Endoscopic transnasal approach to pterygopalatine fossa and infratemporal fossa: case series utilizing endoscopic modified medial maxillectomy (EMMM)/direct approach to the anterior and lateral part of the maxillary sinus (DALMA).

BACKGROUND: Endoscopic transnasal surgery (ETS) has become an established treatment for skull base lesions; however, its lateral reach to the pterygopalatine fossa (PPF) and infratemporal fossa (ITF) remains limited due to restricted maneuverability within the nasal corridor. To address this limitation, the endoscopic modified medial maxillectomy (EMMM) and the direct approach to the anterior and lateral part of the maxillary sinus (DALMA) provides lateral access through the maxillary sinus. This study evaluated the technical feasibility and outcomes of ETS combining EMMM and DALMA. METHOD: A

Combined endoscopic endonasal & transoral approach to transpatial lesion involving the infratemporal fossa and masticator space: A case study and literature review.

Transpatial skull base lesions involving the infratemporal fossa (ITF) are challenging due to the complex neurovascular structures of the region. Open approaches have traditionally been utilized to access these spaces. We present a 55-year-old woman presented with a mesenchymal mass involving the left ITF and masticator space. A combined endoscopic endonasal transpterygoid approach was performed followed by an endoscopic transoral-transmandibular corridor to access and resect the tumor. The post-operative course was unremarkable with no recurrence during her follow-up. Combined endoscopic appr

Management of a recurrent right juvenile nasopharyngeal angiofibroma using direct tumorous puncture embolisation and a subtemporal-preauricular infratemporal fossa surgical approach.

A juvenile nasopharyngeal angiofibroma (JNA) is a benign vascular tumour that arises from the pterygopalatine fossa. It is seen near exclusively in young males though female cases have been reported. Symptoms are due to their high vascularity and mass effect. Commonly reported symptoms include: nasal obstruction, epistaxis and nasal discharge. The mainstay of treatment is surgical resection either via an endonasal endoscopic approach or open surgical resection. Preoperative embolisation has been shown to decrease intraoperative bleeding. Embolisation may be undertaken via a transarterial (TA)

Anatomic Step-by-Step Dissection of Complex Skull Base Approaches for Trainees: Surgical Anatomy of the Infratemporal Fossa Approach to the Jugular Foramen.

BACKGROUND AND OBJECTIVES: The infratemporal fossa (ITF) is a complex region bounded by the temporal bone, maxilla, sphenoid, pterygoid plates, and mandibular ramus. Containing a high density of neurovascular and musculoskeletal structures, the ITF can house a number of pathologies, and access is challenging. The ITF approach and its variations can be challenging due to complex anatomy and unfamiliarity by many surgeons. The objective of this study was to present a step-by-step 3-dimensional anatomic dissection for the classic Fisch Type A and modified ITF approach from the surgeon's perspecti

Characterization of Anatomical Landmarks for Exposing the Internal Carotid Artery in the Infratemporal Fossa Through an Endoscopic Transmasticator Approach: A Morphometric Cadaveric Study.

BACKGROUND: The Eustachian tube and sphenoid spine have been previously described as landmarks for endonasal surgical identification of the most distal segment of the parapharyngeal internal carotid artery (PhICA). However, the intervening space between the sphenoid spine and PhICA allows for error during exposure of the artery. In the present study, we have characterized endoscopic endonasal transmasticator exposure of the PhICA using the sphenoid spine, vaginal process of the tympanic bone, and the "tympanic crest" as useful anatomical landmarks. METHODS: Endonasal dissection was performed

Functional Reconstruction of Temporomandibular Joint after Resection of Pigmented Villonodular Synovitis with Extension to Infratemporal Fossa and Skull Base: A Case Report.

Introduction  Pigmented villonodular synovitis (PVNS) is a benign but aggressive lesion arising from sinovia. The temporomandibular joint (TMJ) is hardly ever involved. Methods  We describe a case of PVNS arising in the left TMJ involving infratemporal fossa soft tissue and the skull base; we also present the reconstruction. Results  A 37-year-old woman had progressive mandibular swelling for 6 months. Computed tomography of the skull revealed an osteolytic lesion in the left TMJ, involving the upper mandible, condyle, and glenoid fossa and extending to the infratemporal fossa and fossa media

Neuroendoscopic Resection of Trigeminal Schwannoma in the Pterygopalatine/Infratemporal Fossa via the Transnasal Perpendicular Plate Palatine Bone or Transnasal Maxillary Sinus Approach.

