidities, leading to polypharmacy, inappropriate prescribing, and increased risk of adverse drug events. Deprescribing has emerged as an effective strategy to mitigate these consequences. Evidence-based guidelines are essential to support appropriate deprescribing practices in this population. A variety of deprescribing tools and guidelines are now widely available, but little is known about their utility in CKD setting. This study aimed to identify and characterize

Inappropriate ceftriaxone utilization and predictor factors in Ethiopia: a systematic review and meta-analysis.

Ceftriaxone stands as a cornerstone in global antibiotic therapy owing to its potent antibacterial activity, broad spectrum coverage, and low toxicity. Nevertheless, its efficacy is impeded by widespread inappropriate prescribing and utilization practices, significantly contributing to bacterial resistance. The aim of this study is to determine the overall national pooled prevalence of inappropriate ceftriaxone utilization and its predictor factors in Ethiopia. A systematic search was conducted across multiple databases including, PubMed, Science Direct, Hinari, Global Index Medicus, Scopus, E

Prescribing cascades among hospitalized older adults: a retrospective cohort study from the REPOSI registry.

A prescribing cascade occurs when an adverse drug reaction (ADR) generated from an index drug (Drug A) is mistaken for a new medical condition that is in turn managed with a new drug prescription (Drug B). Prescribing cascades represent a factor influencing inappropriate prescribing and unnecessary polypharmacy, especially among older adults. Using the REPOSI registry, we conduct a retrospective observational study to examine the prevalence of potential prescribing cascades, as identified in the ThinkCascades framework, among patients aged ≥ 65 years admitted to geriatric or internal medicine
