Advances in population-based interventions to control falciparum malaria.

Malaria is a complex disease and transmission can be prevented in multiple ways. A range of interventions to achieve this became widely available from the year 2000, and cases continually declined, but progress has plateaued since 2015. This review aims to cover the population-level prevention strategies responsible for this and those that could continue this progress, focussing on how they can be successfully integrated. Insecticide-treated nets (ITNs) made the most substantial contribution to reducing malaria mortality, but their distribution, access and use remains suboptimal while developm

Permethrin-treated baby wraps for the prevention of malaria: results of a randomized controlled pilot study in rural Uganda.

BACKGROUND: Progress against malaria has stalled and may even be slipping backwards in high-burden countries. This is due to a range of factors including insecticide resistance and mosquito feeding behaviours that limit contact with widely-employed interventions including long-lasting insecticidal nets and indoor-residual spraying. Thus, further innovations in malaria control are urgently needed. METHODS: The pilot was a randomized, placebo-controlled pilot study of permethrin-treated baby wraps-known locally as lesus-in children 6-18 months of age at a single site in rural western Uganda. An evidence synthesis approach for combining different data sources illustrated using entomological efficacy of insecticides for indoor residual spraying.

BACKGROUND: Prospective malaria public health interventions are initially tested for entomological impact using standardised experimental hut trials. In some cases, data are collated as aggregated counts of potential outcomes from mosquito feeding attempts given the presence of an insecticidal intervention. Comprehensive data i.e. full breakdowns of probable outcomes of mosquito feeding attempts, are more rarely available. Bayesian evidence synthesis is a framework that explicitly combines data sources to enable the joint estimation of parameters and their uncertainties. The aggregated and com

Community knowledge, perceptions, and practices regarding malaria and its control in Jabi Tehnan district, Amhara Region, Northwest Ethiopia.

BACKGROUND: Use of long-lasting insecticidal nets (LLINs), indoor residual spraying (IRS), community-based malaria education, prompt diagnosis and treatment are key programme components of malaria prevention and control in Ethiopia. However, the effectiveness of these interventions is often undermined by various challenges, including insecticide and drug resistance, the plasticity of malaria vectors feeding and biting behaviour, and certain household factors that lead to misuse and poor utilization of LLINs. The primary objective of this study was to document households' perceptions towards ma

An Experimental Hut Evaluation of PBO-Based and Pyrethroid-Only Nets against the Malaria Vector Anopheles funestus Reveals a Loss of Bed Nets Efficacy Associated with GSTe2 Metabolic Resistance.

Growing insecticide resistance in malaria vectors is threatening the effectiveness of insecticide-based interventions, including Long Lasting Insecticidal Nets (LLINs). However, the impact of metabolic resistance on the effectiveness of these tools remains poorly characterized. Using experimental hut trials and genotyping of a glutathione S-transferase resistance marker (L119F-GSTe2), we established that GST-mediated resistance is reducing the efficacy of LLINs against Anopheles funestus. Hut trials performed in Cameroon revealed that Piperonyl butoxide (PBO)-based nets induced a significantly

Effect of large-scale installation of deltamethrin-impregnated screens and curtains in Bam, a major focus of anthroponotic cutaneous leishmaniasis in Iran.

INTRODUCTION: Installation of deltamethrin-impregnated screens and curtains was assessed as a preventive measure against transmission of anthroponotic cutaneous leishmaniasis (ACL) by Phlebotomus sergenti in Bam, a well-known focus of ACL in Iran with a population of nearly 100,000. METHODS: This was a quasi-experiment based on official data from an ACL control program in which one section of the city with about one-quarter of the population received the intervention. In the analysis, the rest of the city was used as control. Data covered June 2007 through October 2010. RESULTS: Comparison o

Differential effects of dual and synergist-based insecticide-treated bed nets on pyrethroid resistance and L995F/S knockdown resistance mutation dynamics in Anopheles gambiae s.l. populations in south-western Burkina Faso.