BACKGROUND: Both the pterygopalatine fossa (PPF) and the infratemporal fossa (ITF) lie outside the midline of the skull base. Lesions in the PPF or ITF include trigeminal schwannoma (trigeminal schwannoma, TS), which originates from the second or third branch of the trigeminal nerve (maxillary nerve or mandibular nerve). Due to their typically deep anatomic location, lesions in the PPF or ITF can be difficult to treat using traditional surgical approaches. In recent years, because of their advantages, which include the fact that they allow the problem to be observed close up, neuroendoscopic t

Successful endoscopic resection of a rare extension of peripheral giant cell granuloma into the infratemporal fossa via the maxillary sinus: A case report study.

INTRODUCTION AND SIGNIFICANCE: Peripheral giant cell granuloma (PGCG) is a benign reactive lesion commonly found in the mandible but rarely in the maxillary sinus and extending to the infratemporal fossa, which poses a major challenge in excision in this case. It can occasionally lead to bone erosion. Complete surgical excision is the primary treatment. CASE PRESENTATION: We present a rare case of PGCG in the maxillary sinus of a male patient following the extraction of the maxillary second molar, resulting in destruction of the bony margins. The patient underwent complete endoscopic resectio

BACKGROUND: Trigeminal neuralgia may be treated via percutaneous access to the foramen ovale (FO). Vascular complications associated with the needle trajectory can result in serious morbidity and mortality. This study aimed to correlate the vascular relationships of the FO at the skull base via cadaveric dissections and computed tomography (CT). METHODS: Two fresh cadaver heads were injected with red and blue latex to delineate arteries and veins. Neck and infratemporal fossa dissections were carried out to delineate the vascular relationships of the FO. High-resolution head CT images of adul

Combined unilateral coronal and intraoral approach with zygomatic arch osteotomy for a dumbbell-shaped atypical lipomatous tumor of the temporal region extending into the infratemporal fossa.

Atypical lipomatous tumor (ALT) is rare in the head and neck, and dumbbell-shaped lesions beneath the zygomatic arch can complicate access. A 58-year-old man presented with an enlarging right temporal mass. Contrast-enhanced computed tomography and noncontrast magnetic resonance imaging showed a 125-mm lipomatous dumbbell-shaped tumor extending into the infratemporal fossa. Incisional biopsy suggested ALT. Marginal en bloc resection was performed using a unilateral coronal scalp incision and intraoral approach with zygomatic arch osteotomy and titanium plate fixation. Prospective evaluation of locoregional control in oral cavity squamous cell carcinoma with lower or mid infratemporal fossa involvement treated with surgery and adjuvant concurrent chemoradiotherapy.

PURPOSE: Oral cavity squamous cell carcinoma (OCSCC) is the most prevalent malignancy of the head and neck malignancy in India. In locally advanced OCSCC, a combination of definitive surgery followed by postoperative radiotherapy (RT), with or without concurrent chemotherapy, offers superior oncologic outcomes compared to definitive chemoradiotherapy alone. The prognosis of T4b OCSCC with infratemporal fossa (ITF) involvement largely depended on the degree of invasion. Tumours confined to the lower or mid ITF tend to have relatively better outcomes than those with extensive or high-level invol

Gross Total Resection of a Recurrent in-and-around Cavernous Sinus Meningioma through a Combined Transcavernous Anterior and Middle Infratemporal Fossa Approach with Extracranial-Intracranial Bypass.

Objectives  The study aims to describe surgical management of an invasive cavernous sinus meningioma with a combination of several skull base approaches and bypass surgery. Design  This study is an operative video. Results  Resection of the recurrent skull base meningioma is still challenging, especially if the tumor involves or encases the carotid artery. Cerebral bypass surgery is an essential adjunct in the armamentarium of skull base surgery when vessel reconstruction is required. In this paper, we describe our experience of successful treatment of an invasive recurrent skull base meningio

Perforation of the inferior alveolar nerve by the maxillary artery: an anatomical study.

The infratemporal fossa is a clinically important anatomical area for the delivery of local anaesthetic agents in dentistry and maxillofacial surgery. We studied the infratemporal fossas in white cadavers, and in particular the topographical relations of the inferior alveolar nerve and the maxillary artery. In 3 of the 50 fossas dissected the maxillary artery passed through the inferior alveolar nerve, splitting it into superficial and deep divisions. Entrapment of the maxillary artery may cause numbness or headache and may interfere with injection of local anaesthetics into the infratemporal

Synovial chondromatosis of the temporomandibular joint with middle cranial fossa extension.

Synovial chondromatosis of the temporomandibular joint (TMJ) is relatively rare. An unusual case with extension through the glenoid fossa and into the middle cranial fossa is reported. Invasion of the infratemporal fossa and the middle cranial fossa was seen on both computed tomography and magnetic resonance imaging. Complete removal of the loose bodies with excision of the affected synovium is the accepted treatment of synovial chondromatosis. A conservative approach should be followed while trying to eliminate any remaining lesion in the infratemporal fossa and the middle cranial fossa. An o
