[Occurrence of retinopathy of prematurity at the Koprivnica Hospital during 2 5-year periods].

The prevalence of retinopathy of prematurity (ROP) in the Koprivnica General Hospital among infants weighting < or = 1500 g at birth and with < or = 31 weeks of gestation, increased in the second of the two five-year periods (I 1984-1988 and II 1989-1993). In period I only one infant with ROP in stage 4 was diagnosed during hospitalization. In period II prevalence of ROP in infants with birth weight < or = 1500 g was 56.5% and in newborns whose gestational age was < or = 31 weeks it was 28.9%. The ROP group of infants had significantly lower birth weight and gestational age. Oxygen therapy and

Effect of posture on the thermal efficiency of a plastic bag wrapping in neonate: assessment using a thermal "sweating" mannequin.

To assess the various heat exchanges with the environment a multisegment, anthropometric, thermal mannequin representing a neonate with a birth weight of 900 g has been designed. The mannequin simulates not only dry heat loss (radiative+conductive+convective body heat exchanges) but also the evaporative skin water loss which can be encountered in low-birth-weight neonates. The model was placed in the supine or prone position in a closed incubator (air temperature, 33 C; relative air humidity, 50%; air velocity below 0.1 m s(-1)). Experiments were performed with the mannequin either naked or wr

Collodion baby dehydration: the danger of high transepidermal water loss.

We describe transepidermal water loss (TEWL) measurements in a collodion baby suffering from severe hypernatraemic dehydration and hypothermia, who required intravenous fluid therapy in a special incubator. The TEWL values 4 days after birth were abnormally high compared with normal infants of the same age. The TEWL measurements returned towards normal within the first month, in parallel with the improvement of both the skin signs and the electrolyte and fluid balance.

Preoperative cardiorespiratory trends in infants with congenital diaphragmatic hernia.

The objective of this study was to determine the cardiovascular and pulmonary adaptations of infants with congenital diaphragmatic hernia (CDH) from birth until delayed surgery through the use of continuous monitoring. Continuous cardiovascular (HR, heart rate variability [HR-SD], BP, blood pressure variability [BP-SD], and oxygen saturation) and ventilatory (minute volume, airway pressure, and effective compliance) measurements were made on-line, using a computerized whole-body plethysmograph-incubator (Vital-trends, VT1000), in nine ventilated infants with CDH. Phagocytosis of group B streptococcus by neutrophils from newborn infants.

The purpose of this study was to investigate the effect of severe "stress" on neonatal neutrophil (PMN) phagocytosis, and to compare the ability of neonatal and adult PMNs to ingest [3H]-labeled type Ic group B streptococcus (GBS). Three patient populations were studied: healthy and "stressed" newborn infants and healthy adults. Severe "stress" was defined as an acute, noninfectious respiratory illness, sepsis or severe birth asphyxia; 94% of these infants required assisted ventilation. Radiolabeled GBS was opsonized with 5% adult serum, and incubated with PMN monolayers adherent to glass cove

Neonatal encephalopathy is associated with altered perinatal systemic neutrophil apoptosis.

Systemic hypoxia-ischemia at birth may alter the neonatal neutrophil phenotype. In this study, we evaluated alterations in perinatal neutrophil phenotype following systemic hypoxia-ischemia compared with normal controls. Neutrophils from adults (n = 15), normal newborns (n = 20), newborns requiring resuscitation at birth (n = 17), and their respective maternal samples were incubated alone or with lipopolysaccharide (LPS). Surface receptor CD11b (neutrophil activation) and the percentage apoptosis (persistence of inflammatory response) were assessed using flow cytometry. Neutrophil apoptosis wa

Some factors affecting mother-child relationships following low birthweight delivery in a Nigerian sociocultural environment.

Twenty mothers with low birthweight babies, cared for in neonatal unit and kept in incubators (as index group) and 20 mothers with full-term babies (as control group) were studied to determine the extent of maternal separation and deprivation following delivery and the effects of these on mother-child relationships. The population was prospectively sampled and an interview method using a structured questionnaire, was adopted to obtain the information for the study. The result showed a delayed post-partum contact, negative reactions and some delays in maternal attachment in mothers of premature

Bacillus Calmette Guerin vaccination of human newborns induces a specific, functional CD8+ T cell response.

Mounting evidence points to CD8+ T cells playing an important role in protective immunity against Mycobacterium tuberculosis. The only available vaccine against tuberculosis, bacillus Calmette Guérin (BCG), has traditionally been viewed not to induce these cells optimally. In this study, we show that vaccination of human newborns with BCG does indeed induce a specific CD8+ T cell response. These cells degranulated or secreted IFN-gamma, but not both, when infant blood was incubated with BCG. This stimulation also resulted in proliferation and up-regulation of cytotoxic molecules. Overall, the

Skin-to-skin contact at birth for very preterm infants and symptoms of depression and anxiety in parents during the first year - A secondary outcome of a randomized clinical trial.