BACKGROUND: The introduction of next-generation insecticide-treated nets (ITNs) in Burkina Faso aims to mitigate pyrethroid resistance in malaria vectors. This study evaluated the impact of different ITN types on phenotypic resistance and kdr mutation frequencies in Anopheles gambiae sensus lacto (s.l.) populations across three health districts over 3 years. METHODS: Annual mosquito collections were conducted in Banfora (where pyrethroid-chlorfenapyr nets had been distributed), Gaoua (pyrethroid-only ITNs) and Orodara (pyrethroid-piperonyl butoxide [PBO] ITNs). Two populations were analysed:

Insecticide resistance and behavioural adaptation as a response to long-lasting insecticidal net deployment in malaria vectors in the Cascades region of Burkina Faso.

The decline in malaria across Africa has been largely attributed to vector control using long-lasting insecticidal nets (LLINs). However, this intervention has prompted widespread insecticide resistance (IR) and been associated with changes in mosquito behaviour that reduce their contact with LLINs. The relative importance and rate at which IR and behavioural adaptations emerge are poorly understood. We conducted surveillance of mosquito behaviour and IR at 12 sites in Burkina Faso to assess the magnitude and temporal dynamics of insecticide, biting and resting behaviours in vectors in the 2-y

Long-lasting microbial larvicides for controlling insecticide resistant and outdoor transmitting vectors: a cost-effective supplement for malaria interventions.

The issues of pyrethroid resistance and outdoor malaria parasite transmission have prompted the WHO to call for the development and adoption of viable alternative vector control methods. Larval source management is one of the core malaria vector interventions recommended by the Ministry of Health in many African countries, but it is rarely implemented due to concerns on its cost-effectiveness. New long-lasting microbial larvicide can be a promising cost-effective supplement to current vector control and elimination methods because microbial larvicide uses killing mechanisms different from pyre

Assessing the role of interventions and climate on malaria mortality among children under five years of age: insights from two decades of data from the Health Demographic Surveillance System of Nouna, Burkina Faso.

BACKGROUND: Malaria is a preventable disease that causes serious illness and death. In 2022, it remained the leading cause of death among children under five years of age in Burkina Faso, despite significant intervention efforts over the past two decades. Research on the effects of interventions and climatic factors on malaria morbidity has expanded, but their effects on malaria mortality remain unclear. We aimed to estimate the effects of interventions and lagged climatic factors on malaria mortality among children under five years of age in northwest Burkina Faso. We further evaluated the ro

Delivery of malaria services during a pandemic: lessons from COVID-19 in Nigeria.

The coronavirus disease 2019 (COVID-19) pandemic posed significant threats to maintaining malaria services in Nigeria and threatened to reverse global progress towards elimination of the disease. During the COVID-19 pandemic, we worked in collaboration with the National Malaria Elimination Programme in Nigeria across 11 states to ensure that malaria campaigns and routine services continued. Here, we share the challenges and experiences from developing and implementing operational guidelines that enabled programmes to be adapted during unpredictable situations. The modifications made to long-la

Prediction of mosquito vector abundance for three species in the Anopheles gambiae complex.

BACKGROUND: The dynamics of vector-borne disease transmission depend on the abundances of vectors. The dominant malaria vector species complex of Anopheles gambiae sensu lato is a target of vector control strategies designed to reduce and eliminate malaria transmission. The three most widely distributed dominant malaria vectors within the species complex are An. arabiensis, An. coluzzii and An. gambiae sensu stricto. METHODS: Previous studies across the extent of the species complex range have been restricted to annual predictions of species occurrence or relative abundance. This study incorp

Ownership and use of insecticide-treated nets during pregnancy in sub-Saharan Africa: a review.