BACKGROUND: Mental health issues are common in parents to preterm infants. In the neonatal intensive care unit (NICU), skin-to-skin contact (SSC) as an avenue for parent-infant closeness may improve parental mental health. Few studies exist regarding its benefits when initiated immediately postbirth. The aim was to determine the effect of SSC for very preterm infants at birth on parents' mental health, by assessing symptoms of depression and anxiety within the first year. METHODS: The Immediate Parent-Infant Skin-to-Skin Study was a randomized clinical trial conducted 2018-2021 at three NICUs

[Morphologic changes of the nucleolar system of lymphocytes as the indicator of sensitivity to tuberculin and Toxoplasma antigen].

A study was made of the nucleolar apparatus of peripheral blood lymphocytes of the newborns, infants and their mothers before and after incubation in vitro with tuberculin or toxoplasmic antigen. It was established that in the presence of sensitization to one or another antigen, there was an increase in the relative count of lymphocytes having a zone of clarification near nucleoli. In children vaccinated with BCG, such an effect was noted in 81.3% of the cases, whereas in children with verified toxoplasmosis, in all the cases. It is recommended that the method designed might be used as alterna

Cytoplasmic receptor for glucocorticoids in lung of the human fetus and neonate.

In fetal animals, glucocorticoids accelerate development of the lung and cause precocious appearance of alveolar surfactant. To determine if the human lung also can respond to corticosteroids, we examined lungs of the human fetus and neonate for both cytoplasmic binding and nuclear uptake of glucocorticoids. In slices of fetal lung incubated with [3H]dexamethasone at 2 degrees C, specific macromolecular binding occurs primarily in the "cytoplasmic" fraction. After further incubation at 37 degrees C. nearly 75% of the radioactivity localizes in the "nuclear" fraction with a concentration of 0.3

Technical feasibility of umbilical cannulation in midgestation lambs supported by the EXTra-uterine Environment for Neonatal Development (EXTEND).

EXTEND (EXTra-uterine Environment for Neonatal Development) is a novel system for supporting extremely premature infants that replicates in utero conditions by maintaining a sterile fluid environment and providing gas exchange via a pumpless arteriovenous oxygenator circuit connected to the umbilical vessels. Target gestational age (GA) for EXTEND support in human infants is 23-27 weeks, when immature lungs are most susceptible to injury in the setting of air ventilation. We previously demonstrated physiologic support of premature lambs cannulated at 105-117 days GA (lungs developmentally anal

Staphylococcus aureus Enterotoxin Production in Raw and Pasteurized Milk: The Effect of Selected Different Storage Durations and Temperatures.

Introduction:S. aureus is one of the most prevalent potential pathogenic bacteria found in DHM. Some strains produce heat stable enterotoxins that are able to survive pasteurization. These enterotoxins have been associated with gastritis and potentially necrotizing enterocolitis in preterm infants. The aim of this study is to assess the impact of different storage temperatures on Staphylococcus aureus growth and enterotoxin A and B production in raw and Holder pasteurized donor human milk (DHM). Materials and Methods: Raw and pasteurized DHM were inoculated with enterotoxin A and B producing S

Streptococcus pneumoniae exposure is associated with human metapneumovirus seroconversion and increased susceptibility to in vitro HMPV infection.

It remains largely unknown which factors determine the clinical outcome of human metapneumovirus (HMPV) infections. The aim of the present study was to analyse whether exposure to bacterial pathogens can influence HMPV infections. From 57 children, serum samples and colonization data for Haemophilus influenzae, Moraxella catarrhalis, Staphylococcus aureus and Streptococcus pneumoniae were collected at 1.5, 6, 14 and 24 months of age. Seroconversion rates to HMPV were determined and related to bacterial carriage. Frequent nasopharyngeal carriage (≥2 times in the first 2 years of life) of S. pne

Kangaroo care: cardio-respiratory relationships between the infant and caregiver.

BACKGROUND: Kangaroo care, i.e., skin-to-skin cohabitation (SSC) between an infant and caregiver, is often used in neonatal intensive care units to promote bonding, breastfeeding and infant growth. The direct salutary effects of SSC on cardio-respiratory control in preterm infants remain equivocal; some reports suggest improved breathing stability, others indicate worsening of apnea, bradycardia and hypoxemia. AIM: The purpose of this study was to investigate physiological relationships between the infant and caregiver during SSC. We hypothesized that respiratory stability of the premature in