Over the past decade, significant gains have been made in the implementation of malaria prevention measures in pregnancy in sub-Saharan Africa, including the distribution of insecticide-treated nets (ITNs). These have been shown to cause a reduction in the incidence of malaria and its consequences such as maternal anaemia, stillbirths and intrauterine growth restriction. Currently most nations in Africa have policies for distributing ITNs to pregnant women through various mechanisms, however coverage remains well below the targets. This review summarizes recent evidence regarding the correlati

Population preference of net texture prior to bed net trial in Kala-Azar-endemic areas.

Prior to a community-based efficacy trial of long-lasting insecticidal nets (LLINs) in the prevention of visceral leishmaniasis (VL; also called kala-azar), a pilot study on preference of tools was held in endemic areas of India and Nepal in September 2005.LLINs made of polyester and polyethylene were distributed to 60 participants, who used the nets sequentially for 7 d. Acceptability and preference were evaluated via indirect indicators through questionnaires at three defined time points before and after use of the LLINs and through focus group discussions (FGDs). In the latter, preferences

Perceived Efficacy and Acceptability of a Spatial Repellent for Malaria Control in Western Kenya: Lessons from a Qualitative Social Science Study Conducted in Conjunction with an Epidemiological Efficacy Trial.

Kenya recorded more than 3 million malaria cases in 2023. Insecticide resistance and changing mosquito behavior have reduced the effectiveness of insecticide-treated nets (ITNs) and indoor residual spraying. Spatial repellents (SRs) show promise for malaria control. The present social science study, which is nested within the Advancing Evidence for Global Implementation of Spatial Repellents clinical trial in Kenya, was conducted to explore SR acceptability and perceived efficacy. Modified trials of improved practices were used to assess participant response to Mosquito Shield™, a transfluthri

Long-Lasting Insecticide-Treated Net Use Gaps and Severity Predictors in a Pre-Elimination Landscape: A Retrospective Observational Study in Mberengwa, Zimbabwe.

Although significant progress has been made in reducing malaria transmission in Zimbabwe, the path to elimination remains challenging. The disease remains a persistent threat, particularly in resource-constrained areas such as Mberengwa, necessitating an urgent need to understand the demographic, behavioural, socioeconomic, and structural factors influencing long-lasting insecticide-treated net use and case severity. This study investigated these factors using individual malaria case data to inform the development of locally tailored strategies for malaria elimination. Individual malaria case

Combining statistical and dynamical modelling to guide the design of cluster randomised trials for malaria.

Cluster randomised trials (CRTs) remain key for evaluating the community-wide impact of interventions against infectious diseases such as malaria. Randomising by cluster prevents contamination and enables both the direct and indirect effects of the intervention to be estimated. Although these trials are extremely informative, they can be logistically demanding and costly to carry out, which means it is important that these trials are well powered. Here, we present a framework for planning CRTs that measure malaria prevalence as the outcome using an established mathematical model of malaria tra

Knowledge, access and utilization of bed-nets among stable and seasonal migrants in an artemisinin resistance containment area of Myanmar.

BACKGROUND: Myanmar lies in the Greater Mekong sub-region of South-East Asia faced with the challenge of emerging resistance to artemisinin combination therapies (ACT). Migrant populations are more likely than others to spread ACT resistance. A vital intervention to reduce malaria transmission, resistance spread and eliminate malaria is the use of bed nets. Among seasonal and stable migrants in an artemisinin resistance containment region of Myanmar, we compared a) their household characteristics, b) contact with health workers and information material, and c) household knowledge, access and u

An Epidemiological Model of the Effects of Insecticide-Treated Bed Nets on Malaria Transmission.

Insecticide-treated bed nets (ITNs) have become a central tool for malaria control because they provide personal and community-wide protection through their repellent and insecticidal properties. Here we propose a model that allows to assess the relative importance of those two effects in different epidemiological contexts and we show that these two levels of protection may oppose each other. On the one hand, repellency offers personal protection to the users of ITNs. The repellent action, however, is a two-edged sword, for it diverts infectious mosquitoes to non-users, thereby increasing thei
